The transmission of moving pictures to remote locations is an important part of telemedicine. Although the first videoconferencing demonstrations were performed in the 1930s, the technology is still fragmented and its quality is sometimes too poor for it to be useful. Conventional television technology is analogue (the fundamental television standards were developed before the Second World War) and does not 'fit' the digital world very well. This paper reviews video transmission and videoconferencing technologies and the results that can be expected. While trained professionals may be able to make use of poor-quality video systems, real advances in telemedicine require studio-quality video, which is possible only with high-bandwidth technology.

The Lancashire telemedicine ambulance

An emergency ambulance was equipped with three video-cameras and a system for transmitting slow-scan video-pictures through a cellular telephone link to a hospital accident and emergency department. Video-pictures were transmitted at a resolution of 320 x 240 pixels and a frame rate of 15 pictures/min. In addition, a helmet-mounted camera was used with a wireless transmission link to the ambulance and thence the hospital. Speech was transmitted by a second hand-held cellular telephone. The equipment was installed in 1996-7 and video-recordings of actual ambulance journeys were made in July 1997. The technical feasibility of the telemedicine ambulance has been demonstrated and further clinical assessment is now in progress.

Design and implementation of a telemedicine system in Palestine

The shortage of expert physicians and health-care providers in Palestine has created the need for a telemedicine system. The difficulty and the cost of transfer between Palestinian cities in the current political situation makes it important to use such systems. This paper describes the project's progress. The software and hardware required were developed in our laboratories. The software included modules for the preprocessing, enhancement, manipulation, compression and decompression of medical images. The hardware included the communication protocols, physical layer interface circuit and selection of a suitable medium for communication. In tests a radiograph was transmitted from one computer to another. The second and third phases will be to establish a communication link between Palestinian hospitals at different locations and to examine the realtime transfer of medical images.

The telescience testbed experiments were carried out to test and investigate the tele-manipulation techniques in the intracellular potential recording of amphibian eggs. Implementation of telescience testbed was set up in the two separated laboratories of the Tsukuba Space center of NASDA, which were connected by tele-communication links. Manipulators respective for a microelectrode and a sample stage of microscope were moved by computers, of which command signals were transmitted from a computer in a remote control room. The computer in the control room was operated by an investigator (PI) who controlled the movement of each manipulator remotely. A stereoscopic vision of the microscope image were prepared by using a head mounted display (HMD) and were indispensable to the intracellular single cell recording. The fertilization potential of amphibian eggs was successfully obtained through the remote operating system.

A review of minor injuries telemedicine

Video support in the management of minor injuries was pioneered in the mid-1970s in the USA, but remained little more than a technical curiosity pending the development of acceptable equipment some 20 years later. Minor injuries telemedicine has developed very successfully over the last few years in the UK, the first reported UK minor injuries telemedicine link being set up in 1994. Peripheral services are generally staffed by emergency nurse practitioners, who can use a telemedicine link when required to obtain realtime advice from a doctor at a main hospital accident and emergency department. There is now a considerable body of experience to show that the technique is safe and effective, and also some limited data about its economic benefits. The majority of minor injuries teleconsultations involve transmission of radiographs and most minor injuries teleradiology is undertaken on the understanding that a definitive radiologist's report will be issued in due course. Studies show that satisfactory interpretation of plain radiographs is possible using a low-cost/low-resolution telemedicine link. This is supported by other studies which have shown that even a teleradiology system at reduced cost and technical specifications can yield adequate images. It seems likely that teleconsultation will become an essential component in the provision of accident and emergency services.

Development of a Web-based picture archiving and communication system using satellite data communication

Using the JAVA language we have developed a Web-based picture archiving and communication system (PACS) which allows a remote hospital to access medical images. An asymmetric satellite data communication system (ASDCS) provided a receive-only link for data delivery and a conventional terrestrial link (which could be the conventional telephone network) allowed data transmission. The satellite communication link was 10-30 times faster than the conventional terrestrial link. To increase image transmission speeds over the Internet connection, JPEG and wavelet compression methods were used. The resulting images were evaluated quantitatively by measuring the peak signal:noise ratio and qualitatively by radiologists. Compression ratios of 10:1 or less were deemed acceptable for diagnostic purposes. The system appears to be suitable for teleradiology and telemedicine.

Videoconferencing: what are the benefits for dental practice?

For more than 3 years members of the TeleDent team from Bristol University have been looking at the potential of videoconferencing technology for dentistry Here they explain what videocanferencing is and how it can help the GDP. They discuss examples of its most promising uses for the profession, which include professional updating and providing diagnostic support at a distance. They describe the equipment that is needed, the different types of system available and give an indication of costs. A suggested procedure for using the technology for remote referrals is outlined. 'Store and forward' techniques are also discussed. These do not involve a live video but involve the sending of static electronic files. This approach is compared with videoconferencing, and the article looks at the question of which will be best suited to the GDP, and for what purposes.

Moving the information instead of the patient is often sufficient and telemedicine is a useful and potentially powerful tool which fulfils this idea. It has changed the classical form of health-care delivery by providing those people living in rural and remote areas with comparable services to those in urban areas. Equally, remote hospitals can obtain specialist consultations and diagnosis without the need for the patient to travel. We have reviewed the present status of telemedicine and its future possibilities.

Low-cost ISDN videoconferencing equipment for orthopaedic second opinions

We validated the application of low-cost ISDN videoconferencing equipment for telemedicine. A telemedicine benchmark was designed and four different office videoconferencing systems were evaluated, all using basic-rate ISDN connections (128 kbit/s). All the low-cost systems showed generally good or acceptable performance for clinical use. We also investigated the feasibility of videoconferencing for an orthopaedic second-opinion service. Eight point-to-point conferences were conducted to discuss real clinical cases by use of interactive sharing of medical images. The average duration of each session was 35 min. Encouraging results were obtained.
