Specialist practice in community mental health nursing

Community mental health nurses (CMHNs) work in an increasingly complex health and social care environment. Over recent years, the evolving direction of general health service and specific mental hearth policy has directed CMHNs towards: the provision of clinically-effective interventions;a closer attention to meeting the needs of people experiencing severe and long-term mental health problems;the simultaneous provision of services to meet the needs of people experiencing a wide range of mental health problems presenting in primary care settings;greater collaboration with workers representing other disciplines and agencies;and the development of active partnerships with mental health service users. This paper explores the context within which CMHNs practise, and within which education programmes preparing specialist practitioners in community mental health nursing have been developed. One recently-validated specialist practice course for CMHNs is described in detail, with the intention of stimulating discussion and debate surrounding the practice of, and the educational preparation for, community mental health nursing. The role of advanced practice psychiatric nurses in treating tobacco use and dependence

Tobacco use is treatable and the benefits of smoking cessation are impressive, yet like other health care providers, psychiatric nurses have failed to consistently assess and treat tobacco use effectively, Tobacco use continues to cause illness, disability, and death at unprecedented rates. Nicotine addiction is drug abuse and it is a chronic disease and needs to be treated as such. There are clear health benefits to smoking cessation regardless of the age of smoking initiation or the age of smoking cessation, Advanced practice psychiatric nurses (APPNs) are in a unique position to make an impact on a smoker's risk of suffering from tobacco-related diseases: the treatment of choice is the combination of pharmacotherapy and psychosocial interventions, both within the realm of APPN practice;psychiatric and substance abusing patients consistently demonstrate increased rates of cigarette smoking compared with healthy controls;and tobacco use is drug abuse, a treatable chronic disease, The ability of APPNs to deliver psychotherapeutic and pharmacologic care secures a position in the forefront of treating tobacco use. In addition to intervention, psychiatric nurses need to step up and take an active role in initiating and supporting tobacco control policy and legislation. Research priorities in mental health nursing

This study determined nursing research priorities that focused on mental health nursing in the published literature from 1990 to 1996. Determination of which priorities were related to mental health was completed using 18 sources (experts, organizations, and individual research projects). A content analysis of the 18 sources was completed, and 56 menial health related research priorities were identified. Six categories emerged from the data analysis: support, holism, mental health nursing practice, quality care outcomes, mental health etiology, and mental health delivery systems. As mental health nursing embraces evidenced-based practice, the need for clear research priorities is imperative for knowledge development in the field.

Prescribing practices of advanced practice psychiatric nurses: Part I - Demographic, educational, and practice characteristics

This study was designed to identify and describe the prescribing practices of advanced practice psychiatric nurses. Part I describes the demographic, educational, and clinical setting characteristics. A survey method was used with N = 88 nurses representing seven different states. Nurse prescribers were: younger than nonprescribers, employed in community mental health settings, and voluntarily using a consulting physician arrangement. Limitations on practice included: statute formulary, controlled substances, and formulary/protocols with a consulting physician. Results suggest nurses' prescribing practices are used in outpatient settings, viewed as conservative, with a value on physician collaboration even in the absence of legal mandates. Recent changes in the nature and scope of nursing home practices challenge long-term care nurses to develop treatment programs that are both successful in enhancing residents' remaining quantity and quality of life, as well as cost-effective in treating specific problems. The characteristics of behavioral therapies make them ideal for the open, community environment that characterizes many long-term care (LTC) settings. Under the guidelines of the Omnibus Budget Reconciliation Act of 1987, LTC facilities are expected to initiate behavioral management programs and to train staff in behavioral management practices. The purpose of this article is to discuss five key issues that should be considered in planning behavioral management programs for LTC facilities.

A view of prescriptive practice collaboration: Perspectives of psychiatric-mental health clinical nurse specialists and psychiatrists

A majority of states require collaborative prescribing agreements between advanced practice nurses and physicians, Unfortunately, there is limited research describing the collaboration that occurs between the clinicians who have such prescribing agreements. This exploratory study identifies the characteristics, activities, and outcomes of collaboration between psychiatricmental health clinical nurse specialists (CNSs) with prescribing agreements and their collaborating psychiatrists. Surveys were sent to all the 73 prescribing psychiatric-mental health CNSs identified by the Minnesota Board of Nursing in I998 and their primary collaborating psychiatrists, Forty-nine CNSs and 32 psychiatrists returned the surveys with 31 matched collaborating dyads identified. Overall satisfaction with the collaborative relationship was high, CNSs ((x) over bar = 4.34/5) and psychiatrists ((x) over bar = 4.46/5), Good communication, trust, shared goals for patient outcomes, shared professional values, and respect for clinical competency were identified as important characteristics for effective collaboration, CNSs identified increased professional growth and job satisfaction as professional benefits, while psychiatrists reported shared workload responsibilities. Fewer than half of the CNSs and psychiatrists perceived professional liability as a professional constraint. Psychiatricmental health CNSs and psychiatrists agreed that the continuity of patient care and efficient access to mental health care were patient benefits. The statistically significant differences between the CNSs' and psychiatrists' responses were related to the number of years they had been in practice, the number of years the CNS had been a prescriber, and the length of time the CNS and psychiatrist had worked together within a collaborative prescribing agreement. A review of the sociopolitical context of adolescent mental health and adolescent mental health nursing in Australia.

Examination of their sociopolitical situatedness, in Australia, allows adolescent mental health and adolescent mental health nursing to be understood within a broader context. This examination takes place through a review of: (i) epidemiology, (ii) health funding and services, (iii) training and education, (iv) research, and (v) nurse education. Implications arising from this review are presented. Questions are raised regarding the sociopolitical standing of adolescent mental health, and suggestions are made regarding future imperatives for adolescent mental health nursing.
