Cisplatin-paclitaxel-cyclophosphamide with G-CSF in primary advanced epithelial ovarian cancer

Introduction. In an attempt to increase survival, we performed a prospective trial of high-dose cisplatin-paclitaxel-cyclophosphamide with granulocyte colony-stimulating factor (G-CSF) for three cycles followed by carboplatin-paclitaxel for three cycles after cytoreduction of primary advanced epithelial ovarian cancer, Materials and methods. Thirty consecutive women with Stage 3 or 4 invasive primary epithelial ovarian cancer were treated with cytoreductive surgery. Postoperatively patients received 100 mg/m(2) of cisplatin, 200 mg/m(2) of paclitaxel, and 500 mg/m(2) of cyclophosphamide IV q 21 days x 3 cycles with 300 mu g of G-CSF daily x 5 beginning the first day following chemotherapy. This was followed by carboplatin AUC-5 and 135 mg/m(2) of paclitaxel IV q 21 days x 3. All administration was outpatient and paclitaxel was administered over 3 h, Results. Eighty percent of tumors were Stage 3C, 77% were serous, and 60% were Grade 3, Maximum cytoreduction to <2 cm was performed in 96%. Median follow-up is 30 months. Sixty-three percent of patients developed recurrence. Currently 50% of patients are alive with no evidence of disease. Estimated mean survival is 61 months and estimated mean progression-free survival is 29 months. No patient developed thrombocytopenia, neutropenic sepsis, significant neuropathy, or renal toxicity. Conclusion. This treatment regimen resulted in minimal toxicity and, following aggressive cytoreduction, produced good progression-free and overall survival. (C) 2000 Academic Press.

Low rate of emergence of nevirapine and lamivudine resistance after post-partum interruption of a triple-drug regimen

Introduction: Emergence of nevirapine (NVP) resistance may be a consequence of its use in monotherapy to prevent HIV mother-to-child transmission (MTCT). The aim of this study was to evaluate the emergence of strains resistant to NVP and lamivudine (3TC) after discontinuation of antiretroviral therapy (ART) with 3TC/zidovudine (ZDV)/NVP. Methods: Twenty pregnant women (ART-naive or pre-exposed only to ZDV), to whom 3TC/ZDV/NVP was prescribed as MTCT prophylaxis, were studied. They received ART for a median of 4 months with median viral load (VL) at labour <50 copies/ml. Samples were collected between I and 15 months (median: 3 months) after ART interruption. Sequence-selective real-time PCIR (SPCR), which quantifies minority viral populations containing K103N, Y181C and M184V mutations, and standard genotypic sequencing were assayed. Results: No mutations associated with resistance to 3TC or NVP were found by standard population sequencing. Analysis of K103N by SPCR showed that 35% of the patients contained <= 0.11% of viruses carrying either the AAC or AAT mutations. For Y181C mutation, 10% of the patients contained <0.5% of viruses with TGT codon change. For M184V mutation, one patient contained 6.2% of virus with GTG mutation and 13 patients (65%) contained <0.9% of mutated viruses. Four women were re-exposed to 3TC/ZDV/NVP and achieved HIV VIL <50 copies/ml. No perinatal transmission occurred in any of the 22 births. Conclusions: NVP associated with ZDV/3TC as a regimen to prevent MTCT may involve a low risk for the selection of anti retroviral-resistant strains and may notjeopardize the use of these same drugs for future treatment.

Intraperitoneal cisplatin and intravenous paclitaxel in the treatment of epithelial ovarian cancer patients with a positive second look

Objective. The aim of this study was to review the role and safety of intraperitoneal (IP) cisplatin and intravenous (IV) paclitaxel in platinum-sensitive epithelial ovarian cancer patients who were found to have small-volume disease (<1 cm) at the time of their second-look procedure. Methods. In a retrospective review, 32 patients with small-volume disease had an LP Bardport catheter placed at the time of second look at Memorial Sloan-Kettering Cancer Center (1995-1998). Patients received IP cisplatin (75 mg/m(2)) every 3 weeks and either IV paclitaxel (135 mg/m(2)) every 3 weeks or IV paclitaxel (80 mg/m(2)) weekly for a maximum of five cycles. Results. Twenty-four (75%) of 32 patients received LP cisplatin/IV paclitaxel every 3 weeks and 8 (25%) received LP cisplatin every 3 weeks with weekly IV paclitaxel. Seven (21.9%) of 32 patients required interruption of treatment secondary to neuropathy. Of these, 4 (15.6%) were changed to another TV chemotherapeutic agent, and 3 (9.39%) required discontinuation of TV paclitaxel only. Two (6%) patients required IP port removal secondary to malfunction and were changed to IV therapy and I (3%) requested discontinuation of TP therapy secondary to abdominal pain. Median follow-up was 19 months (mean, 20.1 months;range, 6-36 months). Progression of disease after completion of IP therapy was documented by clinical exam, abnormal CT, and/or rising CA-125 levels. The median progression-free interval was 13 months (mean, 15.1 months;range, 2-33 months). Median overall survival was 27 months (mean, 34.2 months;range, 10-42 months). At the time of review, 13 (40.6%) of the 32 patients were alive with disease, 7 (21.9%) were without evidence of disease, and 12 (37.5%) were dead of disease. Conclusion. LP cisplatin in combination with IV paclitaxel appears to be no more effective than other reported regimens as second-line therapy for patients with persistent small-volume disease. Are activated clotting times helpful in the management of anticoagulation with subcutaneous low-molecular-weight heparin?

Background Enoxaparin has recently been shown to be superior to unfractionated heparin in patients with unstable angina/non-ST-elevation myocardial infarction. Theoretical advantages of low-molecular-weight heparin versus unfractionated heparin include a higher ratio of anti-Xa to anti-IIa activity (3:1 for enoxaparin), a more predictable dose response that precludes the need for frequent monitoring, and the convenience of subcutaneous administration. Both activated partial thromboplastin time and activated clotting time (ACT) are used to monitor anticoagulation with heparin, and ACTs are now standard during percutaneous coronary intervention (PCI) with heparin. At doses of up to 90 mg, subcutaneous enoxaparin leads to a modest dose-related increase in activated partial thromboplastin time, but the effect on ACT is unknown. Methods Thrombolysis In Myocardial Infarction (TIMI) 11A was a multicenter, dose-ranging trial to evaluate the safety and tolerability of subcutaneous enoxaparin in patients with unstable angina/non-ST-elevation myocardial infarction. We obtained peak (mean 4.3 hours after enoxaparin) and trough (mean 11.5 hours after enoxaparin) anti-Xa levels and ACTs for 26 patients in the TIMI 11A trial. Results Despite doses of enoxaparin in the range of 89 +/- 19 mg every 12 hours and significant increases in anti-Xa levels even at trough, there was no change in the ACT measured by HemoTec and only a small increase with Hemachron. The correlation of peak Hemachron ACT with peak anti-Xa levels was poor (R = 0.5, P = .08). Conclusions In contrast to heparin, ACTs are not useful for assessment of anticoagulation with subcutaneous enoxaparin and should not be relied on in patients receiving enoxaparin who require acute PCI. Studies to determine the optimal dose, safety, and efficacy of enoxaparin in patients undergoing PCI are underway.

Osteonecrosis in patients with systemic lupus erythematosus develops very early after starting high dose corticosteroid treatment

Objectives-To investigate the actual time of onset of osteonecrosis (ON) after high dose corticosteroid treatment in systemic lupus erythematosus (SLE). Methods-72 patients with active SLE, who received high dose corticosteroid for the first time, for the development of ON at hips and knees were monitored by magnetic resonance imaging for at least 12 months. Results-ON lesions were detected in 32/72 patients (44%) between one and five months (3.1 months on average) after starting high dose corticosteroid treatment. No osteonecrotic lesion was newly, detected from the sixth month of treatment until the end of the follow up period. Conclusion-The findings suggested that the actual time of onset of ON in SLE is within the first month of high dose corticosteroid treatment.

Carvedilol titration in patients with congestive heart failure receiving inotropic therapy

Background Carvedilol has been shown to improve morbidity and mortality in patients with congestive heart failure (CHF). There are limited data of carvedilol use in patients on inotrope therapy. We present our experience with carvedilol titration in New York Heart Association (NYHA) class IIIb/IV patients stabilized on milrinone therapy, as a nonrandomized study with a parallel control group of patients never on inotropes. These patients achieved volume control and stabilization of their symptoms during the course of milrinone therapy. Methods and Results Seventeen patients in class IIIb/IV CHF (group 1) on intermittent intravenous milrinone therapy and 15 patients in class II/IIIa compensated CHF (group 2) on standard triple heart failure therapy were titrated on carvedilol. Success and adverse events during titration were compared between the 2 groups. Fifteen (88%) patients in group 1 and 14 (93%) patients in group 2 were successfully titrated on carvedilol over 8.1 +/- 1.8 weeks and 6.7 +/- 2.8 weeks, respectively. The target dose of carvedilol (25 or 50 mg twice daily) was achieved in 13 (87%) patients (group 1) and 14 (93%) patients (group 2). Seven (47%) patients in group 1 and 4 (28%) patients in group 2 had adverse events during carvedilol titration. Eight (53%) patients in group 1 were weaned off milrinone over a period of 8.4 weeks after carvedilol titration, whereas the rest of the patients had reduction in the frequency of infusion. Ten (63%) patients in group 1 improved by one or more functional classes. Conclusions Patients in NYHA class IIIb/IV who are treated with inotropic therapy can be titrated on carvedilol after reaching a stable state while on milrinone and standard oral drugs. Most of these patients can be successfully weaned off of milrinone or have decreased frequency of infusions and demonstrate improved functional status. Sequential chemosurgery for advanced head and neck cancer: Another pathway in a complex maze?

In the treatment of advanced head and neck cancer, the chemotherapy era began more than three decades ago. The trends or phases throughout this era can be characterized by what agents, combinations of agents and modalities (surgery, radiotherapy, and chemotherapy), and sequencing of the modalities have been explored. Always with the hope of finding a better regimen to improve survival and preserve organ function, the ongoing chemotherapy era represents a time of intense interest and investigations for this disease. As part of this wave, Loré and colleagues1 have pursued the pathway of sequential chemosurgery. In this issue they report on the experience spanning 20 years during which patients with locally advanced squamous cell carcinoma of the head and neck underwent sequential chemosurgery. Although the overarching goal was to improve survival, the strategy also presented an opportunity to avoid postoperative radiotherapy in many patients. At initial presentation, all patients had disease that could have been treated by primary surgery. Following the induction chemotherapy consisting of cisplatin/bleomycin (regimen A, early phase) or cisplatin/5-flurouracil (regimen B, later phase), all patients had surgery irrespective of the degree of response to induction chemotherapy. Testing the efficacy of this approach and determining if chemotherapy could be substituted for radiotherapy was a worthwhile endeavor. Based on the assumption that survival is the most important outcome for a therapeutic trial, the results are impressive. With a median follow-up of 7.6 years, the 5-year absolute survival rate for the total group of 82 patients was a respectable 60%. For the subset receiving the contemporary regimen of cisplatin/5-fluorouracil, the 5-year survival rate was 77%. Only 23% of patients required postoperative radiotherapy. Functional groups such as nitro, chloro, bromo and methoxy were introduced in the meta and para positions of the phenyl ring of the antineoplastic agent 2-chloromethyl-3-phenyl-1,4-naphthoquinone. Tests for tumor inhibitory potency of these derivatives against sarcoma 180 ascites cells in mice indicated that the para-substituted methoxyphenyl, chlorophenyl and bromophenyl derivatives possessed anti-tumor activity comparable to that of the parent compound 2-chloromethyl-3-phenyl-1,4-naphthoquinone;meta-substituted nitro and bromo derivatives were inactive or only weakly active anticancer agents in this system.

Evaluation of gemcitabine in patients with squamous cell carcinoma of the cervix: A Phase II study of the Gynecologic Oncology Group

Purpose. A multicenter Phase II trial was conducted to evaluate the activity and toxicity of gemcitabine in patients with previously treated squamous cell carcinoma of the uterine cervix, Patients and methods. Patients were required to have measurable disease with adequate performance status, bone marrow, hepatic, and renal function. Histologic confirmation of the primary diagnosis as squamous cell cancer of the uterine cervix was mandatory. Patients were allowed one prior chemotherapy regimen, usually cisplatin-based, The initial dose of gemcitabine was 800 mg/m(2) weekly times three with 1 week off until progressive disease or adverse effects prohibited further therapy. Doses were escalated or reduced based on previous cycle toxicity. Results. Twenty-seven patients were entered into the trial. One patient never received the drug and 1 patient was inevaluable for response. A median of two cycles were administered to each patient (range: 1-7 cycles). The overall response rate (two partial responses) was 8% with 21% of patients having stable disease. The median progression-free interval was 1.9 months (range: 0.5-9.0) and overall survival was 4.9 months (range: 1.5-16.3). Two patients had grade 4 neutropenia;1 patient had grade 4 anemia. The median WBC nadir in the 13 patients experiencing any leukopenia was 2300/mu l (range: 400-3800). There was only one episode of grade 4 gastrointestinal toxicity. Conclusions. Gemcitabine as a single agent demonstrated minimal antitumor activity in previously treated patients with squamous cell cancer of the uterine cervix. Since gemcitabine in the dose and schedule employed is known to potentiate the cytotoxicity of cisplatin and radiotherapy (the current standard therapies for this disease), further development of gemcitabine would only be indicated in combination with these treatment modalities.
