A sensate liner for personnel monitoring applications

This program develops and demonstrates technologies useful for implementing a manageable cost effective systems approach to monitoring the medical condition of personnel by way of an instrumented uniform hereafter referred to as a Sensate Liner (SL). The SL consists of a form fitting garment which contains and interconnects sensing elements and devices to an electronics pack containing a processor and transmitter The SL prototype requires fiber, textile, garment and sensor development. The SL textile consists of a mesh of electrically and optically conductive fibers integrated into the normal structure (woven or knitted) of fibers and yarns selected for comfort and durability. A suite of SL garment compatible embedded biological and physical sensors ore then integrated into the SL. The initial SL sensor suite is selected to improve triage for combat casualties. CONTINUOUS MONITORING INSTRUMENT FOR REACTIVE HYDROCARBONS IN AMBIENT AIR

This modification of a total hydrocarbon analyzer made It capable of simultaneously monitoring ambient air for reactive hydrocarbons with the potential for producing photochemical smog. Metered sample air flows were continuously pumped through an empty column and an absorbent column arranged In parallel. The air stream exiting from each column was directed alternately in a 5-minute cycle by a switching valve to the flame Ionization detector for quantitative measurements. A chromium trioxide-sulfuric acid packing was optimal to absorb olefinic and higher aromatic hydrocarbons, and suitable column dimensions, flow rates, and temperatures were determined to obtain desired separations. Reactive hydrocarbon concentrations, obtained by difference from the instrument recordings, are reported for Cincinnati. These measurements should be more relevant than those of total or non-methane hydrocarbons. © 1975, American Chemical Society. All rights reserved.

An evaluation of an intelligent home monitoring system

A trial was performed of an intelligent monitoring system which used sensors in the home to identify emergencies by detecting deviations from normal activity patterns. The field trial lasted three months. Twenty-two elderly people agreed to participate. Their ages ranged from early 60s to over 85, with two-thirds in the age range 75-84 years. They lived in four different localities within the UK-Ipswich, Northumberland, Merseyside and Nottingham. A total of 61 alerts was recorded, at a mean frequency about one alert per month per client. Of the 61 alerts generated, 46 were classified as false alerts and the other 15 as genuine, although no real emergencies occurred during the study. Many people in the field trial reported enhanced feelings of safety and security, which could help to stimulate independence and help them to remain living in their own homes. The monitoring system increased the care choices available to elderly people and supported and enhanced the carer's role.

NEW NONINVASIVE TECHNIQUES FOR CREW HEALTH MONITORING IN SPACE

The present paper discusses some of the available non-invasive methods that are currently being evaluated at the University of Sheffield. Applied potential tomography (APT) has considerable potential as an imaging system (albeit of low resolution), which can be readily transferable to the space environment and provide information on fluid shifts on insertion into, and subsequently adaptation to, microgravity. Modern biochemical methods for the analysis of urine can provide information on many aspects of metabolic activity. The monitoring of bone formation and resorption is particularly relevant to space. The bodies handling of food can additionally be investigated by monitoring non-invasively the transit of contents through the gastrointestinal tract. These and other non-invasive monitoring techniques offers to move space medicine closer to clinical observation systems used on earth.

DEVELOPMENT OF A GAS CHROMATOGRAPHIC-ULTRAVIOLET ABSORPTION SPECTROMETRIC METHOD FOR MONITORING PETROLEUM PITCH VOLATILES IN ENVIRONMENT

A procedure employing the techniques of gas chromatography (GC) and ultraviolet absorption spectroscopy (UV) has been developed for the specific analysis of volatiles from petroleum pitch. These techniques have been used In conjunction with mass spectrometry to identify nine of the major volatile compounds in petroleum pitch. In contrast to the major compounds present in coal tar pitch volatiles, many of the identified compounds in petroleum pitch volatiles were methyl substituted aromatics, and Included: methylchrysene, dlmethylchrysene, methylphenanthrene and methylpyrene. These compounds together with five other identified aromatic hydrocarbons were used in a combined GC-UV method for monitoring petroleum pitch volatiles. This analytical method, which showed a detection limit of approximately 0.5 μg per compound, was tested by analysis of commercial petroleum pitches. © 1975, American Chemical Society. Use of cardiopulmonary exercise testing with hemodynamic monitoring in the prognostic assessment of ambulatory patients with chronic heart failure

OBJECTIVES We studied whether direct assessment of the hemodynamic response to exercise could improve the prognostic evaluation of patients with heart failure (HF) and identify those in whom the main cause of the reduced functional capacity is related to extracardiac factors. BACKGROUND Peak exercise oxygen consumption (VO2) is one of the main prognostic variables in patients with HF, but it is influenced also by many extracardiac factors. METHODS Bicycle cardiopulmonary exercise testing with hemodynamic monitoring was performed, in addition to clinical evaluation and radionuclide ventriculography, in 219 consecutive patients with chronic HF (left ventricular ejection fraction, 22 +/- 7%;peak VO2, 14.2 +/- 4.4 ml/kg/min). RESULTS During a follow-up of 19 +/- 25 months, 32 patients died and 6 underwent urgent transplantation with a 71% cumulative major event-free a-year survival. Peak exercise stroke work index (SWI) was the most powerful prognostic variable selected by Cox multivariate analysis, followed by serum sodium and left ventricular ejection fraction, for one-year survival, and peak VO2 and serum sodium for two-year survival. Two-year survival was 54% in the patients with peak exercise SWI less than or equal to 30 g m/m(2) versus 91% in those with a SWI >30 g.m/m(2) (p < 0.0001). A significant percentage of patients (41%) had a normal cardiac output response to exercise with an excellent two-year survival (87% vs. 58% in the others) despite a relatively low peak VO2 (15.1 +/- 4.7 ml/kg/min). CONCLUSIONS Direct assessment of exercise hemodynamics in patients with HF provides additive independent prognostic information, compared to traditional noninvasive data. Study objective: The rhythm detection algorithms of automated external defibrillators have been derived from adult rhythms, and their ability to discriminate between shockable and nonshockable rhythms in children is largely unknown. This study evaluates the performance of 1 automated external defibrillator algorithm in infants and children and evaluates algorithm performance with anterior-posterior versus sternal-apex lead placement. Methods: We enrolled pediatric patients in a critical care unit, an electrophysiology laboratory, and a cardiac operating room. A monitor-defibrillator recorded ENS by means of standard defibrillation-monitor pads. Selected 15-second rhythm samples were played into a LIFEPAK 500 automated external defibrillator, and the automated external defibrillator "shock/no shock" decision was documented. To determine sensitivity and specificity, the automated external defibrillator decision was compared with the "shockable" versus "nonshockable" rhythm classification provided by 3 expert clinicians who were blinded to the automated external defibrillator decision. Results: We recorded 1,561 rhythm samples from 203 pediatric patients (median age 11 months;range, day of birth to 7 years). The automated external defibrillator recommended a shock for 72 of 73 rhythm samples classified as coarse ventricular fibrillation by expert review (sensitivity 99%;95% confidence interval [CI] 93% to 100%);and correctly reached a "no shock advised" decision for 1,465 of 1,472 rhythm samples classified as nonshockable by experts (specificity 99.5%). Specificity was 99.1% (95% CI 97.8% to 99.8%) with the sternal-apex lead and 99.4% (95% CI 98.1% to 99.9%) with the anterior-posterior lead. Conclusion: This automated external defibrillator algorithm has high specificity and sensitivity when used in infants and children with either sternal-apex or anterior-posterior lead placement.

ARTIFACTS IN PORTABLE ELECTROCARDIOGRAPHIC MONITORING

The development of portable ECG monitoring techniques has brought new insights into the electrical activity of the heart, and new problems in interpretation of artifacts. This article summarizes and classifies 15 different types of artifacts observed from dynamic ECG. The artifacts appear partly as pseudo-arrhythmias mimicking supraventricular, ventricular, junctional and dissociative rhythms. There are also non-arrhythmic artifacts which can be misleading in the interpretation of Q-waves, S-T-segments and T-waves. Of the 15 artifacts 8 have potentially serious consequences if not understood, and in 2 instances an artifact almost led to the unnecessary implantation of an artificial cardiac pacemaker.
