Neutrophils in galactose-fed dogs: Suppressed apoptosis and increased adhesion to retinal capillary endothelial cells

Dogs fed a diet containing 30% galactose develop diabetes-like retinal capillary changes. As retinal capillary occlusion is commonly observed in diabetic retinopathy, neutrophil apoptosis and the interaction of neutrophils with retinal capillary endothelial cells were investigated. Neutrophils were isolated with Ficoll-Hypaque centrifugation from dogs fed a 30% galactose diet and dogs fed a normal, control diet containing 30% non-nutrient filler. Apoptosis of neutrophils was microscopically examined after incubation at 37 degrees C for 3 hours with either 100 U/mL tumor necrosis factor alpha (TNF-alpha), 2 mu g/mL cycloheximide or 50 ng/mL phorbol 12-myristate 13-acetate (PMA). Neutrophil adhesion to dog retinal capillary endothelial cells was examined by counting the cells attached to the surface of endothelial cells after the incubation in the presence of either 100 U/mL TNF-alpha or 5 mu g/mL lipopolysaccharides (LPS) at 37 degrees C for 3 hours. With all three stimulants TNF-alpha, cycloheximide and PMA, the rate of apoptosis was significantly lower for neutrophils isolated from galactose-fed dogs compared to control dogs fed a normal diet. Preincubation of neutrophils from control dogs in medium containing 30% galactose for 3 hours did not affect the rate of apoptosis. Neutrophil adhesion to retinal capillary endothelial cells induced by incubation in the presence of either 100 U/mL TNF-alpha or 5 mu g/ml LPS was significantly higher with neutrophils isolated from galactose-fed dogs than those from control dogs. The data indicate that long-term galactose feeding is essential with development of various neutrophil dysfunctions. These neutrophil changes may contribute to the development of retinal microangiopathy associated with diabetes and galactosemia. (C) 1999 Elsevier Science Inc.

Pathogenesis of African horse sickness:: Ultrastructural study of the capillaries in experimental infection

African horse sickness (AHS) was induced in five horses by inoculation, to determine the ultrastructural changes in endothelial cells of capillaries in the myocardium, lung, spleen and liver. The animals developed cardiac and mixed forms of the disease. Alterations detected in the endothelial cells of the vessels of infected animals included: the presence of structures associated with viral infection, hypertrophy, degenerative changes, appearance of cytoplasmic projections, changes in permeability, alteration of intercellular junctions, loss of endothelium, subendothelial deposition of cell debris and fibrin, and vascular repair. In association with these changes, oedema, haemorrhages and microthromboses were detected, particularly in the myocardium and lung. This study showed that infection of, and changes to, the capillary endothelial cells of the organs under study was independent of the form in which the disease manifested itself but was dependent on the organ and blood vessel tl;pe Thus, different levels of viral tropism were observed for the endothelial cells of the vessels in different organs. Viral infection was commonest in the endothelial cells of myocardial vessels, followed by those in the lung, whereas in the spleen and liver, endothelial cell infection was rare and, in the case of the liver, limited to the interstitial capillaries. Vascular endothelial growth factor enhances glomerular capillary repair and accelerates resolution of experimentally induced glomerulonephritis

Vascular endothelial growth factor (VEGF) regulates angiogenesis through endothelial cell proliferation and plays an important role in capillary repair in damaged glomeruli. We tested the hypothesis that VEGF might be beneficial in rats with severe glomerular injury in glomerulonephritis (GN) based on its angiogenic and vascular remodeling properties. Acute GN with severe glomerular destruction was induced in rats by injection of anti-Thy-1.1 antibody (day 0) and Habu-snake venom (day 1). Rats were intraperitoneally injected with recombinant human VEGF(165). (10 mug/100 g body wt/day) or vehicle from day 2 to day 9, and monitored changes in glomerular capillaries, development of glomerular inflammation, and progression to glomerular sclerosis after acute glomerular destruction in both groups. Rats that received anti-Thy-1.1 antibody and Habu-snake venom showed severe mesangiolysis and marked destruction of capillary network on day 2. VEGF was expressed on glomerular epithelial cells, proliferating mesangial cells, and some infiltrating leukocytes, and VEGF(165) protein levels increased in damaged glomeruli during day 5 to day 7. Normal, damaged, and regenerating glomerular endothelial cells expressed VEGF receptor flk-1. However, endothelial cell proliferation and capillary repair was rare in vehicle-treated rats with severe glomerular damage, which progressed to global sclerosis and chronic renal failure by week 8. In contrast, in the VEGF-treated group, VEGF(165) significantly enhanced endothelial cell proliferation and capillary repair in glomeruli by day 9 (proliferating endothelial cells: VEGF(165), 4.3 +/- 1.1;control, 2.2 +/- 0.9 cells on day 7, P < 0.001;and glomerular capillaries: VEGF(165), 24.6 +/- 4.8;control, 16.9 +/- 3.4 capillaries on day 7, P < 0.01). Capillary density of skeletal muscle - A contributing mechanism for exercise intolerance in class II-III chronic heart failure independent of other peripheral alterations

OBJECTIVES The study was conducted to determine if the capillary density of skeletal muscle is a potential contributor to exercise intolerance in class II-III chronic heart failure (CHF). BACKGROUND Previous studies suggest that abnormalities in skeletal muscle histology, contractile protein content and enzymology contribute to exercise intolerance in CHF. METHODS The present study examined skeletal muscle biopsies from 22 male patients with CHF compared with 10 age-matched normal male control patients. Aerobic capacities, myosin heavy chain (MHC) isoforms, enzymes, and capillary density were measured. RESULTS The patients with CHF demonstrated a reduced peak oxygen consumption when compared to controls (15.0 +/- 2.5 vs. 19.8 +/- 5.0 ml . kg(-1). min(-1), p < 0.05). Using cell-specific antibodies to directly assess vascular density, there was a reduction in capillary density in CHF measured as the number of endothelial cells/fiber (1.42 +/- 0.28 vs. 1.74 +/- 0.35, p = 0.02). In CHF, capillary density was inversely related to maximal oxygen consumption (r = 0.479, p = 0.02). The MHC IIx isoform was found to be higher in patients with CHF versus normal subjects (28.5 +/- 13.6 vs. 19.5 +/- 9.4, p < 0.05). CONCLUSIONS There was a significant reduction in microvascular density in patients with CHF compared with the control group, without major differences in other usual histologic and biochemical aerobic markers. The inverse relationship with peak oxygen consumption seen in the CHF group suggests that a reduction in microvascular density of skeletal muscle may precede other skeletal muscle alterations and play a critical role in the exercise intolerance characteristic of patients with CHF. The clinical syndrome of recurrent macroscopic hematuria, unassociated with hypertension, edema or anatomical urinary tract abnormalities, is relatively common in children and young adults. Baehr [1] in 1926 described 14 young adults with a "benign and curable form of hemorrhagic nephritis", who appeared to be examples of this syndrome. The syndrome is characterized by recurrent episodes of hematuria which often begin at the height of an upper respiratory infection; one of the most striking clinical features of these patients. Typically, dark urine, red blood cell casts and hematuria appear the day of, or within two to three days of, the onset of the infection. Macroscopic hematuria rarely lasts longer than two to five days and is often accompanied by transient low grade ( < 1.0 g/24 hr) proteinuria. Microscopic hematuria without proteinuria usually persists between the attacks of macroscopic hematuria. Renal function is usually normal during the attacks, although an occasional patient has been described who has exhibited a transient reduction of renal function during the episode of gross hematuria. In addition to respiratory infection, a clear relationship between the onset of hematuria and strenuous exercise has been emphasized by several reports [2-3]. In spite of multiple attacks of hematuria, renal failure has been rarely observed in the reported series followed three to five years; hence, the terms "benign" and "asymptomatic" have often been applied to these patients.

HYPERACUTE REJECTION OF GUINEA-PIG TO RAT CARDIAC XENOGRAFTS .1. MORPHOLOGY

The morphological changes occurring in xenograft hyperacute rejection, using guinea-pig to rat cardiac transplants, have been studied. Ultrastructural evidence of platelet aggregation in myocardial capillaries and veins first occurred two minutes after revascularisation. Small amounts of fibrin, associated with platelet degranulation occurred at three minutes. From four minutes, platelet aggregates increased in number and endothelial damage first appeared in those vessels containing platelet aggregates. Rejection was complete by 12 minutes and platelet aggregates then filled many capillaries, veins, arterioles and arteries. Polymorphonuclear leucocyte infiltration was not seen.

Microvascular dysfunction of skin vessels is well known in diabetes (Mever and Schatz. 1998). whereasonly minor alterations of the morphological microvas-cular pattern were revealed by capillaroscopy without dyes. Capillary density was described to be reduced in type 1 diabetic patients (Gasser and Berger. 1992) or unchanged in patients with type 1 (Trapp ***- 1986) or 2 diabetes (Pazos-Moura et al.,1990). An enlargement of capillary loop width (Mahler et al, 1987) and an increased degree of capillary tortuosity (Pazos-Moura et al, 1990; Kiesewetter et al, 1986) wore observed in patients with type 1 and 2 diabetes A widened arterial capillary limb was seen in type i diabetic patients with retinopathy (Gasser and Berper. 1992). but not in type 2 diabetes (Pazos-Moura et al. 1990). Alterations at the capillary apex in types i (Zaugg-Vesti et al, 1995) and 2 diabetic patients (Pazes-Moura et al, 1990) were depicted as aneurysms or nodular elongations (Y-pattems).

This report describes a 7-year-old boy who underwent successful surgical excision of a saccular aneurysm of the ascending aorta caused by giant cell aortitis. The aneurysm was removed by lateral aortorrhaphy without cardiopulmonary bypass. The clinical manifestations and appropriate surgical therapy of granulomatous aortitis in childhood are discussed.

Cutaneous collagenous vasculopathy with generalized telangiectasia: an immunohistochemical and ultrastructural study

We report a 54-year-old male, with a 5-year history of spreading asymptomatic generalized cutaneous telangiectases. The patient had no mucosal or nail involvement, no positive family history and no clinical evidence of systemic disease or bleeding diathesis, Histologically, the superficial small dermal blood vessels were dilated and showed thickened walls with hyaline perivascular material, staining as collagen. The vessel walls were PAS and colloidal iron stain positive, and immuno-histochemically lacked actin staining. Collagen IV;fibronectin and laminin antibodies showed the material deposited around the basement membrane zone. Ultrastructurally, the vessels were post-capillary venules (PCV) and showed marked collagen deposition around the basal lamina, There were many abnormally banded widely spaced fibres with 100-150 nm periodicity (Luse bodies), in addition to regular banded collagen. Pericytes were sparse and lacked intracytoplasmic filaments, and few veil or fibroblastic cells were seen embedded within the collagen. We believe this is a form of cutaneous microangiopathy not previously described, with distinct morphology and unique ultrastructural features. It may be due to a genetic defect with erroneous production of disorganized collagen in the cutaneous microvasculature, Dermatologists and Dermatopathologists should be aware of this unusual cutaneous vasculopathy.
