目的观察辛伐他汀对糖尿病大鼠血中单核细胞趋化蛋白-1(MCP-1)、细胞间黏附分子-1(ICAM-1)水平的影响,以了解其对糖尿病大血管病变的保护作用。方法建立糖尿病大鼠模型,分为正常对照组(A组)、糖尿病组(B组)、辛伐他汀治疗组(C组)。糖尿病大鼠模型建立1周后,C组以辛伐他汀20mg/(kg·d)灌胃,A组和B组灌以相应量的蒸馏水,检测各组每周血糖、检测第8周血清中MCP-1、ICAM-1水平。结果血糖水平B组和C组相近,均显著高于A组(P<0.01);第8周B组和C组血中MCP-1、ICAM-1水平均显著高于A组(P<0.01),其中C组较B组明显减少(P<0.01)。结论辛伐他汀在调脂治疗的同时有一定的抗炎作用,对糖尿病大血管病变提供保护。

Clinical efficacy of metformin against insulin resistance parameters - Sinking the iceberg

It has been increasingly recognised in recent years that type 2 (non-insulin-dependent) diabetes is part of a cluster of cardiovascular risk factors known as the metabolic syndrome, but also endorsed with such names as the deadly quartet, syndrome X and the insulin resistance syndrome. Atherosclerosis is the most common complication of type 2 diabetes among Europeans, and coronary artery, cerebrovascular and peripheral vascular disease are 2 to 5 times more common in people with this condition than in those without diabetes. These observations indicate that the treatment of type 2 diabetes requires agents that do more than simply lower blood glucose levels, and a therapy with both antihyperglycaemic effects and beneficial effects on dyslipidaemia, hypertension, obesity, hyperinsulinaemia and insulin resistance is likely to be most useful. In this respect, metformin has an important and established role: this drug has been shown to lower blood glucose and triglyceride levels, and to assist with weight reduction and to reduce hyperinsulinaemia and insulin resistance. Studies in the Israeli sand rat, Psammomys obesus, have indicated hyperinsulinaemia/insulin resistance to be the initial and underlying metabolic disorder in obesity and type 2 diabetes. Thus, the well established effect of metformin in reducing insulin resistance makes this drug an excellent candidate for the prevention of progression of impaired glucose tolerance to type 2 diabetes, and for the reduction of mortality associated with cardiovascular disease.

Prevention and treatment of microvascular and neuropathic complications of diabetes

The most important factors that influence development of the eye, kidney, and nerve complications of diabetes are the duration and degree of exposure to hyperglycemia. Hypertension is also very important. The risk of complications appears to be similar for all patients with diabetes;differences in complication rates between patients with type 1 and type 2 diabetes are largely due to differences in duration of diabetes and glycemic control. The benefits of lowering blood sugar are also similar for patients with diabetes, regardless of the type. Significant reductions in complications san be seen with relatively short-term treatment of hyperglycemia.

Implications of the United Kingdom prospective diabetes study

The United Kingdom Prospective Diabetes Study (UKPDS) is the largest ever study to clarify the role of glycemia and blood pressure control in improving morbidity and mortality in patients with type 2 diabetes. The study examined the impact of glycemia and blood pressure control on microvascular and macrovascular risk, and if any particular therapy was advantageous for these patients. This articles discusses the background, methodology and results of the UKPDS and develops recommendations for treatment and management of patients with type 2 diabetes in the clinical setting.

Morphological changes in the sciatic nerve of diabetic rats treated with low molecular weight heparin OP 2123/parnaparin

This study points out the early alterations that affect the sciatic nerve of rats with experimentally induced diabetes. It emphasizes the advantages deriving from treatment with low molecular weight heparin (LMWH) OP 2123/parnaparin and correlates these findings with observations emerging from a review of the relevant literature. In fact, the number and diameter of the capillaries within the sciatic nerve of diabetic rats appear to be increased consistent with a microangiopathy (the main characteristic of which is the fragility of new vessels), that is related to the evolution of diabetes in humans and animals. Our results suggest a possible therapeutic role for OP 2123/parnaparin in both diabetic neuropathy and microangiopathy, frequent complications of diabetes.

Treatment of diabetic patients with hypertension

This review attempts to highlight the potential of calciumchannel blockers in the prevention of sequelae of diabetes mellitus and hypertension in patients who have both disorders. Evidence-based medicine is driven by the results of randomized, clinical trials. Major contributions were therefore derived from post hoc analyses of the diabetic patients enrolled in placebo-controlled trials, such as Systolic Hypertension in the Elderly Program (SHEP), Systolic Hypertension in Europe (Syst-Eur), and Systolic Hypertension in China (Syst-China), and stepped-care blood-pressure- oriented trials, such as the Hypertension Optimal Treatment (HOT) and United Kingdom Prospective Diabetes Study (UKPDS). Several studies, such as the Fosiniprl;versus Amlodipine Cardiovascular Events Trial (FACET) and Appropriate Blood Pressure Control in Diabetes (ABCD) Trial, have compared the relative merits of angiotensin- converting enzyme and calcium-channel blockers in preserving renal function and metabolic balance in diabetic patients with hypertension, but their publications focused on cardiovascular disorders, which were only secondary end points. On balance, the articles reviewed prove that dihydropyridine calcium-channel blockers score particularly well in the prevention of cardiovascular complications in diabetic patients with hypertension. 目的探讨健康管理对糖尿病血管并发症的影响效果。方法本次研究共选择120例糖尿病患者作为研究对象,均为本院2013年1月至2014年1月收治的糖尿病患者,通过数字标号和随机分组将患者分为管理组和常规组各60例,给予管理组患者进行常规药物降糖治疗联合降糖管理,给予常规组患者仅进行常规药物降糖治疗。统计分析2组患者的糖尿病血管并发症发生状况。结果进行管理后,管理组患者血管并发症发生率低于常规组患者,差异有统计学意义(χ~2=4.444,P<0.05)。结论健康管理能有效控制糖尿病患者的糖尿病血管并发症,降低危险性,保证患者生命安全,具有重要的临床推广价值。

Protection from vascular risk in diabetic hypertension

Diabetes mellitus is a metabolic disorder characterized by hyperglycemia and associated with a high incidence of complications affecting both the microvascular and the macrovascular systems. Macrovascular disease affects the coronary arteries, the cerebral vessels, and the large peripheral arteries of the lower extremities. Microangiopathy affects the kidneys, eyes, and nerves. Both forms of complication are major causes of death and disability in diabetes. The precise pathophysiology of these vascular complications is becoming better understood, but specific treatment and prevention remain complex. Copyright © 2000 by Current Science Inc.
