Dental radiographs constitute 25% of all exposures made in the UK and therefore in terms of radiation protection the population risk is not insignificant(1). Paragraph 11 of The Core of Knowledge in the Schedule to the Protection of persons undergoing medical examination or treatment (POPUMET) regulations(2) specifies the importance of using existing radiological information, whether films or reports, about a patient. The Guidance Notes(3) recommends that in order to reduce unnecessary radiographic examinations, there should be ready availability of previous radiographs. Access to previous radiographs is also commended by the National Radiological Practice Board (NRPB) and the Royal College of Radiologists (RCR) in their report on Patient Dose Reduction in Diagnostic Radiology(4). In addition to dose limitation, patient care is improved.

Sir: I read with interest the original article Introduction of a proforma in the management of under-aged attendees at a genitourinary clinic\'1 published in the April 2005 issue. We also introduced an under-16 policy and a proforma in October 2003 to improve the management anddocumentation of the consultations of under-16 attendees at The Sandyford Initiative in Glasgow. This included its use in family planning clinics, the genitourinary (GU) medicine service, and \'The Place\', a dedicated young person\'s *** audited the management and documentation before and after the introduction of the proforma in GU medicine over two six-month *** results are shown in Table *** can be seen from Table 1, recording of essential information did increase significantly although there was an extremely low completion rate of the actual proforma. This improvement is most likely due to the increased awareness of the specific issues involved with this client group. This has occurred through our induction training and also through regular teaching sessions for all medical, nursing and clerical staff.
