Morphology of initial lesions of enamel treated with different commercial dentifrices using a pH cycling model: Scanning Electron Microscopy observations

The aim of this study was to evaluate and compare the 'calcium fluoride-like reaction products' formed, both on the surface and in the subsurface caries-like lesions of enamel, from different commercial dentifrices manufactured locally from developed and developing countries. The experiment was conducted by using the in vitro single-section technique under the pH-cycling system used for 10 days. The tested dentifrices include;Maxam DFP(R) Maxam Tartar Controls, First(R), Tianqui Medicated(R) (China);Vicco Vajradenti(R) (India);Colgate MFP2(R). Crest Tartar Control (USA);and a non-fluoride dentifrice, Jie Yin (China) used as control. The surface and subsurface of the lesions, before and after treatment, were examined using scanning electron microscopy. Varying degrees of fine globular patterns of calcium fluoride-like material were observed on the enamel surfaces of five out of eight groups and in the subsurface of four groups. Two treatment groups showed a 'similar appearance' compared with the control and untreated groups. This study suggests and supports that, when compared with 'multinational dentifrices', some Chinese and Indian dentifrices manufactured locally failed to induce the typical morphological appearance of globules as seen with fluoride dentifrices.

AN INVIVO STUDY OF MICROHARDNESS AND FLUORIDE UPTAKE IN PARTIALLY DEMINERALIZED HUMAN-ENAMEL COVERED BY PLAQUE

In this investigation, microhardness changes in partially demineralized human enamel were studied after in vivo use of fluoridated toothpaste systems. Flattened enamel specimens were demineralized in vitro and subsequently postioned in approximal positions in the prostheses of 27 participants for three weeks;the samples were plaque-covered in vivo. After brushing for one week with a non-fluoridated paste to achieve an in vivo equilibrium, participants brushed with an assigned product for a two-week period. This test was a six-way cross-over design which used randomization of subjects. Five fluoridated and one non-fluoridated paste were tested. Knoop hardness measurements were carried out on sound and on in vitro demineralized enamel, and after in vivo exposure of the enamel to the dentifrice treatments. The results showed that during the two weeks of in vivo exposure to the fluoride products, net rehardening of the demineralized enamel did not occur, and no correlation was observed between fluoride uptake into the demineralized enamel and changes in microhardness. That we failed to observe rehardening may be due to the fact that the duration of this study was too short for any net remineralization to have occurred, especially because the samples were constantly covered with plaque. Other possibilities, such as the type and/or severity of the lesions used in this study, may account for the lack of rehardening.

THE USE OF SATURATED DCPD IN REMINERALIZATION OF ARTIFICIAL CARIES LESIONS INVITRO

Dicalcium phosphate dihydrate (DCPD) may play a significant role in the caries lesion since it is a stable calcium phosphate phase under acidic conditions. The reaction of DCPD and fluoride, forming fluor-apatite (FAP), may provide a potentially promising treatment regimen for remineralization of caries lesions in vivo. The purpose of this study was to determine whether a two-step DCPD and inorganic wash with fluoride can remineralize artificial caries-like lesions in vitro. We used the single-section technique to facilitate quantitation of the same tissue before and after the experimental regimen. The two-step remineralizing treatment was repeated three times and consisted of a two-minute saturated DCPD treatment (pH 2.1) followed by a 24-hour inorganic wash. Lesion parameters were recorded before and after treatment by the taking of polarized light photomicrographs of each section after imbibition in several media. The changes in the tissue following treatment were expressed as a percent change in the area of the initial pre-treatment lesion. Significant reductions (p < 0. 02) in lesion pore volume were observed in all aqueous media examined. In the lesions after imbibition in quinoline, remineralization was also apparent from the significant increase in the area of the dark zone following treatment. This two-step DCPD treatment appears to remineralize artificial caries-like lesions effectively, but additional work is needed to determine whether it affords any protection against subsequent cariogenic challenges.

COMPARISON OF THE EFFECTS OF FLUORIDE AND THE IONOPHORE NIGERICIN ON ACID PRODUCTION BY STREPTOCOCCUS-MUTANS AND THE RESULTANT INVITRO ENAMEL DEMINERALIZATION

In an in vitro demineralization experiment with a plaque of S. mutans C180-2 over enamel, the effect of 0.5 mmol fluoride/L was compared with that of 10 nmol nigericin/L. Since the effects of both substances on the acid production in a dense cell suspension were of the same magnitude, and since nigericin did not affect the enamel solubility, the present demineralization experiment may differentiate between the effect of fluoride on bacterial metabolism alone and the combined effect of fluoride on both the bacterial metabolism and enamel solubility. In our experiments, the effect on bacterial metabolism accounted for 75% of the caries-inhibiting effect of *** data showed subsurface lesions in the control and nigericin group and a slight surface-softening in the fluoride group. In addition, the effects of fluoride and nigericin on the cellular internal pH during glycolysis were studied. Analysis of these data suggested that glycolysis was inhibited when the intracellular pH reached about pH 5.2, which supports the view that acidification of the cell cytoplasm is a mechanism by which fluoride inhibits glycolysis.

A scanning electron microscopical study of normal and fluorotic enamel demineralized by EDTA.

Normal and fluorotic primary and permanent teeth were demineralized in 10% EDTA at pH 7 for varying periods of time up to 4 weeks. The fluorotic teeth initially dissolved at a much slower rate than non-fluorotic specimens and appeared to contain more organic material. After almost complete removal of the outer enamel a soft organic layer remained on the dentin surface. This consisted of a fine fibrillar mesh and some more dense material. After four weeks much of the organic material had disappeared leaving only a thin membrane on the dentin surface.

Extracted human third molars were treated with a solution of titanium tetrafluoride. Calcium, phosphorus and fluoride microanalysis showed a high fluoride uptake and reduced enamel solubility for the treated surfaces. Scanning electron microscope examination showed the presence of a coating that was only partially removed by treatment with perchloric acid.

The morphological appearance of white spot enamel after treatment with various calcium phosphate and sodium fluoride solutions was examined using the scanning electron microscope. The reaction products formed after the sequential application of the calcifying solutions and a high concentration of fluoride appeared to fill in the porosity created in the enamel surface by white spot formation.
