A study was carried out to ascertain if those older persons at risk from influenza in Ireland would respond to a personalised invitation to attend for vaccination. Of the 193 patients invited to attend for vaccination, 126 (65.3 per cent) presented for vaccination. This rose to 142 (73.6 per cent) after a reminder letter was went oat. In the year previous to the study only 80 (41.5 per cent) of the same cohort of patients were vaccinated. This study has demonstrated how a simple invitation for influenza vaccination can dramatically improve the uptake rates.

Orthopaedic shoes are individually handmade after a prescription from an orthopaedic surgeon, hence relatively expensive. Bad compliance is mentioned in the literature but not investigated. In order to evaluate patient compliance and the effect of orthopaedic shoes, 85 patients who were prescribed orthopaedic shoes at the authors' department during a 3 year period received a questionnaire concerning relief of symptoms and daily use of the shoes. The answers from 74 patients were correlated to the prescription procedure and the degree of medical follow-up. Only 60 of 74 patients used their shoes. Some 51 patients had some benefit while 23 had no effect or even worse symptoms. Some patients even used their shoes despite no symptomatic relief. However, patients who felt they were well informed about the purpose and function of their shoes had more benefit than the rest. Only 12 patients of the 74 were checked by the orthopaedic surgeon after delivery of the shoes. In conclusion the authors believe there is a great need for information to be given to the patients about the functions and limitations of orthopaedic shoes and that every patient should be offered a control check-up by the surgeon. Further investigations of the effect of orthopaedic shoes should be carried out to optimise the use of these expensive devices.

The purpose of this study was to document compliance to treatment follow-up for facial pain patients referred for outside services, In addition, we generated a multidimensional model of the psychosocial constructs associated with chronic pain to determine whether these factors were predictive of compliance across recommended therapy modalities or with an overall measure of compliance. These constructs included pain report, depressive symptoms, anxiety, cognitive coping strategies, and physical activity reduction. The sample consisted of 80 facial pain patients evaluated at a tertiary care, facial pain clinic at a large university medical center. Compliance ratings ranged from 93% to 50% and are consistent with the literature that indicates that compliance differs across treatment modality. Furthermore, compliance rates were lower for the more nontraditional facial pain treatments performed by physical therapists or psychologists. Depression was negatively associated with compliance to medication changes, therapeutic injections, and splint therapy, but not psychological counseling or physical therapy. Increased pain was only associated with physical therapy. Measures of psychological distress (depression and anxiety) and pain were predictive of the overall measure of compliance. These results suggest that psychological distress can be a barrier for positive patient outcomes through reduced treatment compliance.

Objective: To evaluate opportunistic reminders in emergency departments (EDs) about cervical smears. Method: Five EDs in Sydney in 1995-96 participated in a block-randomised control trial, whereby unscreened women at risk for cervical cancer who were seen as nonacute cases were allocated either to brief advice at the end of the ED consultation to return to their GP for a smear (intervention) or usual care (control). Blinded follow-up telephone interviews determined women's compliance four weeks later. Results: 217 women at risk for cervical cancer presented, of whom 53 (23%) were overdue for a Pap smear (22 intervention group;31 control group). At follow-up, no women in the intervention group had had a smear, although 3 (10%) in the control group had (p= 0.25). Conclusions: Opportunities in EDs in NSW for opportunistic reminders are infrequent and, even if taken, are unlikely to encourage screening over and above usual care. Implications: General practice remains a more effective location for opportunistic recruitment, although interventions with hospitalised in-patients also show greater promise than reminders in EDs.
