Nicorandil is an antianginal agent with actions at epicardial coronary arteries and arterioles, systemic arterioles, and veins. We report our experience with 7 patients taking oral Nicorandil who had severe vasodilation and hypotension requiring significant vasoconstrictor support after cardiopulmonary bypass. Although the mechanism for this phenomenon remains unknown Nicorandil might be interacting with other factors present during cardiopulmonary bypass, as it has relatively mild hemodynamic effects outside this situation. Formation of platelet aggregates after attacks at coronary spastic angina pectoris

In the present study, using a novel laser-light scattering method, we examined platelet aggregability at baseline and after spontaneous coronary spastic attacks in patients with coronary spastic angina, and before and after acute myocardial ischemia induced by an exercise test in patients with stable exertional angina. We also compared the results with the data from the conventional optical density method.

Randomized controlled study of inhaled nitric oxide after operation for congenital heart disease

Background. Inhaled nitric oxide selectively decreases pulmonary vascular resistance. This study was performed to determine whether inhaled nitric oxide decreases the incidence of pulmonary hypertensive crises after corrective procedures for congenital heart disease. Methods. Patients with a systolic pulmonary arterial pressure of 50% or more of the systolic systemic arterial pressure during the early postoperative period were randomized to receive 20 parts per million inhaled nitric oxide (n = 20) or conventional therapy alone (n = 20). Acute hemodynamic and blood gas measurements were performed at the onset of therapy. The efficacy of sustained therapy was determined by comparing the number of patients in each group who experienced a pulmonary hypertensive crisis. Results. In comparison to controls, there were no significant differences in the baseline and 1-hour measurements of patients who were treated with nitric oxide. Four patients in the control group and 3 patients in the nitric oxide group experienced a pulmonary hypertensive crisis. Conclusions. Nitric oxide did not substantially improve pulmonary hemodynamics and gas exchange immediately after operation for congenital heart disease. Nitric oxide also failed to significantly decrease the incidence of pulmonary hypertensive crises. (C) 2000 by The Society of Thoracic Surgeons.

Effect of topical vasodilators on gastroepiploic artery graft

Background. Mobilization of the gastroepiploic artery (GEA) often results in a vasospasm with reduction of early graft flow. In order to prevent or suppress this highly reactive artery's spasm, we have compared the effect of 4 vasodilators, used in external application to prepare the GEA graft, prior to myocardial revascularization. Methods. We performed a double-blind clinical study to compare the effects of external application of vasodilators on gastroepiploic artery grafts. Fifty patients, whose gastroepiploic artery was used for coronary artery bypass grafting, were randomized into 5 groups of 10 patients. Gastroepiploic artery free flow and hemodynamic measurements were evaluated immediately after harvesting, before any pharmacological manipulation, and 10 minutes after the topical application of vasodilators, respectively: papaverine, linsidomine, nicardipine, glyceryl trinitrate, and normal saline solution. Results. A significant increase in free now occurred in all groups except for the normal saline solution group with measurements from 26.1 +/- 3.6 mL/min to 26.4 +/-. 6.5 mL/min;p = 0.9. The most important increase in now before and after local application occurred with glyceryl trinitrate and papaverine: from 25.5 +/- 2 mL/min to 50 +/- 6.1 mL/min (p less than or equal to 0.01) and from 36.8 +/- 3.2 mL/min to 62 +/- 7.8 mL/min (p < 0.01) respectively. Nicardipine and linsidomine produced a less significant increase in flow: from 33.1 +/- 3.6 mL/min to 47.7 +/- 8.9 mL/min (p < 0.05) and from 28 +/- 3.8 mL/min to 39.8 +/- 7.5 mL/min (p < 0.05) respectively. When comparing percentage of flow increase, glyceryl trinitrate appeared to be significantly more efficient than nicardipine and linsidomine (p < 0.01 versus both groups). Although papaverine was more efficient than nicardipine and linsidomine, it did not reach statistical significance. Conclusions. Cardiovascular effects of inhaled nitric oxide in a canine model of cardiomyopathy

Background. The inhalation of nitric oxide (NO) in patients with heart failure decreases pulmonary vascular resistance (PVR) and is associated with an increase in pulmonary artery wedge pressure (PAWP). The mechanism for this effect remains unclear. Methods. In dogs rapid-paced for 8 weeks to induce cardiac dysfunction, we performed left ventricular pressure-volume analysis of unpaced hearts in situ to determine whether during NO inhalation (80 ppm), the mechanism for the rise in PAWP is due to: 1) primary pulmonary vasodilation;2) a direct negative inotropic effect;or 3) impairment of ventricular relaxation. Results. Inhalation of NO decreased PVR by 51% +/- 3.8% (257 +/- 25 vs 127 +/- 18 ***(-5) [NO 80 ppm];p < 0.001) and increased PAWP (15.4 +/- 2.4 vs 18.1 +/- 2.6 mm Hg [NO 80 ppm];p < 0.001). Calculated systemic vascular resistance remained unchanged. Left ventricular (LV) end-diastolic pressure rose (16.4 +/- 1.9 vs 19.1 +/- 1.8 mm Hg [NO 80 ppm];p < 0.001), as did LV end-diastolic volume (83.5 +/- 4.0 vs 77.0 +/- 3.4 mt [NO 80 ppm];p = 0.006). LV peak +dP/dt was unchanged by NO (1,082 +/- 105 vs 1,142 +/- 111 mm Hg/sec [NO 80 ppm];p = NS). There was a trend toward a stroke volume increase (17.4 +/- 1.2 vs 18.8 +/- 1.3 mL;p = NS), but the relaxation time constant and end-diastolic pressure-volume relation were both unchanged. Conclusions. In this canine model of cardiomyopathy, inhaled NO decreases pulmonary vascular resistance. The associated increase in left ventricular filling pressure appears to be secondary to a primary pulmonary vasodilator effect of NO without primary effects on the contractile or relaxation properties of the left ventricle. The efficacy, safety, and cost of prostaglandin E-1 (PGE(1)) in the treatment of severe intermittent claudication was studied comparing a long-term treatment protocol (LTP) with a short-term treatment protocol (STP) in a randomized 20-week study. The study included 980 patients (883 completed the study) with an average total walking distance of 85.5 +/- 10 m (range 22-119). Phase 1 was a 2-week run-in phase (no treatment) for both protocols. In LTP, phase 2 was the main treatment phase. In the LTP, treatment was performed with 2-hour infusions (60 mu g PGE(1), 5 days each week for 4 weeks. In phase 3 (4-week interval period) PGE(1) was administered twice a week (same dosage). In phase 4 (monitoring lasting 3 months, from week 9 to 20) no drugs were used. In STP phase 2 treatment was performed in 2 days by a 2-hour infusion (first day: morning 20 mu g, afternoon 40 mu g;second day morning and afternoon 60 mu g). The reduced dosage was used only at the first cycle (week 0) to evaluate tolerability or side effects. Full dosage (60 mu g bid) was used for all other cycles. The same cycle was repeated at the beginning of weeks 4, 8, and 12. The observation period was between weeks 12 and 20. A treadmill test was performed at inclusion, at the beginning of each phase, and at the end of 20th week. A similar progressive physical training plan (based on walking) and a reduction in risk factors levels plan was used in both groups. Intention-to-treat analysis indicated an increase in walking distance, which improved at 4 weeks and at 20 weeks in the STP more than in the LTP group. At 4 weeks the variation (increase) in pain-free walking (PFWD) was 167.8% (of the initial value) in the LTP group and 185% in the STP group (p < 0.05). At 4 weeks the variation (increase) in total walking distance (TWD) was 227.6% of the initial value in the LTP group and 289% in the STP group (p < 0.05). Chronopharmacology and chronotherapy of cardiovascular medications: Relevance to prevention and treatment of coronary heart disease

Biological functions and processes, including cardiovascular ones, exhibit significant circadian (24-hour) and other period rhythms. Ambulatory blood pressure assessment reveals marked circadian rhythms in blood pressure both in normotensive persons and hypertensive patients, whereas Holter monitoring substantiates day-night patterns in electrocardiographic events of patients with ischemic heart disease. The concept of homeostasis, that is, constancy of the milieu interna which has dominated the teaching, research, and practice of medicine during the 20th century, is now being challenged by emerging concepts from the field of chronobiology-the science of biological rhythms. Epidemiologic studies document the heightened morning-time risk of angina, myocardial infarction, and stroke. Circadian rhythms in coronary tone and reactivity, plasma volume, blood pressure, heart rate, myocardial oxygen demand, blood coagulation, and neuroendocrine Function plus day-night patterns in the nature and strength of environmental triggers all contribute to this morning vulnerability Homeostatically devised pharmacotherapies, that is, medications formulated to ensure a near-constant drug concentration, may not be optimal to adequately control diseases that vary in risk and severity during the 24 hours. Moreover, circadian rhythms in the physiology of the gastrointestinal tract, vital organs, and body tissues may give rise to administration-time differences in the pharmacokinetics and effects of therapies. Thus the same medication consumed in the same dose under identical conditions in the evening and morning may not exhibit comparable pharmacokinetics and dynamics. New technology makes possible chronotherapy, that is, increase of the efficiency and safety of medications by proportioning their concentrations during the 24 hours in synchrony with biological rhythm determinants of disease. The frequency of atrioventricular block during adenosine stress testing in young, middle-aged, young-old, and old-old adults

The frequency of atrioventricular (AV) block was evaluated in 600 consecutive patients of different age groups referred for pharmacologic stress testing. Adenosine was infused at 140 mg/kg/min for a total of 6 minutes in conjunction with radionuclide myocardial perfusion imaging. Seventy-five patients (12.5%) were young adults (<49 years old), 214 (35.6%) were middle-aged (50-65 years), 195 (32.6%) were young-old (66-75 years), and 116 patients (19.3%) were old-old (over 75 years). The frequencies of first-degree AV block in young adults, middle-aged, young-old, old-old, and all ages were 14.6%, 9.3%, 14.3%, 17.24%, and 13.16%, respectively. The frequencies of second-degree A V block were 14.65%, 7.0%, 8.7%, 15.5%, and 10.1%, respectively. The frequencies of third-degree AV block were 2.66%, 2.33%, 1.0%, 2.58%, and 2.0%, respectively. Comparison of the incidence of the three types of AV block for each age group vs. all patients showed no statistically significant difference (p>0.08). All types of AV block were of short duration, were well tolerated, and did not require discontinuation of the adenosine infusion or specific treatment. This study confirms the safety of adenosine stress testing in all age groups for evaluation of coronary artery disease and risk assessment. © 2001 by Cardiovascular Reviews & Reports, Inc.
