The DICOM standard has been widely adopted for the exchange and management of biomedical images. Its hierarchical structure allows representing data and metadata of medical imaging studies. However, other patient data not directly related with the study, such as prescriptions and treatments, are stored in independent Electronic Health Record (EHR) systems. With the increasing production of medical imaging studies, repositories responsible for storing DICOM images started to contain massive amounts of data. Therefore, retrieving a subset of images based on similar criteria as the ones used in EHR systems, is a complex task for a medical researcher. In this paper, we propose an architecture to define cohorts over medical imaging data sets. This proposal uses a DICOM archive to index and to retrieve images, while the studies' selection is performed through a web application, ATLAS, which is normally used on observational studies upon EHR data. The presented architecture was validated using a public data set with synthetic EHR data.

With the increased push to promote data-driven methods in modern healthcare, there is a tremendous need for fast access to clinical datasets in order to pursue medical breakthroughs in the areas of personalized medicine and big data knowledge discovery. However, the inherent lack of trust between the data custodians and data consumers/users has resulted in a fully manual honest broker approach to access and process protected healthcare data. Such a manual approach leads to slow data handling, and adds to overheads needed to address data auditability and assurance needed for compliance with healthcare data security standards. In this paper, we address these challenges by proposing a trust model to enable semi-automation of the honest broker process to increase its efficiency. The trust model is based on multi-dimensional risk management principles and considers risk associated with data identifiers, as well as requestor profile and reputation. We implement and evaluate a semi-automated honest broker that uses our trust model in a community cloud testbed using the SynPUF synthetic dataset. Our experiment results show that our multidimensional risk management approach consistently identifies the lower confidentiality risk configuration in the semi-automation in comparison with a one-dimensional strategy. Thus, our semi-automated honest brokering approach improves efficiency for data custodians and data consumers by facilitation of fast and secure data access, while also ensuring compliance in the processing of the protected datasets.

We developed a standardized framework named RHEA to represent longitudinal status of patient with cancer. RHEA generates a dashboard to visualize patients' data in the Observational Medical Outcomes Partnership-common data model format. The generated dashboard consists of three main parts for providing the macroscopic characteristics of the patient: 1) cohort-level visualization, 2) individual-level visualization and 3) cohort generation.

Railway Traffic Management Systems (TMSs) handle data from multiple railway subsystems, including Rail Business Services (such as interlocking, RBC, maintenance service, etc.) and external services (such as passenger information systems, weather forecast, etc.). In turn, the data from these subsystems are described in several models or ontologies contributed by various organizations or projects which are in a process of converging or federation. The challenge of the Shift2Rail OPTIMA project, which is implementing a communication platform for virtual testing of new applications for railway TMS, is to allow the exchange of data between different services or users and to support new traffic management applications, enabling access to a large number of disparate data sources. In this paper, the core activities of the OPTIMA project related to the formulation and standardization of a common data model are described. A new common data model is developed based on standardized data structures to enable the seamless exchange of large amounts of data between different and heterogeneous sources and consumers of data, that contributes to the building of next generation of a more effective and efficient railway TMS suitable to offer precise and real-time traffic information to railway operators and other end users.

Collecting data in any domain is often only the first step in creating intelligence. This is the beginning of a process which includes fusing that data with other data sources to create something more than the individual data elements. This has been historically difficult because collection sensors are optimized for their respective conventional domains. We have developed a multi-sensor system combining an electro-optical camera and infi-ared camera, using a revolutionary data handling and manipulating process to fuse multiple types of data to create inteffigence. To address the data fusion needs of such a system, we have developed an internationally recognized, open data fusion language called Transducer Markup Language (TML). TML was used in conjunction with the multi-sensor system to describe data from the EO and IR cameras and support sensors all within a self-contained mobile delivery system during mifitary exercises in July 2007. The TML data was collected and disseminated to multiple users within a Service Oriented Architecture (SOA), after which it was Ruther distributed using Cursor-On-Target (COT) messages over a second network. This demonstrated both data fusion of multiple sensor data sources and integration into existing data distribution infrastructures. Key innovations of this demonstration included the use of multiple sensors including EO & IR cameras and a single sensor data exchange language to capture and describe sensor output.

Enhancing Clinical data and Clinical Research data with Biomedical Ontologies - Insights from the Knowledge Representation Perspective

OBJECTIVES: There exists a communication gap between the biomedical informatics community on one side and the computer science/artificial intelligence community on the other side regarding the meaning of the terms "semantic integration" and "knowledge representation". This gap leads to approaches that attempt to provide one-to-one mappings between data elements and biomedical ontologies. Our aim is to clarify the representational differences between traditional data management and semantic-web-based data management by providing use cases of clinical data and clinical research data re-representation. We discuss how and why one-to-one mappings limit the advantages of using Semantic Web Technologies (SWTs). METHODS: We employ commonly used SWTs, such as Resource Description Framework (RDF) and Ontology Web Language (OWL). We reuse pre-existing ontologies and ensure shared ontological commitment by selecting ontologies from a framework that fosters community-driven collaborative ontology development for biomedicine following the same set of principles. RESULTS: We demonstrate the results of providing SWT-compliant re-representation of data elements from two independent projects managing clinical data and clinical research data. Our results show how one-to-one mappings would hinder the exploitation of the advantages provided by using SWT. CONCLUSIONS: We conclude that SWT-compliant re-representation is an indispensable step, if using the full potential of SWT is the goal. Rather than providing one-to-one mappings, developers should provide documentation that links data elements to graph structures to specify the re-representation. Georg Thieme Verlag KG Stuttgart.

Introduction: The expansion of electronic health record (EHR) data networks over the last two decades has significantly improved the accessibility and processes around data sharing. However, there lies a gap in meeting the needs of Clinical and Translational Science Award (CTSA) hubs, particularly related to real-world data (RWD) and real-world evidence (RWE).Methods: We adopted a mixed-methods approach to construct a comprehensive needs assessment that included: (1) A Landscape Context analysis to understand the competitive environment;and (2) Customer Discovery to identify stakeholders and the value proposition related to EHR data networks. Methods included surveys, interviews, and a focus ***: Thirty-two CTSA institutions contributed data for analysis. Fifty-four interviews and one focus group were conducted. The synthesis of our findings pivots around five emergent themes: (1) CTSA segmentation needs vary according to resources;(2) Team science is key for success;(3) Quality of data generates trust in the network;(4) Capacity building is defined differently by researcher career stage and CTSA existing resources;and (5) Researchers' unmet ***: Based on the results, EHR data networks like ENACT that would like to meet the expectations of academic research centers within the CTSA consortium need to consider filling the gaps identified by our study: foster team science, improve workforce capacity, achieve data governance trust and efficiency of operation, and aid Learning Health Systems with validating, applying, and scaling the evidence to support quality improvement and high-value care. These findings align with the NIH NCATS Strategic Plan for data Science.

Background: Amines are biogenic amino acid derivatives, which play pleiotropic and very important yet complex roles in animal physiology. For many other relevant biomolecules, biochemical and molecular data are being accumulated, which need to be integrated in order to be effective in the advance of biological knowledge in the field. For this purpose, a multidisciplinary group has started an ontology-based system named the Amine System Project (ASP) for which amine-related information is the validation bench. Results: In this paper, we describe the Ontology-Based Mediator developed in the Amine System Project (http://***) using the infrastructure of Semantic Directories, and how this system has been used to solve a case related to amine metabolism-related protein structures. Conclusions: This infrastructure is used to publish and manage not only ontologies and their relationships, but also metadata relating to the resources committed with the ontologies. When using personal information, researchers face difficulty complying with Korea's Personal Information Protection Act. Therefore, a clinical common data model (CDM)-based research methodology for multi institutional sharing of code and statistical analysis results has been used. However, the current multi institutional CDM study environment lacks the considerations of personal information protection or institutional arrangements. Therefore, we propose a two-factor secure framework for the clinical distributed multicenter study environment. In this framework, two-factor (security status and security awareness) are considered. The security status is based on applying objective security factors and technologies to the clinically distributed multicenter study infrastructure and related systems. The security awareness is based on objective security factors for a user-oriented security application to a clinically distributed multicenter study environment. This two-factor assessment-based approach identifies objective factors for complex clinically distributed multicenter study environments, research procedures, and users, as well as applies security factors in detail. The proposed framework investigates the thoughts of users who use the CDM and are known to be safe so far. It compares and analyzes the security status of the CDM and reflects it in the framework design to enhance security and support a smooth clinical data use environment.

The Molecular Biology Toolkit (MBT): a modular platform for developing molecular visualization applications

Background: The large amount of data that are currently produced in the biological sciences can no longer be explored and visualized efficiently with traditional, specialized software. Instead, new capabilities are needed that offer flexibility, rapid application development and deployment as standalone applications or available through the Web. Results: We describe a new software toolkit - the Molecular Biology Toolkit (MBT;http://***) - that enables fast development of applications for protein analysis and visualization. The toolkit is written in Java, thus offering platform-independence and Internet delivery capabilities. Several applications of the toolkit are introduced to illustrate the functionality that can be achieved. Conclusions: The MBT provides a well-organized assortment of core classes that provide a uniform data model for the description of biological structures and automate most common tasks associated with the development of applications in the molecular sciences (data loading, derivation of typical structural information, visualization of sequence and standard structural entities).
