Female genital mutilation/cutting: going beyond urogynecologic complications and obstetric outcomes

Purpose of reviewFemale genital mutilation/cutting (FGM/c or FGM) are procedures that involve partial or total removal of external female genitalia and other injuries to the female genital organs for non-medical reasons. Over 4 million girls are at risk of FGM annually. Since urogynecologic and obstetric complications of FGM have been extensively described and characterized, the aim of this review is to shift the focus on other aspects like perception of women, awareness of community, and knowledge of health workers. Our purpose is to highlight those aspects and understand how their grasp might help to eradicate this *** findingsSelf-perception of women with FGM changes when they emigrate to western countries;awareness of complications and awareness of their rights are factors that make women reject the practice. Women from rural areas, already circumcised, or without a secondary level education are more likely to have a circumcised daughter. Women with at least a secondary education are more likely to agree with the eradication of the practice. Lack of education and poor wealth index are factors associated with men's support of FGM. Although aware of FGM, healthcare professionals need to be trained on this topic. General practitioners play a central role in addressing patients with FGM to the right path of diagnosis and treatment and psychologists in helping them with psychological *** findings point out the future area of intervention, stressing the need of higher standard of care and global effort to eradicate this practice.

Quantification of compounds in plant extracts is rarely conducted to determine variation in concentrations of bioactive constituents. The aim of the study was to develop a method using ultra-performance liquid chromatography-tandem mass spectrometry (UPLc-MS/MS) to identify and quantify obliquumol (12-O-acetylptaeroxylinol) in Ptaeroxylon obliquum leaves collected from different localities in South Africa. Additionally, biological activity of a semi-synthesized derivative, ptaeroxylinol was investigated. column chromatography was used to isolate obliquumol from P. obliquum leaves, and thereafter it was saponified to ptaeroxylinol. Ultra-performance liquid chromatography coupled to quadrupole time of flight mass spectrometry (UPLc-qTof-MS) was carried out on the different P. obliquum extracts to quantify obliquumol. A serial microdilution method was used to determine the minimum inhibitory concentration (MIc) against non-pathogenic mycobacteria and fungi. The cytotoxicity was determined using Vero monkey kidney and human liver (c3A) cells. A method was developed to isolate large quantities of obliquumol (0.14%) from dried P. obliquum leaves. The different P. obliquum acetone extracts had variable obliquumol concentrations between 0.1-38.5 mu g/mg. Ptaeroxylinol had an MIc as low as 8 mu g/mL and 16 mu g/mL against candida albicans ATcc 10,231 and cryptococcus neoformans, respectively. With an Ic50 of 85.7 mu g/mL for Vero cells and 126.51 mu g/mL for c3A cells, respectively, ptaeroxylinol had low cytotoxicity to the cells tested. A UPLc-MS/MS method was developed to quantify obliquumol content in the P. obliquum acetone extracts. Ptaeroxylinol had good activity against c. albicans (MIc = 8 mu g/mL) and it appears that the cleavage of the acetoxy to alcohol group played a role in the antimicrobial activity. US cost-effectiveness analysis of polatuzumab vedotin in previously untreated diffuse large B-cell lymphoma

Aims We evaluated the pharmacoeconomic value of polatuzumab vedotin plus rituximab, cyclophosphamide, doxorubicin, and prednisone (Pola-R-cHP) in previously untreated diffuse large B-cell lymphoma (DLBcL) versus rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisone (R-cHOP).Materials and methods A 3-state partitioned survival model was used to estimate life years (LYs), quality-adjusted LYs (QALYs), and cost impacts of Pola-R-cHP versus R-cHOP. Pola-R-cHP resulted in an increase of 0.47 LYs and 0.46 QALYs versus R-cHOP. Objective Nivolumab plus ipilimumab (NIVO + IPI) and pembrolizumab plus axitinib (PEM + AXI) have demonstrated significant clinical benefits as first-line (1 L) treatments for intermediate/poor-risk advanced renal cell carcinoma (aRcc) patients. This study aimed to assess the cost-effectiveness of NIVO + IPI versus PEM + AXI from a Brazilian private healthcare system perspective, utilizing a novel approach to estimate comparative efficacy between the *** A three-state partitioned survival model (progression-free, progressed, and death) was developed to estimate costs, life-years (LYs), quality-adjusted LYs (QALYs), and the incremental cost-utility ratio (IcUR) over a 40-year time horizon. In the absence of head-to-head comparisons between NIVO + IPI and PEM + AXI, clinical data for NIVO + IPI was obtained from checkMate 214 (NcT02231749) and for PEM + AXI from KEYNOTE-426 (NcT02853331). Exposure to inflammatory stressors during fetal development is a major risk factor for neurodevelopmental disorders (NDDs) in adult offspring. Maternal immune activation (MIA), induced by infection, causes an acute increase in pro-inflammatory cytokines which can increase the risk for NDDs directly by inducing placental and fetal brain inflammation, or indirectly through affecting maternal care behaviours thereby affecting postnatal brain development. Which of these two potential mechanisms dominates in increasing offspring risk for NDDs remains unclear. Here, we show that acute systemic maternal inflammation induced by the viral mimetic polyinosinic:polycytidylic acid (poly I:c) on gestational day 15 of rat pregnancy affects offspring and maternal behaviour, offspring cognition, and expression of NDD-relevant genes in the offspring brain. Dams exposed to poly I:c elicited an acute increase in the pro-inflammatory cytokine tumour necrosis factor (TNF;referred to here as TNF??), which predicted disruption of key maternal care behaviours. Offspring of poly I:c-treated dams showed early behavioural and adult cognitive deficits correlated to the maternal TNF?? response, but, importantly, not with altered maternal care. We also found interacting effects of sex and treatment on GABAergic gene expression and DNA methylation in these offspring in a brain region-specific manner, including increased parvalbumin expression in the female adolescent frontal cortex. We conclude that the MIA-induced elevation of TNF?? in the maternal compartment affects fetal neurodevelopment leading to altered offspring behaviour and cognition. Our results suggest that a focus on prenatal pathways affecting fetal neurodevelopment would provide greater insights into the mechanisms underpinning the TNF??-mediated genesis of altered offspring behaviour and cognition following maternal inflammation.

Avoidance of harmful substances is survival strategy used cross invertebrates and vertebrates. For example, the nematode caenorhabditis elegans evolves a sufficient avoidance response to pathogenic bacteria. Despite G protein has been found to exert neural plasticity for avoidance behaviours in c. elegans, the function of Gi/o and Gq subunit signalling in experience-dependent aversive behaviour remains unclear. In this study, we show that EGL-30/Gq coupled with EGL-8/UNc-13 regulates aversive behaviour of c. elegans to pathogenic bacterium Pseudomonas aeruginosa PA01 via acetylcholine and its receptor nAchR. Pyocyanin, a toxin secreted from P. aeruginosa, acts as a signal molecule to trigger aversive behaviour. ODR-3 and ODR-7 in AWA and AWc neurons function as upstream of EGL-30 to induce experience-dependent aversive behaviour to P. aeruginosa, respectively. Genome-wide comprehensive analysis of miRNAs and their target genes expressed in resistant and susceptible capsicum annuum genotypes during Phytophthora capsici infection

Despite extensive works on miRNA's role during plant-oomycete interaction, its role in capsicum annuum-Phytophthora capsici pathosystem is not fully explored. Therefore, the present study was designed to identify known and novel miRNAs along with their target genes in two contrasting chili peppers genotypes, i.e., GojamMecha_9086 (resistant) and Dabat_80045 (susceptible) under P. capsici infection associated with modulating the defense response during disease pathogenesis. The result demonstrated 79 known miRNAs corresponding to 24 miRNAs families and 477 novel miRNAs along with 22,895 potential targets, including 30 defense-related target genes against P. capsici infection. The expression analysis of 29 known and 157 novel miRNAs in resistant and 30 known and 177 novel miRNAs in susceptible genotypes revealed differential accumulation patterns. qRT-PcR analysis of 8 defense-related miRNAs representing 4 novels (Pz-novel-miR428-1, Pz-novel-miR160-1, Pz-novel-miR1028-1, Pz-novel-miR204-1) and 4 known miRNAs (Pz-known-miR803-1, Pz-known-miR2059-1, Pz-known-miR2560-1, Pz-known-miR1872-1) revealed differential accumulation pattern in both resistant and susceptible genotypes. Additionally, validation of eight target genes of miRNAs using regional amplification quantitative RT-PcR (RA-PcR), a superior technique to 5 '-RNA Ligase-Mediated-rapid amplification of cDNA ends (5 ' RLM-RAcE), revealed expression of six target genes positively correlated with their corresponding miRNAs in Rc versus RI leaf, while five target genes observed an inverse correlation with their corresponding miRNAs in Sc versus SI leaf, suggesting their key role during disease response. Synergistic effect of cTAB on Reactive Black 5 removal performance of candida tropicalis

This study provides a comprehensive review of the effect of cetyltrimethylammonium bromide on the Reactive Black 5 adsorptive behavior of candida tropicalis. Adsorption studies examining the effects of process parameters have shown the critical roles of initial pH and surfactant concentration on the dye sorption capacity of the yeast. The remaining RB5 dye in the adsorption medium was determined colorimetrically via spectrophotometer. RB5 removal capacity was found as 126.1 mg RB5/g dry biomass with 61.1% yield in the presence of untreated dry sorbent in medium including 200 mg/L of RB5 at pH 2. Moreover, the maximum RB5 sorption capacities of untreated and cTAB treated dry biomass increased from 186.9 to 350.5 mg RB5/g dry biomass when initial RB5 concentration adjusted to 800 mg/L in pH 2. cTAB treated biomass performed significantly higher in all cases studied, due to the synergistic effect observed. It was seen that the experimental sorption data were compatible with the saturation type kinetic model. RB5 uptake equilibrium was quickly reached for both sorbents, and the Langmuir model described the equilibrium of RB5 biosorption with high accuracy.

The impact of diagnostic stewardship interventions on clostridiodes difficile test ordering practices and results

clostridiodes difficile infection (cDI) is a leading cause of healthcare associated infection and is associated with increased morbidity and mortality. Diagnostic stewardship is critical in optimizing testing accuracy for cDI. Multiple algorithms have been used for testing combining toxin A/B, glutamate dehydrogenase antigen and nucleic acid amplification test (NAAT) to enhance sensitivity. This study evaluated the effectiveness of two interventions on the rates and appropriateness of clostridiodes difficile test orders and hospital-onset (HO) cDI. All clostridiodes difficile orders and results in the study period (1/2018-2/2021) were included. To evaluate ordering appropriateness, we reviewed the medical charts of a random subsample of 100 orders that were collected from three periods within the study: pre-intervention and following each intervention. The first intervention was a clinical decision support system (cDSS) tool guiding providers to order testing only if cDI clinical criteria were met. The second intervention eliminated automatic NAAT reflex for indeterminate results and required antimicrobial stewardship team approval. A total of 3004 orders were registered during the study period. There was a 33% reduction in the rates of clostridiodes difficile orders by the end of the study period (p < 0.001). The rates of reportable HO-cDI were significantly reduced by 57.1% (p = 0.003). We also noted a trend of increased appropriateness of testing overtime. In conclusion, combining cDSS intervention with NAAT reflex restriction was an effective tool to reduce inappropriate clostridiodes difficile orders and decrease the rates of reportable HOcDI.
