The Joint Commission on Accreditation of Healthcare Organizations has accredited behavioral health care organizations since 1957. Accredited behavioral health core organizations include organizations providing services to persons with psychiatric disorders, substance abuse disorders, or developmental disabilities/mental retardation;or provide social services to other related populations. In the late 1980s, the Joint Commission initiated the redesign of accreditation to include performance measurement. Performance measurement initiatives include the IMSystem Request for Indicators, National Library of Healthcare Indicators,(TM) and a national Council on Performance Measurement. These initiatives are steps toward the integration of performance measurement data into the accreditation process.

A growing number of children in the United States are being placed into foster care. Past studies indicate that effective case manager interventions have helped foster families with a variety of different problems. This study enrolled a randomly selected sample of 130 children under age four who had been newly placed into foster care. The purpose of this study was to identify the services needed by foster care families and determine which services require the most case manager effort. Consistent with other research, many foster care children in this study exhibited developmental, medical and psychosocial concerns. Nevertheless, we found that it was services aimed at the foster care parents, rather than the foster care children, that required the most labor-intensive case management services.

The delivery of services for people with severe mental illness (SMI) is described as healthcare's most difficult challenge because SMI patients suffer from the combined effects of conditions that have 1) a chronic course, 2) disabling consequences, and 3) a high risk of poverty. The fundamental advantages of managed care for persons with chronic, disabling conditions are discussed along with an examination of how well these assumptions hold in actual practice for persons with SMI. The legal risks assumed by clinicians in managed care are reviewed, including the inherent risk of discriminating against high cost enrollees, such as patients with SMI. Finally, recommendations are made regarding the measures that might be taken to increase the ''match'' between the principles of managed care and the specialized needs of people with severe mental illness.

Public mental health has long struggled to be accepted as a part of health care. Its interface with social services and its broad spectrum of professionals make a clear definition of public mental health's boundaries difficult, fueling policymakers' skepticism about such acceptance. Examination of their sociopolitical situatedness, in Australia, allows adolescent mental health and adolescent mental health nursing to be understood within a broader context. This examination takes place through a review of: (i) epidemiology, (ii) health funding and services, (iii) training and education, (iv) research, and (v) nurse education. Implications arising from this review are presented. Questions are raised regarding the sociopolitical standing of adolescent mental health, and suggestions are made regarding future imperatives for adolescent mental health nursing.
