目的:探讨W T 1基因与骨髓增生异常综合征(MDS)发病和病情进展的关系及临床意义。方法:应用逆转录-聚合酶链反应(RT-PCR)方法,检测53例MDS患者和10例正常人骨髓中W T 1mRNA表达,并进行半定量分析。结果:MDS患者W T 1mRNA表达阳性率和表达水平均高于正常对照组。W T 1mRNA表达阳性率在MDS-RA和MDS-RA S组,明显低于MDS-RAEB和MDS-RAEB-t组,其表达水平从MDS-RA和MDS-RA S组向MDS-RAEB组和MDS-RAEB-t组逐渐升高。结论:W T 1基因表达与MDS病情发展有关。W T 1基因与疾病进展关系密切,是临床上对MDS病情的高危评估、预测病情变化和动态监测治疗效果及判断预后的重要指标。

One of the most common structural rearrangements in myelodysplastic syndrome (MDS) is a deletion of the long arm of chromosome 5, del(5q). The 5q- syndrome is a distinct entity, that presents with specific morphologic abnormalities of the megakaryocytic lineage. Thus, we evaluated the presence or absence of the del(5q) in these cells. We performed fluorescence in situ hybridization analysis using unique sequence probes (one for 5q31, the other for the 5p telomeric band), and tested bone marrow specimens from 10 patients with MDS (including 6 patients with the 5q- syndrome) and a del(5q). Megakaryocytes were identified by nuclear morphology, size, and ploidy index. Our results demonstrate the presence of the del(5q) in the megakaryocytic lineage and, thus, the involvement of these cells in the disease process. (C) 2000 Wiley-Liss, Inc.

The 5q- syndrome is a distinct subtype of myelodysplastic syndrome (MDS) characterized by refractory anemia, deletion of the long arm of chromosome 5, del(5q), as the sole cytogenetic abnormality, and a low frequency of transformation to acute leukemia. Using combined immunophenotyping and fluorescence in situ hybridization (FISH), studies were carried our, on bone marrow smears of three 5q- syndrome cases to identify the cell lineages carrying the 5q deletion. In all three cases, the granulocytic, monocytic, and erythroid lineages possessed the del(5q) clonal marker, whereas the T-lymphocytes did not. interestingly, in one case, cells of B-lymphoid lineage also showed the presence of the del(5q). This is the first report to date showing involvement of an acquired 5q deletion associated with MDS in B-cells. This result suggests chat in some cases, MDS arises in a multipotent cell with a capacity to differentiate into both myeloid and lymphoid cells. (C) 2000 Wiley-Liss, Inc.

Intramedullary apoptosis of hematopoietic cells in myelodysplastic syndrome patients can be massive: apoptotic cells recovered from high-density fraction of bone marrow aspirates

A higher percentage of apoptotic cells (apoptotic index or Al) is consistently found in bone marrow (BM) biopsies compared to BM aspirates of patients with myelodysplastic syndrome (MDS). Most studies have only investigated the low-density fraction (LDF) mononuclear cells from BM aspirates following density separation for Al determination, In the present study, both LDF and high-density fraction (HDF) cells for Al were examined by electron microscopy (EM) in 10 MDS patients and 4 healthy donors, Matched BM biopsies were subjected to Al detection by in situ end labeling (ISEL) of fragmented DNA, The results indicate that in LDF and HDF cells, Al is consistently higher in MDS patients (8.5% vs 1.5%, respectively;P=.039) compared to healthy donors (27% vs 4%, respectively;P=.004). The BM biopsy Al was also higher in MDS patients than in healthy donors (3+ vs 0+, respectively;P =.036). In addition, in MDS patients, more apoptotic cells were found in HDF cells than in LDF cells (27% vs 8.5%, respectively;P=.0001). All stages of maturation, ranging from blasts to terminally mature cells belonging to all 3 lineages, were represented in the dying cells in both compartments. Using EM, typical Pelger-Huett-type cells appeared to be apoptotic granulocytes. Both LDF and HDF cells should be examined for an accurate estimation of apoptotic cells because Al would be underestimated if only the LDF cells were studied, Ultrastructural studies consistently show a higher Al in BM biopsies compared to BM aspirates despite the correction factor of HDF cells provided by Al, This may represent the actual extant state, which could conceivably be due to a higher concentration of proapoptotic signals in the biopsies. Histological, enzyme histochemical and ultrastructural findings in three cases of feline bone marrow neoplasia are described. The following changes were observed: in myelodysplastic syndrome (MDS), a low medullary blast count, strongly atypical (micromegakaryocytic) proliferative megakaryocytopoiesis, hypoplastic erythrocytopoiesis with impairment of differentiation, multifocal extravasation and lymphoid aggregates;in acute myeloid leukaemia (AML), medullary proliferation of undifferentiated cell types;in chronic myeloproliferative disorder (CMPD), trilinear medullary proliferation with complete cellular maturation, osteomyelosclerosis and extramedullary haemopoiesis. In two cases (MDS, AML), ultrastructural demonstration of C-type virus particles (feline leukaemia virus) suggested a viral aetiology. (C) 1999 Harcourt Publishers Ltd.

骨髓增生异常综合征(MDS)是由一种造血干细胞的恶性克隆性疾病,以外周血血细胞减少,骨髓病态造血为特征,有明显的白血病转化倾向。40%～70%的原发性 MDS 和95%的继发性 MDS 有克隆性染色体异常。与慢性粒细胞白血病(CML)仅有一种细胞遗传学异常(Ph 染色体)形成鲜明对照的是,MDS 细胞遗传学异常存在极大的异质性,影响 MDS 预后分类及细胞遗传学异常的分子背景研究,同时,不同于AML,易位及倒位等平衡结构异常在 MDS 少见。染色体核型分析对于 MDS 的诊断、鉴别诊断、预后判断和克隆消长的监测等具有十分重要的意义。

45例骨髓增生异常综合征患者SF3B1、TET2、RUNX1及TP53基因突变分析

目的研究骨髓增生异常综合征(myelodysplastic syndromes,MDS)患者splicing factor 3B subunit 1(SF3B1)、tet methylcytosine dioxygenase 2(TET2)、runt-related transcription factor 1(RUNX1)及tumour suppressor protein p53(TP53)基因突变的临床特征及意义。方法回顾性分析45例初治MDS患者临床资料,采用PCR扩增产物直接测序法分别检测患者SF3B1、TET2、RUNX1及TP53基因的突变情况。结果 45例患者中位年龄为62岁(42~72岁),其中男性28例(62.2%),女性17例(37.8%)。SF3B1基因突变者8例(17.8%),TET2基因突变9例(20.0%),核型异常16例(35.6%),RUNX1基因突变者3例(6.7%),TP53基因突变者4例(8.9%)。SF3B1、TET2基因突变及核型异常比例在性别及年龄方面比较,差异均无统计学意义(均P>0.05)。结论 MDS患者常伴有SF3B1、TET2、RUNX1及TP53突变,多数患者常合并染色体异常,环形铁粒幼细胞增多性贫血多有SF3B1基因突变。
