We describe the construction of a nostril splint made from heavy body silicone-based dental impression material. This bio-compatible material supported a large chondrocutaneous auricular graft during early healing. The splint immobilized and maintained the shape of the alar rim and was further used nightly during the expected period of wound contraction.

The severity of injuries secondary to bicycling range from the pain and nuisance of skin abrasions to life-threatening brain injuries. For most injuries, the key to treatment and prevention is careful assessment of bike fit and training techniques. In general, the treatment of musculoskeletal and neuropathic lesions should follow sound, physiatric treatment principles. Clearly, all clinicians who treat bicyclists have a professional responsibility to educate them on the use of helmets and safe riding rules.

When providing trauma care, there is a danger that staff might forget what a frightening and confusing experience it can be for the patient, particularly if that patient is a child. As part of an academic exercise in reflection, I recently examined a critical incident involving the trauma care of a 9-year-old boy. In Accident and Emergency (A&E) the doctor inappropriately performed a rectal examination, which I witnessed in horror. The doctor failed to consider the effect of his actions on the child, the legal necessity for consent and the importance of a full explanation. Deeply disturbed by this incident and determined to avoid any repetition, I set out to find documented evidence to support my assertion that no child should be subjected to such intimate examinations, unless absolutely unavoidable. It is hoped that discussion of this incident will serve to raise the awareness of A&E staff working throughout the country with regard to paediatric rectal examination, as has been the case in my own workplace.

Accident and Emergency units are often regarded as the 'shop window' of a health service. While the priority is to attend to those in need of emergency services, there may also be opportunities for health education amongst a 'captive audience', particularly in relation to accident prevention. This report illustrates how the influence of one A&E department was used amongst primary school children to increase their awareness of cycle safety. Educational materials were developed, a series of visits to schools was planned, which over 600 children attended, and a competition organised. Initial responses from the children were very positive, and an analysis of subsequent cycle injury levels is now under way. The project formed part of a wide ranging practice development programme within a large district general hospital in Whitehaven in collaboration with St Martin's College Department of Nursing. It combined the need to develop quality services and staff in tandem. The outreach educational approach is now being examined for adoption in other areas of the Trust.

Despite the proven efficacy of bicycle helmets, helmet use among children remains low. Helmet use among children riding bicycles significantly reduces head injuries and death. A variety of promotional programs to increase helmet use in children have demonstrated limited success. This article reviews a variety of approaches to increase helmet use among children. The PRECEDE-PROCEED model is applied and recommendations for community health nurses to promote helmet use among children are provided.
