A telescience ground testbed experiment was conducted by the National Space Development Agency of Japan (NASDA) at the Tsukuba Space Center in March 1991. The objectives of the ground testbed experiment were to extract scientists' requirements for a communication method, to evaluate the influence of transmission delay and capacity on experiment operations, and to evaluate performance and functions of the system for the testbed experiment. The microscopic operations experiment, the image furnace experiment and the onboard training experiment were selected as typical ground testbed Experiments. In these experiments, motion video transmission at 320 kbps was acceptable for observing the experiments and communicating between the principal investigator and the payload specialist. In the microscopic operations experiment, motion video transmission at 1.5 Mops or more was required for detailed observation. A 4-second transmission delay (roundtrip) was allowable for mutual communication.

For more than 15 years, Spacelab, has provided a laboratory in space for an international array of experiments, facilities, and experimenters. In addition to continuing this important work, Spacelab is now serving as a crucial stepping-stone to the improved science, improved operations, and rapid access to space that will characterize International Space Station. In the Space Station era, science operations will depend primarily on distributed/remote operations that will allow investigators to direct science activities from their universities, facilities, or home bases. Spacelab missions are a crucial part of preparing for these activities, having been used to test, prove, and refine remote operations over several missions. The knowledge gained from preparing these Missions is also playing a crucial role in reducing the time required to put an experiment into orbit, from revolutionizing the processes involved to testing the hardware needed for these more advanced operations. This paper discusses the role of the Spacelab program and the NASA Marshall Space Flight Center- (MSFC-) managed missions in developing and refining remote operations, new hardware and facilities for use on Space Station, and procedures that dramatically reduce preparation time for flight. Successful implementation of telemedicine services depends on an adequate telecommunications infrastructure. At present the main infrastructure in Russia and the countries of the former Soviet Union consists of analogue telephone lines. These are sufficient for email and access to the World Wide Web but do not provide sufficient bandwidth for the transmission of realtime teleconsultations. ISDN lines and fibre optic cables are becoming more widespread in the major cities but have yet to be installed in the regions.

A functional description of a computerized system for analysis of electrocardiograms (EKGs) is presented. Although it is recognized that the diagnostic accuracy of the program is the critical parameter in determining system acceptance, other system features such as quality control, turn around time and cost are shown to be of high importance. A digital simulation of alternative configurations for on-line data acquisition is presented. The simulation predicts the system throughout and wait times in different phases of a telephone EKG transmission for combinations of interfaces, telephone lines and acquisition procedures. These results should be helpful in selecting the appropriate configuration for each special situation.

For more than 3 years members of the TeleDent team from Bristol University have been looking at the potential of videoconferencing technology for dentistry Here they explain what videocanferencing is and how it can help the GDP. They discuss examples of its most promising uses for the profession, which include professional updating and providing diagnostic support at a distance. They describe the equipment that is needed, the different types of system available and give an indication of costs. A suggested procedure for using the technology for remote referrals is outlined. 'Store and forward' techniques are also discussed. These do not involve a live video but involve the sending of static electronic files. This approach is compared with videoconferencing, and the article looks at the question of which will be best suited to the GDP, and for what purposes.

A scarcity of health care workers attributable to the present maldistribution of physicians is one of the most pressing health care delivery problems in rural America. Expanded utilization of non-physician primary health care providers is a possible solution to this problem. The capability of satellite clinics staffed by these personnel to provide ambulatory services efficiently and effectively can be enhanced by reducing the need for physical referral of the patients to a physician at a central clinic. The use of telecommunication technologies for conducting consultation between remotely located non-physicians and physicians can potentially reduce the required number of such referrals. This paper describes the procedures used for estimating the impact of alternative telemedicine technologies on the delivery of primary care in rural areas. Non-physician protocols were used to measure the impact of technology.
