Clinical support can make the difference in exclusive breastfeeding success among working women

Exclusive breastfeeding has generally been considered incompatible with working separated from the infant. This prospective, controlled intervention trial shows that breastfeeding support, including anticipatory counseling and monthly clinical follow-up of the mother and infant, can significantly increase the percentage of infants exclusively fed with breastmilk at the end of 6 months of life. Over 80 per cent of women from control and intervention groups expressed a desire to breastfeed for more than 6 months and more than 50 per cent thought it was best for the infant to be exclusively breastfed for 6 months. Only 6 per cent of women in the control group were able to complete 6 months of exclusive breastmilk feeding compared to 53 per cent of those in the intervention group. The most important difference between the strategies used by both groups of mothers for maintaining exclusive breastmilk feeding after returning to work was that only 23 per cent of the control group practiced milk expression compared to 66 per cent in the intervention group. All women from the supported group stated that they would advise a friend to combine exclusive breastfeeding and work and that they would like to do so again with another child.

Predictors of breastfeeding in very low birthweight infants at the time of discharge from hospital

In a case-control study carried out in the Kuala Lumpur Maternity Hospital between Ist July 1995 and 31st January 1996 the objectives were (1) to determine the rate of breastfeeding in surviving very low birthweight (VLBW, less than or equal to 1500 g) Malaysian infants following the introduction of the Baby Friendly Hospital Concept, and (2) to identify significant predictors associated with successful breastfeeding in these infants. During the study period, 201 (1.24 per cent) of live-born infants were VLBW infants, 192 (95.5 per cent) were Malaysians, and 141 (73.4 per cent) of them survived to go home. The breastfeeding rate among all surviving VLBW Malaysian infants at the time of discharge was 40.2 per cent (57/141), The mothers of 126 (89.4 per cent) VLBW Malaysian infants were interviewed before discharge. Logistic regression analysis showed that, after controlling for various confounders, the significant predictors associated with successful breastfeeding were: (a) Malay mothers (odds ratio: 6.0;95 per cent CI: 1.9, 19.4), (b) mothers with educational levels of between 7 and 9 years (odds ratio: 3.6;95 per cent CI: 1.0, 12.2), and (c) earlier age of commencement of enteral feeds in the VLBW infants (for each additional day delay in commencement of feeding, odds ratio of breastfeeding was 0.5;95 per cent CI: 0.4, 0.8).

A national survey of infant feeding in Asian families: summary of findings relevant to oral health

In 1994 the Department of Health commissioned a survey of early feeding practices within the Asian community. The survey conducted by the Office for National Statistics (ONS) was designed to assess from a nationally representative sample of infants from the Bangladeshi, Indian and Pakistani communities from across England their feeding practices from birth to 15 months. A small sample of White infants were also included in the sample although they cannot be considered as nationally representative of the majority White population in England. Data was collected from 2382 mothers in total. The survey was carried out between 1994 and 1996 and was published in 1997. This article will summarise the main findings relevant to oral health. These include patterns of breastfeeding, use of bottles, care and advice on infant feeding, weaning practices and drinks consumption. Implications for oral health promotion are considered.

The experience of sexuality during breastfeeding among primiparous women

The purpose of this study was to describe various aspects of sexuality for primiparous breastfeeding women. The conceptual framework used for the study was Bernhard's theory of women's integrated sexuality which describes female sexuality as a multidimensional, biopsychosocial phenomenon. The investigators used a descriptive design, analyzing data from the 576 primiparous breastfeeding women who, as part of a larger study, completed the Breastfeeding and Sexuality Tool at the time of complete weaning. The women were from a large, private hospital in urban Minnesota. Subjects completed initial questionnaires during the postpartum hospitalization. Follow-up data were collected by phone at I, 3, 6, and 12 months postpartum. Those who had not weaned by 12 months were followed every 3 months until complete weaning was reported. Overall, women perceived that breastfeeding had a slightly negative impact on the physiologic aspects of sexuality, but did not greatly affect the woman's sexual relationship with her partner. In addition, breastfeeding mothers perceived their partners' attitudes towards breastfeeding and sexuality as slightly positive, and did not worry that sexual activity would harm their milk supply or their ability to nurse. Overall, breastfeeding had a slightly negative impact on sexuality. However, a wide range of responses were reported by the women. Health care providers should be familiar with the whole range of possible responses to sexuality while breastfeeding in order to appropriately counsel women about what is normal and what to expect during this time. What influences the uptake and early cessation of breast feeding?

Objective: to examine obstetric, maternal and social factors associated with the uptake and early cessation of breast feeding and women's reasons for altering from breast to bottle feeding. Design: women who responded to a postal questionnaire on long-term postpartum health were contacted and asked to participate in a home-based interview, In addition to health problems, the interview obtained information on baby feeding and a number of social factors. Women were also asked to complete the Edinburgh Postnatal Depression Scale (EPDS), Obstetric and maternal data were obtained from maternity records. Setting: deliveries from a large maternity hospital in Birmingham. Participants: 906 women were interviewed at a mean of 45 weeks after delivery, Findings: 63% of the women said they had breast fed, but 40% of these stopped within three months of delivery, Many of the women gave physical problems with lactation as reasons for stopping. The factors found to be predictors of early cessation were: return to work within three months of birth;regular childcare support from other female relatives, and a high EPDS score. Non-initiation of breast feeding was predicted by a different set of factors: multiparity;general anaesthetic (GA);and unmarried status. Discussion and conclusion: despite evidence of the benefits of breast feeding, this remains an unacceptable long-term option for many women, and for over one-third it is never attempted. Factors within the woman's social environment were found to influence early cessation, Women who had a GA during or immediately following labour and delivery were less likely to initiate breast feeding, Implications for practice: if breast-feeding incidence and duration are to increase, more attention should be paid to establishing early, successful breast feeding and countering the negative influences of factors within the social environment.
