Intraoperative and laboratory evaluation of skeletonized versus pedicled internal thoracic artery

Background. The skeletonization of internal thoracic artery is postulated to improve graft length early blood flow, sternal blood supply, and postoperative respiratory function. Concern exists that skeletonization may injure internal thoracic artery, precluding good results of surgery. Reports on endothelial function of skeletonized internal thoracic artery are lacking. Methods. A prospective assessment of early clinical outcomes of 357 consecutive patients undergoing coronary artery bypass grafting was performed: 287 patients with nonskeletonized and 70 with skeletonized left internal thoracic artery (LITA). The lengths of LITA and of its discarded distal segment, as well as free LITA blood flow, were measured. The dose-effect relationship for relaxation to acetylcholine was studied in the organ bath. Results. Apart from a higher incidence of breaching the pleura with nonskeletonized LITA the clinical outcomes were comparable. The length of skeletonized LITA was 17.8 +/- 1.14 cm versus 20.3 +/- 0.52 cm skeletonized (p = 0.11). The length of discarded LITA was shorter in nonskeletonized artery (0.8 +/- 0.28 cm versus 2.6 +/- 0.49 cm;p = 0.022). The free LITA blood flow was 66.3 +/- 7.42 mL/min in nonskeletonized vessel versus 100.3 +/- 14.84 mL/min in skeletonized (p = 0.058). The acetylcholine-induced relaxation was similar in both groups (maximal relaxation, 80.7% +/- 5.95% in nonskeletonized versus 72.9% +/- 9.11% in skeletonized;not significant;negative logarithm of half-maximal effect, 7.43 +/- 0.18 versus 7.1 +/- 0.10, respectively;p = 0.063). Conclusions. Skeletonization does not damage the endothelial function of the LITA. Higher free blood flow and available LITA length should encourage the use of skeletonized LITA in clinical practice. Changes in ANP responsiveness of normal and hypertensive porcine intrapulmonary arteries during maturation

Pulmonary vascular resistance falls rapidly after birth, but endothelium-dependent relaxation is relatively poor during the perinatal period. Atrial natriuretic peptide (ANP) is a potent vasodilator;however, its role in the process of perinatal adaptation is uncertain. Porcine intrapulmonary conduit arteries (IPA) from fetal, newborn (<5 min), 3-, 6, and 17-d-old, and adult pigs, and from piglets made hypoxic from 0 to 3, 3 to 6, or 14 to 17 d, were isolated and mounted for isometric force recording. Rings were precontracted with prostaglandin-F-2 alpha (PGF(2 alpha), 10 mu M) or KCl (40 mM). ANP was added cumulatively (10 pM to 100 nM). C-type natriuretic peptide (CNP) was added as a single concentration of 100 nM. Accumulation of cGMP under basal conditions and stimulated by ANP or CNP was measured by radioimmunoassay system. Frozen sections of lung tissue were incubated with I-125-labeled alpha-ANP, and binding site density was assessed on IPA with an image analysis system. ANP relaxed IPA in pigs at all ages, but the effect was significantly greater at 6 and 17 d of age. Hypoxia in animals from 14 to 17 d old impaired ANP-induced relaxation. CNP relaxed IPA poorly: <12% at all ages. ANP increased cGMP accumulation in both normal and hypo:tic animals. CNP did not increase cGMP generation in IPA fi om normal animals but did so in IPA from 3-d-old hypoxic animals. ANP-specific binding sites were demonstrated on the pulmonary artery smooth muscle cells, with greater binding in the young animals. The increased relaxant responses to ANP during adaptation may be important in maintaining low pulmonary vascular resistance. In contrast, CNP was largely ineffective in relaxing pulmonary arteries.

Nitric oxide modulation of the growth hormone-releasing activity of Hexarelin in young and old dogs

The growth hormone (GH)-releasing activity of Hexarelin, a potent GH-releasing peptide (GHRP) analog, was evaluated in eight young (aged 1 to 6 years) and five old (10 to 16 years) beagle dogs pretreated with erythrityl tetranitrate, a liposoluble nitric oxide (NO) donor, and/or indomethacin, an inhibitor of cyclooxygenase enzymes, and N-nitro-L- or N-nitro-D-arginine methylester (L-NAME and D-NAME), active and inactive NO synthase (NOS) inhibitors, respectively, Erythrityl tetranitrate (0.3 mg . kg(-1) oral [PO]) strikingly potentiated Hexarelin-stimulated GH secretion (31.25 mu g . kg(-1) intravenous [IV]) in both young (area under the time-concentration curve at 0 to 90 minutes AUC(0.90)] 878.50 +/- 267.02 v 1,994.04 +/- 434.20 ng . mL(-1) . h, P < .01) and aged animals (314.82 +/- 117.11 v 1,314.12 +/- 484.75 ng . mL(-1) . h, P < .01). The NO donor alone did not modify baseline GH levels in either young dogs (188.68 +/- 85.24 ng . mL(-1) . h) or old dogs (120.49 +/- 22.03 ng . mL(-1) . h). L-NAME (5 mg . hg(-1) x 2 IV) suppressed GH release induced by the peptide in young dogs (1,367.68 +/- 251.87 v 411.12 +/- 68.49 ng . mL(-1) . h, P < .01), but potentiated it in old dogs (314.73 +/- 117.10 v 1,103.97 +/- 374.11 ng . mL(-1) . h, P < .01). D-NAME (5 mg . kg(-1) x 2 IV) did not affect the GH response to Hexarelin in either young (1,328.68 +/- 433.54 ng . mL(-1) . h) or aged (342.32 +/- 84.82 ng . mL(-1) . h) dogs. Indomethacin (1.5 mg . kg(-1) IM) abolished the NO-donor potentiation of the GH response induced by Hexerelin in both young dogs (1,627.25 +/- 260.90 v 1,163.37 +/- 334.84 ng . mL(-1) . h, P < .05) and old dogs (1,061.47 +/- 210.38 v 365.69 +/- 79.27 ng . mL(-1) . h, P < .01) without affecting the plasma GH peak evoked by the peptide alone (young dogs, 786.04 +/- 153.44 v 960.04 +/- 444.44 ng . mL(-1) . h, P = NS;old dogs, 474.55 +/- 47.30 v 490.82 +/- 144.86 ng . mL(-1) . h, P = NS). Endothelial regulation of vascular contraction in radial and internal mammary arteries

Background. The extent to which the endothelium regulates radial artery (RA) contractions is unknown. The goals of this study were to characterize endothelium-dependent relaxations in the RA, compare these responses with those in the internal mammary artery (IMA), and, subsequently, manipulate nitric oxide production in the RA with adenovirus-mediated gene transfer. Methods. Segments of RA and IMA from 43 patients were studied initially in organ chambers. Endothelial function was evaluated and gene transfer, was examined. Results. After precontraction to 80% maximum tension with prostaglandin F-2 alpha, acetylcholine produced lesser relaxations in the RA (21.5% +/- 5.8%) than in the IMA (66.7% +/- 10.6%);human thrombin and adenosine 5'-diphosphate yielded similar results. Reduced relaxations in the RA (16.8% +/-: 4.2%) compared with those IMA (71.6% +/- 11.9%) were noted with calcium ionophore. Superfusion bioassay demonstrated a similar baseline release in both arteries but a reduced stimulated production of vasoactive substances in the RA, results confirmed by cyclic guanosine monophosphate level determination. The RA produced less 6-keto-prostaglandin F-1 alpha than the IMA. Light microscopy demonstrated an intact endothelium in both arteries. Adenovirus-mediated gene transfer of nitric oxide synthase augmented relaxations of the RA to acetylcholine. Conclusions. Reduced production of endothelium-derived relaxing factors suggests diminished endothelial regulation of vascular smooth muscle in the RA compared with the IMA. This finding may explain, in part, the predisposition to vasoconstriction in RA grafts.

Cardiovascular effects of inhaled nitric oxide in a canine model of cardiomyopathy

Background. The inhalation of nitric oxide (NO) in patients with heart failure decreases pulmonary vascular resistance (PVR) and is associated with an increase in pulmonary artery wedge pressure (PAWP). The mechanism for this effect remains unclear. Methods. In dogs rapid-paced for 8 weeks to induce cardiac dysfunction, we performed left ventricular pressure-volume analysis of unpaced hearts in situ to determine whether during NO inhalation (80 ppm), the mechanism for the rise in PAWP is due to: 1) primary pulmonary vasodilation;2) a direct negative inotropic effect;or 3) impairment of ventricular relaxation. Results. Inhalation of NO decreased PVR by 51% +/- 3.8% (257 +/- 25 vs 127 +/- 18 ***(-5) [NO 80 ppm];p < 0.001) and increased PAWP (15.4 +/- 2.4 vs 18.1 +/- 2.6 mm Hg [NO 80 ppm];p < 0.001). Calculated systemic vascular resistance remained unchanged. Left ventricular (LV) end-diastolic pressure rose (16.4 +/- 1.9 vs 19.1 +/- 1.8 mm Hg [NO 80 ppm];p < 0.001), as did LV end-diastolic volume (83.5 +/- 4.0 vs 77.0 +/- 3.4 mt [NO 80 ppm];p = 0.006). LV peak +dP/dt was unchanged by NO (1,082 +/- 105 vs 1,142 +/- 111 mm Hg/sec [NO 80 ppm];p = NS). There was a trend toward a stroke volume increase (17.4 +/- 1.2 vs 18.8 +/- 1.3 mL;p = NS), but the relaxation time constant and end-diastolic pressure-volume relation were both unchanged. Conclusions. In this canine model of cardiomyopathy, inhaled NO decreases pulmonary vascular resistance. The associated increase in left ventricular filling pressure appears to be secondary to a primary pulmonary vasodilator effect of NO without primary effects on the contractile or relaxation properties of the left ventricle. Nitric oxide donors selectively potentiate thrombin-stimulated p70^S6k activity and morphological changes in Swiss 3T3 cells

Thrombin stimulates both DNA synthesis and cell morphological changes in Swiss 3T3 cells, although the mechanism of signal coordination leading to these responses is unknown, We report here that nitric oxide (NO) donors selectively enhance thrombin-stimulated p70(S6k) activity by 40-60%, an effect that was sustained for 24 h, Potentiation of p70(S6k) also was observed with cGMP analogues indicating that this effect is mediated by cGMP-activated protein kinase, NO donors also induced morphological changes characterized by spindle-shaped cells in confluent, nondividing cells or by extended protrusions from the trailing edge in subconfluent, polarized cells. NO donors had no significant effects on intracellular Ca2+ mobilization, DNA synthesis, proliferation, or ERKs 1 and 2 and p90RSK activities, indicating that mitogenic responses and cell division are not altered by NO donors. We conclude that NO donors modulate the morphological changes associated with cellular motility in response to thrombin stimulation through selective enhancement of p70(S6k) activity. Endothelium-derived nitric oxide enhances the effect of intraaortic balloon pumping on diastolic coronary flow

Background. High shear rate with pulsation is one of the major stimuli for the release of endothelium-derived nitric oxide leading to coronary arteriolar dilation. Intraaortic balloon pumping mechanically enhances shear rate and diastolic-to-systolic now oscillation. We aimed to evaluate whether or not coronary blood flow augmentation during intraaortic balloon pumping is mediated by coronary arteriolar dilation through endothelium-derived nitric oxide release. Methods. Using a charge-coupled device intravital videomicroscope, we observed epicardial coronary arterioles (40 to 220 mu m in diameter) in anesthetized open-chest dogs (n = 10) during 2:1 mode of intraaortic balloon pumping. Endothelium-derived nitric oxide-mediated vasodilatory effects of intraaortic balloon pumping were evaluated by comparing end-diastolic arteriolar diameters between the coupled beats of on and off intraaortic balloon pumping before and after intracoronary endothelium-derived nitric oxide synthesis inhibition with N-omega-nitro-L-arginine (L-NNA, 2 mu mol/min) administration. Results. Intraaortic balloon pumping increased coronary arteriolar diameters and coronary blood now by 11.4% +/- 1.8% (p < 0.0001) and 33.4% +/- 4.1% (p < 0.001), respectively. Vasodilation was greater in small arterioles (<110 mu m;15.4% +/- 2.2%) than in large arterioles (greater than or equal to 110 mu m;4.2% +/- 1.2%, p < 0.0001). L-NNA attenuated the intraaortic balloon pumping-induced vasodilation and augmentation of coronary blood flow to 4.6% +/- 1.0% (p < 0.001) and to 20.8% +/- 2.1%, (p < 0.05), respectively. Attenuation of vasodilatory effect by L-NNA was observed mainly in small arterioles (from 15.4% +/- 2.2% to 5.9% +/- 1.2%). Conclusions. Intraaortic balloon pumping augmented coronary blood flow by dilating coronary arterioles in diastole, more significantly in small arterioles than in large arterioles. Effects on coronary endothelial function of the Cohn stabilizer for beating heart bypass operations

Background. The effects of different stabilizing methods for minimally invasive beating heart coronary artery bypass grafts (CABG) on coronary endothelial function are unknown. Methods. We compared the effects on endothelial function of the Cohn stabilizer (used with proximal snaring by Retract-o-tape silicone air cushion) and a coronary shunt (CTS flow coil shunt) on an in vivo model of beating heart CABG. The two techniques were applied for 15 minutes on porcine epicardial coronary arteries. Control rings were taken from the same coronary artery. Endothelial function of control and instrumented arterial rings was studied in organ chamber experiments. Evaluation of endothelial coverage was performed with silver nitrate staining. Results. Endothelium-dependent relaxation to serotonin and bradykinin was significantly decreased in the shunt group compared to control, Cohn stabilizer, and snare groups. There were no significant differences in the endothelium-independent relaxation to sodium nitroprusside between groups. Conclusions. Greater endothelium-dependent relaxation with the Cohn stabilizer suggests better preservation of endothelial coverage at the site of application and reduced propensity for coronary spasm and later development of intimal hyperplasia.
