Rural Primary Care Program Survival: An Analysis of Financial Variables

This study examines the effect of financial characteristics of rural primary care programs on the probability of their continuing as federally funded entities. A randomly selected national cohort of rural primary care programs (n=162) was used to compare financial measures of programs that were continuing and those that were noncontinuing. Financial data were obtained from 1978‐1987 Bureau Common Reporting Requirements (BCRR) forms submitted to the Bureau of Health Care Delivery and Assistance of the Department of Health and Human Services as part of the requirement to receive federal grant support for the programs. The results emphasize the importance of both outside funding and increased level of self‐sufficiency in the continuation of rural primary care programs. Noncontinuing programs often suffer from both a lack of self‐sufficiency and a lack of outside funding, mostly from federal sources. To a lesser extent, the number of patients also affects the program's chance of continuation. From a policy perspective, government intervention is both necessary and likely if rural primary care programs are to succeed and fulfill their mission of providing primary care for the medically underserved who are primarily poor, uninsured, and unable to pay. PLANNING AN EVALUATION AND ESTIMATING ITS COST

Scientific evaluations can provide funders and program administrators with useful input concerning where to allocate scarce service dollars. But evaluation itself is legitimately the subject of cost concerns. What will it cost to get the various potential benefits of a scientific evaluation? What are the evaluation options of a given program, given its budget and the size and expertise of its staff? Where can a program administrator go for consulting help? This article provides a helpful framework for answering these important questions. First, it describes two types of evaluations which vary in both the questions they can answer and in their consequent cost. Second, it delineates and briefly describes the technical elements or steps required by each evaluation type. Third, it describes the nature and potential variability of costs associated with each technical step. Finally, it steers the reader to available sources of expert information and help.

Enhancing response rates to a smoking survey for enlisted US navy women

This study examines the effectiveness of a stepped approach for increasing response rates to a mailed follow-up smoking survey among newly enlisted women in the navy. The effect of the steppes approach on response rates and on the characteristics of respondents was evaluated. Also, costs were estimated for each of the steps to determine their relative benefits Results showed that the stepped approach was effective in more than doubling the survey response rate among smokers. Reluctant respondents did not differ from on-time respondents in of demographics or baseline smoking, although nonrespondents ware less educated and heavier smokers than on-time and reluctant respondents. Strategies documented here could well apply to survey efforts with other hard-to-reach populations.

This article provides an overview of evaluation in the Public Health Service (PHS) of the U.S. Department of Health and Human Services. The kinds of evaluation activities are outlined with illustrations of recently completed studies. The overview includes information on resources, policies, and management of evaluations conducted by the various PHS agencies and programs. In particular, the department's new efforts to better disseminate evaluation results through the Internet are described. The article concludes with a discussion of future prospects of how PHS evaluations will play a role in implementation of the Government Performance and Results Act (GPRA).

Using cost-effectiveness analysis to evaluate targeting strategies: The case of vitamin A supplementation

Given the demonstrated efficacy of vitamin A supplements in reducing childhoood mortality, health officials now have to decide whether ii: would be efficient to target the supplements to high risk children. Decisions about targeting are complex because they depend on a number of factors;the degree of clustering of preventable deaths, the cost of the intervention, the side-effects of the intervention, the cost of identifying the high risk group, and the accuracy of the 'diagnosis' of risk. A cost-effectiveness analysis was used in the Philippines to examine whether vitamin A supplements should be given universally to all children 6-59 months, targeted broadly to children suffering from mild, moderate, or severe malnutrition, or targeted narrowly to pre-schoolers with moderate and severe malnutrition. The first year average cost of the universal approach was US$67.21 per death averted compared to $144.12 and $257.20 for the broad and narrow targeting approaches respectively. When subjected to sensitivity analysis the conclusion about the most cost-effective strategy was robust to changes in underlying assumptions such as the efficacy of supplements, clustering of deaths, and toxicity. Targeting vitamin A supplements to high risk children is not an efficient use of resources. Based on the results of this cost-effectiveness analysis and a consideration of alternate strategies, it is apparent that vitamin A, like immunization, should be provided to all pre-schoolers in the developing world. Issues about targeting public health interventions can usefully be addressed by cost-effectiveness analysis.

Programme budgeting and marginal analysis: An approach to priority setting in need of refinement

Programme budgeting is enjoying a revival in the British National Health Service primarily because of its recent coupling with marginal analysis. As well as providing a means of mapping activity and expenditure from the institutions providing care to the population receiving it, programme budgeting and marginal analysis aim to assist strategic planning by indicating where net benefits might be increased by reallocating spending within programmes. However, the method to achieve this is not yet sufficiently refined and results to date have been mixed. In particular, the approach to identifying options for evaluation is flawed because of the importance attached to the role of ”expert' groups. A more systematic technique is suggested to overcome present methodological deficiencies. © 1996, SAGE Publications. All rights reserved.

The Dutch heart transplantation programme was subjected to a prospective economic evaluation and costs and effects with or without such programme were estimated. The no-programme estimates were derived from pretransplant patient data. Future projections of both options were based on micro-simulation using additional data on severe heart disease prevalence and on multi-organ donation. Costs per life year gained are estimated at NLG 57,650 (quality adjusted: NLG 71,900). Sensitivity analysis showed these results to depend highly on long term incidence of costs and on quality of life after transplantion.

Batch sampling to improve power in a community trial - Experience from the Pawtucket Heart Health Program

Experiments involving large social units, such as schools, work sites, or whole cities, are commonly limited in statistical power because the number of randomized units is small, leaving few degrees of freedom for residual (between-unit) error: The authors describe a method for increasing residual degrees of freedom in a community experiment without substantially increasing cost or difficulty. In brief they propose that the experimental units should be divided into random subsamples (batches). Batch sampling can improve statistical power if the community endpoint means are stable over time or if their temporal variation is comparable in period to the batch-sampling schedule. The authors demonstrate the theoretical advantages of the batch system and illustrate its use with data from the Pawtucket Heart Health Program in which such a design was implemented.

Costing medical libraries: the feasibility of functional cost analysis.

As a result of organizational changes in the National Health Service (NHS) there is a need for a coherent costing model for NHS libraries, particularly for the smaller libraries based in postgraduate medical centres. One possible model is functional cost analysis, whereby all costs are assigned to library functions representing services to users, such as loans from stock, interlibrary loans, enquiry services, etc. Using data from a 1991 survey of postgraduate centre-based libraries in the North West Thames (NWT) Region of the NHS, the feasibility and appropriateness of functional cost analysis is examined.
