玻璃体腔注射曲安奈德围手术期的护理

With the volume of laser in situ keratomileusis procedures growing exponentially, ophthalmologists and other eye-care providers are becoming aware of an uncommon postoperative condition, diffuse lamellar keratitis, that can affect an otherwise ideal outcome. We present our strategy for diagnosing and managing this syndrome, developed from experience in a high-volume refractive surgical practice. Understanding the time course of the disease, along with proper identification, staging, and intervention, can help eliminate visual loss associated with this condition. Lack of correlation between intraocular inflammation and after-cataract formation in the rabbit eye

Purpose: To determine whether an increased or reduced inflammatory response following cataract surgery influences the development of after-cataract. Setting: Department of Preclinical Ophthalmology, Pharmacia, Uppsala, Sweden. Methods: Rabbits that had had cataract surgery were given endotoxin, ovalbumin, dexamethasone, or diclofenac. Aqueous humor, leukocytes, and prostaglandin E-2 (PGE(2)) were analyzed. and the wet weight of the after-cataract was determined. Results: The wet weight of the after-cataract was unaffected by endotoxin and 67% lower in the eyes treated with ovalbumin than in the control eyes on day 56. Aqueous humor concentrations of leukocytes and PGE(2) were 94% and 87% lower in the group treated with dexamethasone than in the control group on day 7, and the concentration of PGE(2) was 98% lower in the diclofenac group;however, the wet weight of the after-cataract was unaffected by both treatments. Conclusion: This study suggests that an increased inflammatory response does not increase the development of after-cataract and a reduction in the inflammatory response does not reduce the development of after-cataract. Effects of high-dose intravenous corticosteroid therapy in Landau-Kleffner syndrome

Two children with Landau-Kleffner syndrome were successfully treated with antiepileptic drugs and a high-dose intravenous corticosteroid. A combination of valproate and a benzodiazepine (clonazepam or diazepam) ameliorated epileptic seizures and electroencephalographic spikes and waves, but speech disturbances persisted. Both patients were treated with an intravenous infusion of high-dose methylprednisolone sodium succinate (20 mg/kg daily) for 3 consecutive days. This infusion was repeated three times with a 4-day interval between treatments, which resulted in a rapid improvement in speech ability. After intravenous therapy, prednisolone was given orally (2 mg/kg daily for 1 month, then gradually withdrawn), which maintained the clinical improvement in speech. (C) 2000 by Elsevier Science Inc. Bilateral corneal infiltrates after excimer laser photorefractive keratectomy

A 26-year-old man developed painless inferior subepithelial infiltrates away from the site of ablation in both eyes after excimer laser photorefractive keratectomy (PRK) for myopia. Clinical characteristics of the corneal infiltrates resembled staphylococcal-immune infiltrates. The condition responded to treatment with topical diluted steroids and antibiotics. There was no residual corneal scarring. The infiltrates did not affect the refractive outcome of the surgery. Recognition of this rare entity will help clinicians avoid aggressive investigative and treatment modalities that can affect the results of PRK. Cinnamon-induced oral erythema multiformelike sensitivity reaction

Background, Erythema multiforme, or EM, is a hypersensitivity reaction to agents such as herpes virus, drugs or foods. Oral EM-like reactions may be induced by cinnamon. It is important for dentists to recognize this condition. Case Description, The authors report an unusual case of cinnamon-induced oral EM-like reaction in an older female patient. The disease manifested itself as chronic extensive surface ulcerations and lip swelling. The patient was treated repeatedly with topical and systemic steroids but responded to them only temporarily. The patient was cured when the offending agent-cinnamon-was eliminated from her diet. Clinical Implications, Oral EM or EM-like reactions pose a diagnostic dilemma due to their varied etiologies and clinical manifestations. Correct diagnosis is essential for effective treatment. A negative skin test does not rule out an allergic reaction to food.

Wound healing after photorefractive keratectomy

For more than 15 years, the excimer laser has been used as a surgical instrument on the cornea. Photorefractive keratectomy (PRK) followed radial keratotomy as researchers sought a more precise technique. In PRK, precision turned out to depend on surgical technique as well as the wound-healing process, with the 2 factors interdependent. The PRK technique has evolved toward a large diameter, flat ablation curvatures, and an even surface, The role of such factors as cytokines and interleukins has become more clear in the past 10 years. However, understanding the wound-healing process becomes more complicated with increasing knowledge. Learning the contributing factors and performing trials with new drugs and antibodies to modulate wound healing have shown positive results on the experimental level. Patient selection based on the concentration of epidermal growth factor in tears may be another way to increase PRK's precision. The PRK technique has taught much about wound healing. For the technique to be competitive, increased precision, particularly in eyes with high myopia, is needed. Two other factors are imperative: controlling postoperative pain and decreasing visual rehabilitation time. J Cataract Refract Surg 2000;26:432-447 (C) 2000 ASCRS and ESCRS.
