Analysis of prevalence and trends of dental caries in the Americas between the 1970s and 1990s

Data on the prevalence and severity of dental caries collected by country members of the Pan American Health Organisation (PAHO) were summarised to analyse current status and trends since the 1970s. The mean number of decayed, missing, and filled teeth (DMF-T) among 12-year-old children and the relative contribution of each DMF-T component were collected from official reports and publications in the scientific literature. Overall, a secular trend toward lower caries prevalence was observed in most countries, more notably among those with large prevention programmes using fluorides. Many countries have reached the World Health Organization (WHO) year 2000 goal of a mean DMF-T of less than or equal to 3 but others are still far from reaching that goal. Few countries have reached the status of having large proportions of disease prevalence localised in a small percentage of the population, a pattern observed as prevalence decreases. Since 1994, PAHO's Regional Oral Health Program has developed two strategies to address these issues: the introduction and reinforcement of national preventive programmes using fluorides and the introduction of the atraumatic restorative treatment (ART).

Root lesions in a group of 50-60 year-old Germans related to clinical and social factors.

From a preventive point of view collection of data concerning carious and non-carious cervical tooth defects is definitely important. Consequently, the prevalence and distribution of different root lesions were studied and correlated with behavioral and biological factors in 50- to 60-year-old German individuals (n = 298). Additionally, the data were correlated with characteristics concerning oral health and known risk factors such as gender, educational level, and presence of plaque. An interview included questions on sociodemographic and socioeconomic characteristics, dental and general health status, and various behavioral parameters. During clinical examination data concerning coronal and root lesions, restorations, probing depth, gingival bleeding, and dental plaque were obtained. The participants represented a social middle class population with a high awareness of dental health. Obviously, for the participants, known risk factors for root decay such as gender, educational level and plaque index were of minor importance. Factors correlating with root caries were: (a) number of missing teeth, (b) probing depth, (c) smoking habit, (d) regular dental attendance and (e) the reason for the last dental treatment. Additionally, the prevalence of non cariogenic lesions, primarily resulting from increased but wrongly performed oral self care, seems gradually to relieve carious root destruction.

Future needs for dental restorative materials.

While the population of the United States increases from 250 to 310 million people, the number in older age groups will increase dramatically from 28 million to about 64 million. Tooth retention has improved remarkably in the 65-74 age groups, from 7.4 in 1962 to 17.9 in 1986. While younger age groups will require less treatment due to decline in dental caries, older age groups appear to require more treatment than did similar age cohorts in previous generations. Hence, the need for restorative procedures by the United States population will be on an upward trend for the next decade or two.

The impact of the ART approach on the treatment profile in a Mobile Dental System (MDS) in South Africa

The changing profile of oral care rendered through the Mobile Dental System (MDS), after the introduction of the Atraumatic Restorative Treatment (ART) approach is described. During the first year of introduction of ART, the percentages of amalgam restorations and tooth extractions decreased significantly (P<0.0001). This is partly ascribed to a change in choice of treatment by dental operators in favour of ART and also due to an increase in acceptance by patients because of the reduced fear, and the patient-friendly nature of the ART approach. The reduction in amalgam restorations was 16.0 per cent for permanent and 1.4 per cent for primary posterior teeth. Extraction of posterior teeth was reduced by 17.4 per cent in the permanent and 35.7 per cent in the primary dentitions. The restorative component of oral care increased by 33.4 per cent in the permanent and 37.1 per cent in primary posterior teeth. The one-year survival of one-surface ART restorations using Fuji IX and KetacMolar was 93.6 per tent. Full and partial (more than 90 per cent) retention of the sealant part of the ART restoration was obtained in 75 per cent of the cases after one-year. During the one-year period, infection control was made more simple and this facilitated easier maintenance of mobile dental equipment. The introduction of the ART approach reduced extraction, restored more teeth and made oral care in the MDS more preventive, less threatening and thus more patient-friendly.

Caries prevalence in 12-year-old schoolchildren in Senegal in 1989 and 1994

Senegal, in middlewest Africa, has an estimated area of 197,000 km(2) with a population of about 8.3 million of whom 45 per cent are less than 14-years of age. Apart from densely populated areas around the capital, Dakar, and other large towns, the population is scattered sparsely in rural areas. In the absence of any recent and representative oral health data, two cross-sectional studies on caries prevalence were conducted in 1989 and 1994 respectively to assess and compare the caries status among 12-year-old schoolchildren living in the urban and rural areas of Senegal. The results of the study are presented in this paper.

Trends in total caries experience: Permanent and primary teeth

Background. This article is the last in a series of three that focuses on recent changes in the caries status of children aged 18 years or younger in the United States. Methods. This study is based on analyses of data regarding total carious (treated and untreated) permanent and primary teeth among children 6 to 18 years old and children 2 to 10 years old from the first and third National Health and Nutrition Examination Surveys, or NHANES I and NHANES III. The NHANES is periodically conducted by the National Center for Health Statistics of the Centers for Disease Control and Prevention. Results. The cumulative number of carious permanent teeth, both treated and untreated, among 6- to 18-year-olds decreased 57.2 percent, from 4.44, as measured in NHANES I, to 1.90, as measured in NHANES III. The cumulative number of carious primary teeth, both treated and untreated, among 2- to 10-year-olds decreased 39.7 percent, from 2.29, as measured in NHANES I, to 1.38, as measured in NHANES III. Conclusions. Since the 1970s, the cumulative number of carious permanent and primary teeth, both treated and untreated, has declined substantially among children in the United States. Practice Implications. Effective prevention has reduced caries in children. As a result, dental practice will be more focused on maintaining intact dentitions than on repairing teeth damaged by disease.

Clinical performance of bonded amalgam restorations at 42 months

Background, Despite a lack of data based on clinical research, many positive characteristics have been attributed to the placement of amalgam restorations with an adhesive resin liner. Methods, For 42 months, the authors followed two groups of subjects who had amalgam restorations placed in a previous study. In this double-blind study, these subjects had been randomly assigned to have amalgam restorations placed with an adhesive liner or with a copal varnish placed under all restorations and a bulk base of zinc phosphate cement for deeper lesions. The authors evaluated anatomical form, marginal adaptation, retention and the presence of secondary caries at six, 18, 30 and 42 months. Results, At 42 months, the authors found that all restorations in both groups still were retained, were free of secondary caries and were rated clinically acceptable. No difference between the groups was found for any category (P > .05;analysis). Conclusions, Placement of amalgam restorations with adhesive liners was found to produce results equivalent to that of traditional methods over a 42-month period. Clinical Implications, Practitioners wary of using new methods that have not undergone thorough clinical testing can feel comfortable placing adhesive liners under amalgam restorations.

Efficacy of preformed metal crowns vs. amalgam restorations in primary molars: A systematic review

Background. The authors evaluated the treatment efficacy of preformed metal crowns, or PMCs, vs. amalgam restorations in primary molars by means of a literature review and meta-analysis. Types of Studies Reviewed. From a literature search, the authors selected clinical studies that evaluated treatment with PMCs vs. amalgam control restorations in primary molars and provided data against which treatment outcomes could be compared. Results. Ten studies with durations ranging from 1.6 to 10 years fulfilled the selection criteria. Their failure rates, based on need for subsequent treatment or retention of the restoration at final evaluation, ranged from 1.9 to 30.3 percent for PMCs and 11.6 to 88.7 percent for amalgam restorations. Overall, PMCs demonstrated greater longevity and reduced retreatment need compared with amalgam control restorations. The odds ratio for all studies fell within the boundary favoring treatment with PMCs. Clinical Implications. Analysis of the literature, though mainly retrospective studies, demonstrated evidence of a more favorable outcome for PMCs than for amalgam restorations in primary molars requiring multisurface restorations.

Use of discriminant and regression analyses to modify a clinical certification board examination.

The National Dental Examining Board of Canada (NDEB) conducts mandatory, high stakes, pass/fail, certification examinations for dental licensure. One of these examinations was a seven-part, simulated clinical examination in which candidates were required to perform procedures on typodonts. These requirements were two intracoronal and two extracoronal preparations, an amalgam restoration, a provisional crown, and a diagnostic wax-up. Feedback from candidates and examiners indicated that one or more of the requirements may not have been contributing effectively to the overall evaluation of candidates. The NDEB's Clinical Examination Committee therefore requested that an in-depth statistical analysis be performed to identify potential areas of concern and to provide a basis for modifying the examinations. The results of two examination sessions with a total of 168 candidates were subjected to both a discriminant and a logistic regression analysis. Every candidate had results for each of the seven requirements, and no candidate participated in both sessions of the examination. The discriminant analysis revealed that six of the seven requirements could be used to reliably assign examinees according to their true pass/fail classifications. Stepwise discriminant analysis resulted in a 98.81 percent classification success rate with a corresponding 2.50 percent false-positive classification error rate. The logistic regression analysis showed that five components correctly predicted 99.40 percent with a 1.25 percent false-positive rate. The Clinical Examination Committee concluded that one requirement (diagnostic wax-up) should be eliminated and that a second requirement (PFM preparation) be significantly modified and reevaluated. This study demonstrates the usefulness of statistical methods in the analysis and modification of a clinical certification board examination.
