Given the high prevalence of the atherogenic forms of apolipoprotein B (apoB) dyslipoproteinemias and the effectiveness of appropriate therapy, we feel that access for all physicians to simple, effective diagnostic aids that enable use of widely available technology is vitally important ill clinical lipidology. In this Review, therefore, we present a diagnostica* algorithm for the diagnosis of these disorders that is based on concentrations of total cholesterol, triglyceride and apoB. By including apoB values, lipoprotein number and composition can be deduced and each of the classic dyslipoproteinemias identified. All three parameters can be accurately and inexpensively determined ill clinical laboratories and, therefore, thisa* algorithm can be used by any physician to make an accurate diagnosis without use of specialist research laboratories. just as the application of LDL cholesterol measurement moved clinical practice forward from plasma lipids to lipoprotein lipids, we believe that the use of apoB will further advance diagnosis and treatment of dyslipoproteinemias.

Purpose: To investigate the efficacy of a newa* algorithm to increase the volume of tissue ablation via gradual ramp-up of power using an internally cooled electrode for ablating hepatomas 3 cm or less. Materials and Methods: 44 patients with 62 hepatomas were treated from March 4, 2004 to May 24, 2009. Ablation with a gradual ramp-up of power was performed using a single needle with an internally cooled electrode. Evaluation for tumor response was performed with 4-phase CT at 24 hours and 3 months. All immediate and follow-up complications were recorded. Results: Complete tumor ablation was achieved in 86%. The ablation volumes were 16 cm3 +/- 12 cm3 for tumors 3 +/- 12 cm3 for tumors 2 - 3 cm. Out of 68 total ablation sessions, there were 2 major complications (pleural effusions) requiring intervention (thoracentesis). Conclusion: Compared with existing techniques using a constant full-power setting, ablation of small hepatomas using ana* algorithm of gradual ramp-up of power provides comparable rate of complete tumor ablation, adequate ablation volumes, and a low rate of complications that require treatment.

This paper summarizes the optimization process for deflectors designed to lower the water level and ceiling spillage in the S-shaped outflow channel of HPP Vinodol. Parameterized designs of ten deflectors were optimized using a genetica* algorithm that evaluated each setup by running 2D CFD simulations. Optimal designs were chosen by considering total pressure loss and flow uniformity. The resulting best case design set showed a 16.7% reduction in energy losses providing a solid basis for future simulations using a 3D fluid flow numerical model of the channel.
