目的研究急诊护理流程在急诊颅脑外科手术患者管理中的实践效果。方法选取本院近年收治的急诊颅脑外科手术患者100例,随机分为急诊护理流程组和对照组各50例。对照组采用传统护理,急诊护理流程组应用急诊护理流程进行护理,比较两组患者生存率、恢复率、残废率、急救时间等护理指标。结果急诊护理流程组在生存率、恢复率、残废率、急救时间等方面显著优于对照组,差异有统计学意义(P<0.05)。结论对急诊颅脑外科手术患者应用急诊护理流程能够明显提高患者生存率和恢复率,降低残废率,缩短急救时间,提高患者生活质量,减轻其痛苦和经济负担,值得护理推广应用。

目的:寻找一种适合急诊护理工作特点的管理方法,提高管理效应。方法:成立质量控制小组,制订量化考核标准及考核细则,随时进行督促、检查、考核,定期评比总结。结果:量化考核实施后,护理差错、缺陷、投诉、纠纷明显减少,分诊准确率、服务满意度、危重患者抢救成功率明显提高。结论:护理质量量化考核克服了经验式管理的弊端,调动了护理人员的工作积极性,提高了护理质量。

Living forensics: a natural evolution in emergency care.

'Throughout history, health care professionals have been called upon to assist the legal system in the prosecution of cases where patient care overlaps with the law and physiological realities collide' (Lynch 1995). Working for the last 21 years in the Accident & Emergency setting, the author is dedicated to increasing the awareness and assisting in the establishment of basic forensic evidence collection guidelines for the emergency care provider. Due to the nature of the clientele and setting, emergency personnel will inevitably care for 'victims of violence'. Domestic violence injuries, abuse and neglect in the elderly and young, the addictive client seeking emergency care, the sexual assault victim, and sufferers of occupational injuries, are but a few of the cases that would be classified in the forensic arena. Holistic care dictates looking after the patient as a whole. The nurse must meet the patient's physical and psychosocial needs. The forensic nurse ensures that the patient's civil and constitutional rights are also met. This forensic health care role can be achieved and strengthened by recognizing potential evidence and maintaining a 'chain of custody' of this evidence.

Research directions for specialist practice

Emergency nursing is a specialist area of practice that demands particular knowledge and skills. This paper reports on a study to identify the research directions necessary to advance emergency nursing as a specialist area of practice. Nurses working in nine Australian hospital emergency departments responded to questions about current research practices, and the research priorities necessary to inform the development of knowledge and practice in the evolving specialty of emergency nursing. Forty-four core topics considered relevant to the knowledge base of emergency nursing were identified by nurses who were directly involved in providing patient care. The highest ranked topics were clustered into four priority areas for further research. The key areas for further research are education needs and opportunities, specialist roles of triage, trauma and practitioner, and nurses' coping mechanisms. Discussion of these topics is framed by the increasing specialisation of nursing practice, thus this paper aims to encourage consideration and debate about the research based development of the specialty of emergency nursing. © 2000 Harcourt Publishers Ltd.

The extent and nature of Emergency Nurse Practitioner services in Scotland

Emergency Nurse Practitioners (ENPs) are being used in an increasing proportion of A&E departments across England and Wales. This paper reports the findings of a postal survey sent to all (94) A&E departments in Scotland including the smaller GP run units. The aim of the study was to document the extent and nature of ENP services in Scotland. Nurses were found to be practising as ENPs in 47% of Scottish A&E departments. The majority (70%) of nurses practising as ENPs had been educated for the role on courses for ENPs. Nurses working as ENPs were being paid at all grades ranging from the lowest grade for a staff nurse (D-grade) through to H-grade. ENPs are practising in all types of A&E department. Most ENPs have been formally trained for the role, however huge variation exists in educational preparation and in remuneration for this expanded nursing role. © 2001 Elsevier Science Ltd. Managing thrombolysis in the accident and emergency department

The optimal management of acute myocardial infarction (AMI) today is founded upon the 'open artery' theory and driven by the need for early reperfusion strategies. Numerous randomised controlled trials demonstrate the unequivocal benefit of thrombolytic therapy in reducing mortality and improving long-term prognosis. These benefits are most striking when the thrombolytic therapy is given early. The National Service Framework for Coronary Heart Disease introduced national standards to reduce both 'call to needle' and 'door to needle' times to further augment early treatment. Subsequently many hospitals established 'fast track', 'nurse-led' or 'nurse initiated' thrombolysis within both the Accident and Emergency and Coronary Care Departments. This paper reviews these strategies and considers the benefits and limitations associated with the role of the 'thrombolysis nurse'. Thrombolysis nurses work to agreed protocols and play a fundamental role in the assessment and appropriate management of patients with acute MI. The evidence suggests that thrombolysis nurses are safe and effective in their practice and make a significant contribution to patient outcomes and enhance interprofessional education and practice. © 2002 Elsevier Science Ltd. The purpose of this paper is to describe in the broadest sense the scope of flight nursing practice in the Western Australian context. The Royal Flying Doctor Service - Western Operations provides aeromedical and associated health services, through primary health clinics to persons living in remote and rural areas of Western Australia. Flight nurses provide a significant contribution to fulfilling the core business of the Royal Flying Doctor Service, and in doing so demonstrate that they are able to combine the diversity and speciality that is flight nursing, with the challenges not only of the aviation environment, but of the diversity and remoteness of the physical environment that they may find themselves in. © 2000 Harcourt Publishers Ltd.

Patient satisfaction with a new nurse practitioner service

In the UK the role of the Nurse Practitioner (NP) has been slower to develop than in the USA (Tye 1997). However, since its inception, studies of the role have found positive results in areas of patient satisfaction (Touche Ross 1994). James and Pyrgos (1989) reported 94% of patients suitable to seen by a NP would use a NP if introduced. A study by Freij et al. (1996) concluded that emergency NPs were as competent at interpreting X-rays as casualty officers. Despite such evidence, Meek et al. (1995) noted opposition to this new advanced practice from other disciplines who wished to maintain the 'status quo'. In light of these findings, a study was undertaken to examine the new NP Service at Antrim Hospital, Northern Ireland. A patient satisfaction survey, a retrospective X-ray audit and a subjective assessment by other disciplines, were carried out in order to evaluate the service. © 2000 Harcourt Publishers Ltd.
