Contemporary issues for spontaneous abortion - Does recurrent abortion exist?

In the past, clinicians have counted the number of spontaneous abortions among couples in an attempt to determine who should be evaluated for recurrent abortion. A better approach is to examine the environment in which pregnancy has occurred or been planned. This article discusses the previously held etiologies regarding spontaneous abortion. Evidence for and against these causes is presented, and the causes are included in contemporary evaluation and treatment paradigms.

Trajectories of fetal loss in the Czech Republic

Using data for 555,038 pregnancies conceived in the Czech Republic in 1987-1990, we show that pronounced differences in fetal survival in the middle trimester of pregnancy by marital status, educational level, and labor force attachment become much smaller at full term;survival differences by age at conception and number of previous deliveries show relatively constant proportional hazards throughout gestation. Social inequalities in postpartum life chances have been documented previously, but we show that similar inequalities exist before birth.

MANAGEMENT AND OUTCOME OF PREGNANCIES ASSOCIATED WITH COPPER-T INTRAUTERINE CONTRACEPTIVE DEVICE

The clinical outcome of 918 pregnancies in women who conceived while using one or another of four models of the Copper T intrauterine contraceptive device (IUD) is presented. Elective abortion was chosen by 58 per cent of the women. The clinical management and course of 275 women who elected to allow their unplanned pregnancy to continue are described. The Copper T was either expelled or extracted during the pregnancy in 118 of the women, while the IUD remained in situ for the duration of the pregnancy in the remaining 157 patients. The incidence of spontaneous abortion more than doubled when the IUD remained in utero. The chance of a live birth being premature was four times greater when the IUD remained in situ than when it was removed. There were 27 ectopic pregnancies in this entire series, giving an incidence of 3.4 per cent. In addition, the data suggest, but do not prove, that there is an increased risk of having an ectopic pregnancy if the woman has worn the Copper T for more than two years. A benign fibroma of the vocal cord was the only congenital anomaly detected. These observations suggest that the incidence of developmental defects is not adversely affected by the presence of intrauterine copper.

CURRENT STATUS OF ARTIFICIAL-INSEMINATION WITH DONOR SEMEN

As male infertility secondary to oligospermia and azoospermia remains refractory to treatment, donor insemination has gained greater acceptance. Wives of azoospermic and severely oligospermic men were evaluated for donor insemination. One hundred and thirty-four patients were found to be ovulating normally and to be free from pelvic pathology. Sixty-five accepted donor insemination. Fifty-one conceptions occurred in 41 patients, resulting in 43 live births and 8 miscarriages. The mean number of inseminations was 4.2 (range 1 to 13). Eighty-three patients were found to have endocrinopathies or pelvic pathology interfering with fertility; 58 were considered candidates for corrective therapy to be followed by donor insemination. Conception rates approached those observed in the average population; however, there was a greater incidence of abortion and ectopic gestation. Careful consideration of the patient prior to donor insemination ensures greater success and avoids unnecessary procedures when the patient is not considered a candidate for corrective therapy.

COMPLICATIONS OF PREGNANCY WITH AN INTRAUTERINE CONTRACEPTIVE DEVICE INSITU

Thirty-one consecutive pregnant women with intrauterine contraceptive devices in situ were studied. The devices consisted of the coil, loop, or bow. The pregnancies showed a high incidence of abortion, ectopic pregnancy, premature labor, premature rupture of the membranes, sepsis, and hemorrhage. Associated with these maternal complications was a high incidence of fetal wastage. A recommendation is made for early interruption of the pregnancy.

OVARIAN SURGERY IN PREGNANCY

A series of 57 cases of ovarian disease during pregnancy is presented; 10 of the patients were referred to the Mayo Clinic for treatment. During the 17 year period under investigation (1955 through 1971), there were 25,189 deliveries in Rochester, Minnesota. Thus, the corrected incidence of operable ovarian disease in pregnancy was one case in every 536 deliveries. Simple ovarian cysts were the most common lesions found (38 of 57 cases). The over-all abortion rate was about 25 per cent (14 of 57 patients); six of the patients were threatening to abort prior to surgery. Therefore, the incidence of spontaneous abortion after surgery was 16 per cent (eight of 51 patients). Progestational agents given before and after operation did not affect the abortion rate.
