Assessing risk for depression among immigrants at two-year follow-up

This article reports findings from a longitudinal study of what predicts depression 2 years later in a sample of former Soviet immigrants. Study alms were to: (1) investigate change in depression scores and (2) determine the relative contribution of initial depression scores, select demographic and migration characteristics and common demands of immigration to depression at 2-year follow-up. Data were collected at two time points from 468 former Soviet immigrants who had been in the US 5 years or less at the time of the first data collection appointment. Linear regression was used to predict depression at 2-year follow-up. There was little improvement in depression for the sample as a whole over time. Baseline depression, unemployment, novelty, and not having relatives in the local resettlement area were significant predictors of depression at 2-year follow-up. Initial depression in relatively recent immigrants should be treated aggressively and not assumed to be a transient problem. Immigrants should be assisted to find early employment and overcome novel aspects of their new environment. Immigrants without local families will likely need additional support. The influence of background clinical data on infant pain assessments

The purpose of this study was to determine the importance of knowledge of clinical background data on nursing assessments of infant pain. In a quasiexperimental design, the infant pain assessments of two groups of pediatric nurses tuere compared. Both groups assessed the levels of pain of the same videotaped infants. One group also read clinical background data about each infant;the other group did not. The group who read clinical background data evidenced higher mean levels of assessed pain per videotaped infant than did the group who only viewed the videotapes. Findings underscore the importance of clinical data and clinical context in the process of assessing infant pain and the risk of underestimating pain when all factors are not considered. © 1999 Sage Publications, Inc.

HIV infection is a chronic illness that affects individuals of all ages. The pulmonary system is the most common body system affected by the disease. Management of these complications has improved over the years and has resulted in a decline in mortality. Critical care nurses should understand the complications that develop and be knowledgeable about the management to improve the patient's clinical outcomes. Assessment for complications must begin once an individual is diagnosed as being infected with HIV and continue throughout the course of the disease. Prevention is the key to limiting respiratory complications and thus improving survival rates.

The perioperative preparation of adolescents for surgery provides a challenge to nurses and other health care providers because of the wide diversity in adolescents' age, physical maturation, and cognitive and psychosocial development. Perioperative issues, informed consent, and assent differ considerably depending on the age and developmental level of each adolescent. This article provides information about adolescent growth and development and approaches to managing adolescent surgical patients.

Fluid balance assessment is a fundamental aspect of caring for critically ill patients who often have volume disturbances. Since the introduction of hemodynamic monitoring in the critical care setting decades ago, we have become more dependent on technology to assist us in evaluating a patient's fluid status and less skilled in basic physical examination and interpretation of common blood and urine values. Information obtained from these basic clinical skills is equally as, if not more, important as numbers derived by invasive means.

Changing clinical practice through research: The case of delirium

Promoting application of study ndings to the clinical setting is a constant challenge for nurse researchers. This project used change theory to include s tajfRNs in a research study on delirium and to use relevant fndings. The research hypothesis was: Staffnurses who use a standardized protocol lufll haue improved ability to Identify delirium tn elderly hospital patients. Stojf nurses on the intervention unit used this protocol to screen for delirium on all consenting tnpatients 75 years of age and over, and control unit nurses continued using their standard assessment practices. Intervention unit nurses demonstrated an improved ability to identify the presence and absence of delirium, and voluntarily requested to continue using the protocol after the study was terminated. Use of a theoretical model to include nurses tn the study promoted the successful conduct of the research and subsequent use of findings. © 1999 Sage Publications, Inc.

Care of the patient with hyperkalemia provides the critical care nurse with a challenging opportunity to use clinical knowledge, skills, and critical thinking. What is the cause of the hyperkalemia? Finally, how can subsequent episodes of hyperkalemia be prevented in this patient? Testing a model of the nursing assessment of infant pain

The purpose of this study was to test whether elements of an infant pain assessment model interacted as postulated by the model The elements are the infant's response to comfort measures and the principle of consolability. Four different scenarios for each of 16 videotaped infants were prepared. Each scenario represented one of four different combinations of likelihood of pain and consolability and consisted of a videotape plus written clinical information. Forty-eight volunteer pediatric nurses assessed infantpain of 16 scenarios, each depicting one of the 16 infants. Mean level of assessed pain was highest for the "high likelihood of pain and difficult to console" group, second highest for the "high likelihood of pain and easily consoled" group, third highestfor the "low likelihood of pain and difficult to console" group and least for the "low likelihood of pain and easily consoled" group. Findings supported the infant pain assessment model © 1999 Sage Publications. Inc.
