DcE attribute development investigating public policy for the provision of medicinal cannabis

BackgroundDefining attributes and attribute levels for a discrete choice experiment (DcE) poses a significant challenge for practitioners exploring preferences for new or unfamiliar products due to the dearth of available information and limits in stakeholder knowledge. This study outlines a comprehensive process for identifying attributes and levels in a DcE aimed at gauging public preferences for health policies related to medicinal cannabis (Mc).MethodsA rigorous four-stage attribute development process was utilized and included i) the formulation of a preliminary attribute list from a scoping review and document analysis, ii) reduction of attributes via focus groups, iii) removal of inappropriate attributes using Delphi studies and research team knowledge, and iv) refinement of attribute language based on the feedback from think-aloud *** base attribute list formed from the scoping review and document analysis served as effective discussion stimuli in focus groups, especially for participants with limited subject knowledge. Structured focus group activities proved more effective than open-ended discussions in engaging naive participants. Delphi studies were found to be overcomplex for expert-led attribute prioritization. Think-aloud interviews during a pilot DcE were essential for assessing attribute language and clarity and understanding participants' decision-making *** development of DcE attributes measuring preferences for an unfamiliar health product requires a multi-method approach. Evaluating the outcomes from various methods of attribute development yielded a refined list of attributes that were significant to stakeholders, allowed for meaningful trade-offs, and were presented in language accessible to the target population.

An updated cost-effectiveness analysis of axicabtagene ciloleucel in second-line large B-cell lymphoma patients in the United States

AimsThis economic evaluation of axicabtagene ciloleucel (axi-cel) versus previous standard of care (SOc;salvage chemotherapy followed by high-dose therapy with autologous stem cell rescue) in the second line (2L) large B-cell lymphoma population is an update of previous economic models that contained immature survival *** analysis is based on primary overall survival (OS) ZUMA-7 clinical trial data (median follow-up of 47.2 months), from a United States (US) payer perspective, with a model time horizon of 50 years. Mixture cure models were used to extrapolate updated survival data;subsequent treatment data and costs were updated. Patients who remained in the event-free survival state by 5 years were assumed to have achieved long-term remission and not require subsequent *** survival and quality of life benefits were observed despite 57% of patients in the SOc arm receiving subsequent cellular therapy: median model-projected (ZUMA-7 trial Kaplan-Meier estimated) OS was 78 months (median not reached) for axi-cel versus 25 months (31 months) for SOc, resulting in incremental quality-adjusted life year (QALY) difference of 1.63 in favor of axi-cel. Incrementally higher subsequent treatment costs were observed in the SOc arm due to substantial crossover to cellular therapies, thus, when considering the generally accepted willingness to pay threshold of $150,000 per QALY in the US, axi-cel was cost-effective with an incremental cost-effectiveness ratio of $98,040 per *** remained consistent across a wide range of sensitivity and scenario analysis, including a crossover adjusted analysis, suggesting that the mature OS data has significantly reduced the uncertainty of axi-cel's cost-effectiveness in the 2L setting in the US. An efficient continuous-flow (cF) protocol was designed for the hydrogenation of lignin-derived aromatics to the corresponding cycloalkanes derivatives. A parametric analysis of the reaction was carried out by tuning the temperature, the H-2 pressure and the flow rate, and using diphenyl ether (DPE) as a model substrate, commercial Ru/c as a catalyst, and isopropanol as a solvent: at 25 degrees c, 50 bar H-2, and a flow rate of 0.1 mL min(-1), dicyclohexyl ether was achieved in an 86 % selectivity, at quantitative conversion. By-products from the competitive c-O bond cleavage of DPE, cyclohexanol and cyclohexane, did not exceed 14 % in total. Remarkably, prolonged experiments demonstrated an excellent stability of the catalyst whose performance was unaltered for up to 420 min of time-of-stream. A substrate scope evaluation proved that under the same conditions used for DPE, a variety of substrates including alkoxy-, allyl-, and carbonyl-functionalized phenols, biphenyl, aryl benzyl- and phenethyl ethers (10 examples) yielded the ring-hydrogenated products with selectivity up to 99 % at complete conversion.

Real-world total cost of care by line of therapy in relapsed/refractory diffuse large B-cell lymphoma

AimsThere are multiple recently approved treatments and a lack of clear standard-of-care therapies for relapsed/refractory (R/R) diffuse large B-cell lymphoma (DLBcL). While total cost of care (Tcc) by the number of lines of therapy (LoTs) has been evaluated, more recent cost estimates using real-world data are needed. This analysis assessed real-world Tcc of R/R DLBcL therapies by LoT using the IQVIA PharMetrics Plus database (1 January 2015-31 December 2021), in US patients aged >= 18 years treated with rituximab plus cyclophosphamide, doxorubicin, vincristine, and prednisone (R-cHOP) or an R-cHOP-like regimen as first-line *** costs and resources in the R/R setting were assessed by LoT. A sensitivity analysis identified any potential confounding of the results caused by the impact of the cOVID-19 pandemic on healthcare utilization and costs. Overall, 310 patients receiving a second- or later-line treatment were included;baseline characteristics were similar across LoTs. Inpatient costs represented the highest percentage of total costs, followed by outpatient and pharmacy *** Tcc per-patient-per-month generally increased by LoT ($40,604, $48,630, and $59,499 for second-, third- and fourth-line treatments, respectively). costs were highest for fourth-line treatment for all healthcare resource utilization categories. Background current paediatric cancer care requires innovative approaches to predict prognosis that facilitates personalised stratification, yet studies on the performance, composition and limitations of contemporary prognostic models are lacking. We aimed to compare the accuracy of traditional and advanced prognostic models. Methods A systematic search for this systematic review and meta-analysis (cRTN42022370251) was conducted in PubMed, Embase, Scopus, and the cochrane Library databases on 28 June 2024. Studies on the accuracy of prognostic markers or models used in paediatric haematological malignancies, central nervous system (cNS), or non-cNS solid tumours (NcNSST) were included. Three model categories were defined using: 1-clinical parameters, 2-genomic-transcriptomic data, and 3-artificial intelligence (AI). Primary outcomes were area under the receiver operating characteristic curve with a 95% confidence interval (cI) for various overall survival intervals and event-free survival. Two independent groups performed selection and data extraction. We used data published by the authors and publicly available databases. Findings Of 12,982 studies, 358 were included in the meta-analysis and 27 in the systematic review, with limited data on AI-approaches. Most data were reported on NcNSST at 5-year OS, where a statistically significant difference was observed between category-1 (0.75 cI: 0.72-0.79) and category-2 (0.85 cI: 0.82-0.88) (p < 0.001), but not between categories-2 and-3 (p = 0.2834) (0.82 cI: 0.77-0.88). Internal validation studies showed significantly better performance compared to those using external validation, highlighting the high risk of bias (ROB) inherent in internal validation. Developing a high-efficiency predictive model for self-temperature-compensated piezoresistive properties of carbon nanotube/graphene nanoplatelet polymer-based nanocomposites

The thermo-resistive and piezoresistive responses of carbon nanotube (cNT)/graphene nanoplatelet (GNP) polymer-based nanocomposites are analytically investigated. The 3D representative volume element is generated by the Monte carlo approach to incorporate the random distribution of nanofillers. The Monte carlo approach is paired with the percolation model to investigate the percolation behavior of the nanocomposite. The validity of the analytical model is verified by comparing the predicted results with the experimental data. The Poisson's ratio and height of barrier potential influence on the piezoresistivity of nanocomposite are studied. Analytical results determine the aspect ratio and influence of carbon nanotube degree of orientation on thermoresistivity of nanocomposite. The effects of intrinsic and physical properties of GNPs on resistivity change with temperature are investigated. It is found that nanocomposite filled with cNTs presented lower percolation threshold than those filled with GNPs. The results also revealed that the filler alignment caused a higher piezoresistivity.

The effect of different rare earth elements (Gd, Y, Nd) with similar atomic percentage on the corrosion behavior of Mg-1.52Zn (at.%) was investigated. The secondary phases and corrosion film formed on the surface of the alloys during immersion in aqueous Nacl solution affects the corrosion behavior of the alloy. The electrochemical, weight loss and hydrogen evolution tests revealed that corrosion rate of Mg-1.52Zn-0.15Gd was the highest and the corrosion rate of other three alloys were almost similar. The discrete second phases in Mg-1.52Zn-0.15Gd acted as severe cathodes to the matrix and accelerated the micro-galvanic corrosion whereas network shaped and uniformly distributed phases in Mg-1.52Zn-0.15Nd alloy effectively retarded corrosion. The immediate formation of Nd2O3 in the corrosion film of Mg-1.52Zn-0.15Nd alloy also contributed in improved corrosion resistance. Dominant presence of Y2O3 in Mg-1.52Zn-0.16Yalloy contributed to its better corrosion resistance.

Principles for quorum sensing-based exogeneous denitrifier enhancement of nitrogen removal in biofilm: a review

Denitrifying bacteria play critical roles in enhancing biological biofilm denitrification for nitrogen removal in industrial and municipal wastewater treatment and groundwater remediation. Evidence intensively confirms that exogenous quorum sensing (QS) bacteria prompt biofilm formation and denitrification. Yet, exogenous bacteria forfeiting during bioaugmentation may occur despite of its priority effect and improved community function, because of open, intricate traits in wastewater treatment system. critical topics include regulating signal-based microecological structure, effectively assembling 3D biofilm, and regulating carbon and nitrogen metabolization of denitrifiers to reduce cO2 and N2O emissions. This review demonstrates classifications of denitrifiers and the vital role of QS in regulating biofilm denitrification. Furthermore, the ecological principles of microbial community construction with the addition of exogenous denitrifiers are summarized. Moreover, the effects of signaling molecules on carbon and nitrogen transcription and metabolization are discussed. In conclusion, technologies based on quorum sensing for enhanced denitrification have high application potential and challenges. Herein, we provide a guideline for enhancing nitrogen removal by exogenous quorum-sensing bacteria and synthetic biofilms design during upgrading of municipal wastewater treatment plants.

Thermochromic properties of pure NiTiO3 and its cu- or co-doped derivatives

Nickel titanate is a yellow-brown pigment which adopts the ilmenite crystal structure as corundum and hematite. Since cr doped Al2O3 and Fe2O3 present a thermochromic behavior, NiTiO3 was also studied for its potential ability to change colour with temperature. Powders were synthesized via a modified Pechini method followed by a calcination step at 700 degrees c. The thermochromic properties in the 20 degrees c-400 degrees c temperature range were collected via the determination of the colorimetric L*a*b* parameters. Namely, the hue continuously shifts from yellow-brown to red-brown, with L*a*b* parameters moving from 60.4/19.2/58.5 to 42.9/30.5/43.1. This colour change is fully reversible even after several cycling. Temperature dependent UV-vis spectroscopy showed that the transparency window at 550-650 nm at room temperature shifts to 575-650 nm at 400 degrees c. This fully explains the observed colour change. Finally, the effects of cu or co doping on NiTiO3 optical and thermo-chromic properties were also investigated.

Nanosized Hf6Ta2O17 particles reinforced Hfc ceramic coating for high temperature applications

There is an urgent need for the thermal protection of carbon/carbon composites to increase their service span in aerospace field. As members of ultra-high temperature ceramics, Hfc and Zrc are thought to be alternative materials, but their poor oxidation behavior and fracture toughness at temperature over 2000 degrees c prevent them from taking full advantages. Herein, we synthesized nanosized Hf6Ta2O17 powders and introduced them into Hfc ceramic to tackle the above problems. Plasma-sprayed Hfc coatings with Hf6Ta2O17 varied from 0 to 15 mol.% (0, 1.25, 2.5, 5, 15) were exposed to an oxyacetylene torch with a heat flux of 2.38 MW/m2. The one with 2.5 mol.% Hf6Ta2O17 showed the best ablation resistance. Smaller doping was unable to effectively hinder oxygen diffusion given the inadequate compactness of the formed scale, conversely substantial humps and ruptures formed on the samples with higher amounts, acting as straightforward paths for oxygen.
