Virtual planning in orthognathic surgery

Numerous publications regarding virtual surgical planning protocols have been published, most reporting only one or two case reports to emphasize the hands-on planning. None have systematically reviewed the data published from clinical trials. This systematic review analyzes the precision and accuracy of three-dimensional (3D) virtual surgical planning of orthognathic procedures compared with the actual surgical outcome following orthognathic surgery reported in clinical trials. A systematic search of the current literature was conducted to identify clinical trials with a sample size of more than five patients, comparing the virtual surgical plan with the actual surgical outcome. Search terms revealed a total of 428 titles, out of which only seven articles were included, with a combined sample size of 149 patients. Data were presented in three different ways: intra-class correlation coefficient, 3D surface area with a difference <2 mm, and linear and angular differences in three dimensions. Success criteria were set at 2 mm mean difference in six articles;125 of the 133 patients included in these articles were regarded as having had a successful outcome. Due to differences in the presentation of data, meta-analysis was not possible. Virtual planning appears to be an accurate and reproducible method for orthognathic treatment planning. A more uniform presentation of the data is necessary to allow the performance of a meta-analysis. Currently, the software system most often used for 3D virtual planning in clinical trials is SimPlant (Materialise). More independent clinical trials are needed to further validate the precision of virtual planning.

Suppressive effects on allergic contact dermatitis by short-term fasting

Fasting alters various hormonal and immune conditions. It has been reported that delayed type immune response to the injection of keyhole limpet hemocyanin was depressed by short-term fasting. In this study, we adopted the computer-assisted image analyzer for histopathological analysis and evaluated the influence of short-term fasting on allergic contact dermatitis induced by 24-dinitrofluorobenzene (DNFB). Mice were sensitized by painting of DNFB to the abdomen. After the sensitization, mice were challenged by DNFB painting to the car. Fasting started 24 hour before (48-hour fasted group) or immediately after (24-hour fasted group) the challenging. Fasting without DNFB treatment did not induce remarkable change of ear thickness, ear tissue, serum albumin, serum total protein, serum glutamic oxaloacetic transaminase, serum glutamic pyruvic transaminase, or serum creatine phosphokinase. In contrast, fasting suppressed the increment of ear thickness in the DNFB-treated group in this Study. We Could also demonstrate, using the computerized image analyzer, that both lymphocyte infiltration and the edema in the dermis were suppressed in fasted mice treated with DNFB. Further, edema in the dermis was inhibited more strongly in 48-hour fasted mice than in 24-hour fasted mice. These findings indicate that short-term fasting induce histopathological changes in the state of contact dermatitis.

computerized Photogrammetry Used to Calculate the Brow Position Index

Background The orbital region is of vital importance to facial expression. Brow ptosis, besides having an impact on facial harmony, is a sign of aging. Various surgical techniques have been developed to increase the efficacy of brow-lift surgery. However, no consensus method exists for an objective measurement of the eyebrow position due to the curvature of the face. Therefore, this study aimed to establish a method for measuring the eyebrow position using computerized photogrammetry. Methods For this study, 20 orbital regions of 10 volunteers were measured by direct anthropometry using a digital caliper and by indirect anthropometry (computerized photogrammetry) using standardized digital photographs. Lines, points, and distances were defined based on the position of the anthropometric landmarks endocanthion and exocanthion and then used to calculate the brow position index (BPI). Statistical analysis was performed using Student's t test with a significance level of 5 %. Results The BPI values obtained by computerized photogrammetric measurements did not differ significantly from those obtained by direct anthropometric measurements (p > 0.05). The mean BPI was 84.89 +/- A 10.30 for the computerized photogrammetric measurements and 85.27 +/- A 10.67 for the direct anthropometric measurements. Conclusion The BPI defined in this study and obtained by computerized photogrammetry is a reproducible and efficient method for measuring the eyebrow position.

Background Radiomics or computer-extracted texture features derived from MRI have been shown to help quantitatively characterize prostate cancer (PCa). Radiomics have not been explored depth in the context of predicting biochemical recurrence (BCR) of PCa. Purpose To identify a set of radiomic features derived from pretreatment biparametric MRI (bpMRI) that may be predictive of PCa BCR. Study Type Retrospective. Subjects In all, 120 PCa patients from two institutions, I-1 and I-2, partitioned into training set D-1 (N=70) from I-1 and independent validation set D-2 (N=50) from I-2. All patients were followed for 3 years. Sequence 3T, T-2-weighted (T2WI) and apparent diffusion coefficient (ADC) maps derived from diffusion-weighted sequences. Assessment PCa regions of interest (ROIs) on T2WI were annotated by two experienced radiologists. Radiomic features from bpMRI (T2WI and ADC maps) were extracted from the ROIs. A machine-learning classifier (C-BCR) was trained with the best discriminating set of radiomic features to predict BCR (p(BCR)). Statistical Tests Wilcoxon rank-sum tests with P<0.05 were considered statistically significant. Differences in BCR-free survival at 3 years using p(BCR) was assessed using the Kaplan-Meier method and compared with Gleason Score (GS), PSA, and PIRADS-v2. Results Distribution statistics of co-occurrence of local anisotropic gradient orientation (CoLlAGe) and Haralick features from T2WI and ADC were associated with BCR (P<0.05) on D-1. C-BCR predictions resulted in a mean AUC=0.84 on D-1 and AUC=0.73 on D-2. A significant difference in BCR-free survival between the predicted classes (BCR+and BCR-) was observed (P=0.02) on D-2 compared to those obtained from GS (P=0.8), PSA (P=0.93) and PIRADS-v2 (P=0.23). Data ConclusionRadiomic features from pretreatment bpMRI can be predictive of PCa BCR after therapy and may help identify men who would benefit from adjuvant therapy.

Usefulness of automated assessment of nuclear cardiology for prediction of major cardiac events in Japanese patients with known or suspected coronary artery disease: Comparison with conventional visual assessment in a large-scale prognostic study

Background: There are no reports indicating that automated quantification with a total perfusion deficit (TPD) is used to predict future cardiac events in Japanese patients. We, therefore, aimed to determine the prognostic value of the automated assessment with the TPD for risk stratification of major cardiac events (MCEs) in Japanese patients with known or suspected coronary artery disease (CAD). Methods: We retrospectively investigated 2848 patients who underwent rest Tl-201 and stress Tc-99m-tetrofosmin myocardial perfusion single photon emission computed tomography (SPED') between October 2004 and March 2008. The follow-up period was 25.8 +/- 11.0 months. The TPD was automatically derived from the SPED' image through the QPS software with the Japanese normal database. Twenty segments of SPED' images were analyzed with the 5-point visual scoring model to estimate summed scores. The endpoint of the follow-up was the occurrence of MCEs within 1 year after the SPECT, which were identified with medical records or responses to a posted questionnaire. Results: During the first year of the follow-up, 62 patients had MCEs, which comprised cardiac death (n=30), non-fatal myocardial infarction (n = 13), and unstable angina pectoris (n = 19). The MCE rates positively correlated with the stress TPD and the summed stress score. Sensitivity of the automated quantification with the TPD for detection of the MCEs was high and similar to that of the visual semi-quantification. Multivariate Cox regression analysis indicated that significant independent predictors for the MCEs were an estimated glomerular filtration rate and the ischemic variables both in the automated quantification and visual semi-quantification. Conclusion: The automated quantification with the TPD is useful for prognostic risk stratification of MCEs in Japanese patients with known or suspected CAD. Its predictive power is similar to that of the visual semi-quantification by expert interpreters. (C) 2014 Japanese

Spiral CT angiography and 3D reconstruction in patients with aortic coarctation

The objective of this study was to assess the reliability of spiral CT angiography (CTA) and 3D reconstruction in patients with aortic coarctation (CoA). Eighteen patients with suspected or surgically proven coarctation were examined by spiral CT. In addition to the axial slices, 3D reconstructions, such as shaded surface display (SSD) and maximum intensity projection (MIP), were used to determine the diameters of the CoA and the pre-and poststenotic aorta and to visualise the collateral vessels. Diameters derived from cardiac catheterization were compared With those from CTA in 8 patients. The degree of aortic stenosis was correlated with blood pressure gradients (BPG) in 12 patients. The difference between the diameters of the CoA and the pre-and poststenotic aorta derived from MIP and angiography was not statistically significant (p = 0.69). With SSD the internal thoracic artery was detected in 16 and the posterior intercostal artery in 13 cases. The degree of aortic stenosis correlated poorly with the BPG (r = 0.51, r(2) = 0.26). CTA with 3D reconstruction rep.:resents a reliable noninvasive technique for the assessment of the degree of CoA and the visualisation of collateral vessels. It may serve as a follow-up investigation after intervention or surgical treatment.

Soft tissue-based registration of intraoral scan with cone beam computed tomography scan

The aim of this study was to evaluate a novel soft tissue-based method to register an intraoral scan (IOS) with a cone beam computed tomography (CBCT) scan. IOS and CBCT data were obtained from eight dentate patients (mean age 21 +/- 2 years;three male, five female) and 14 fully edentulous patients (mean age 56 +/- 9 years;eight male, six female). An algorithm was developed to create a soft tissue model of the CBCT scan, which allowed a soft tissue-based registration to be performed with the IOS. First, validation was performed on dentate jaws with registration of the palatal mucosal surface and accuracy evaluation at the level of the teeth. Second, fully edentulous jaws were registered using both the palatal and alveolar crest mucosal surfaces. Distance maps were created to measure the method accuracy. The mean registration error was 0.49 +/- 0.26 mm for the dentate jaws. Registration of the fully edentulous jaws had a mean error of 0.16 +/- 0.08 mm at the palate and 0.16 +/- 0.05 mm at the alveolar crest. In conclusion, the high accuracy of this registration method may allow the digital workflow to be optimized when no teeth are available to perform a regular registration procedure.

Quantitative assessment of regional cerebral blood flows by perfusion CT studies at low injection rates: a critical review of the underlying theoretical models

Viability of the cerebral parenchyma is dependent on cerebral blood flow (CBF), which is usually kept in a very narrow range due to efficient autoregulation processes and can be altered in a variety of pathological conditions. An accurate method allowing for a quantitative assessment of regional cerebral blood flows (rCBF) and available for the routine clinical practice would, for sure, greatly contribute to improving the management of patients with cerebrovascular diseases. Different imaging techniques are now available to evaluate rCBF: positron emission tomography;single photon emission CT;stable-xenon CT;perfusion CT: and perfusion MRI. Each of these imaging techniques uses an indicator, with specific biological properties, and is supported by a model, which consists of a few simplifying assumptions, necessary to state and solve the equations giving access to rCBF The obtained results are more or less reliable, depending on whether modeling hypotheses are fulfilled by the used indicator. The purpose of this article is to review the various supporting models in the assessment of rCBF with special emphasis on perfusion CT studies at low injection rates and on iodinated contrast material used as an indicator.

MULTILAYER PEROXIDASE-LABELED ANTIBODY METHOD - COMPARISON WITH LABELED STREPTAVIDIN-BIOTIN METHOD, AVIDIN-BIOTIN-PEROXIDASE COMPLEX METHOD, AND PEROXIDASE-ANTIPEROXIDASE METHOD

A new, extremely sensitive, ''multilayer peroxidase-labeled antibody method'' is introduced. This method utilizes peroxidase-labeled antibodies that immune-react with their antigen, but also can be an antigen to other peroxidase-labeled antibodies. An example of this method is: use rabbit IgG as the first antibody, peroxidase-labeled goat antirabbit IgG as the second antibody, peroxidase-labeled rabbit antigoat IgG as the third antibody, peroxidase-labeled goat antirabbit IgG as the fourth antibody, and peroxidase-labeled rabbit antigoat IgG as the fifth antibody in succession. We compared the sensitivities of this multilayer peroxidase-labeled antibody method with that of commercially available immuno-peroxidase staining kits, i.e., avidin-biotin-complex method and peroxidase-antiperoxidase method, both by immunostaining and enzyme-linked immunosorbent assay, The generated signal and background in immunostained sections were determined by a computer-assisted image analyzer and by enzyme-linked immunosorbent assay spectrophotometrically. It was found that a maximum signal/background ratio was obtained with those sections reacted with the first four antibodies in succession by the multilayer peroxidase-labeled antibody method, and those reacted with the five antibodies in succession by the multilayer peroxidase-labeled antibody method in the enzyme-linked immunosorbent assay. Thus, the multilayer peroxidase-labeled antibody method is more sensitive than the avidin-biotin-complex method and the peroxidase-antiperoxidase method. (C) 1995 Wiley-Liss, Inc.

Radiologist-Trained AI Model for Identifying Suboptimal Chest- Radiographs

Rationale and Objectives: Suboptimal chest radiographs (CXR) can limit interpretation of critical findings. Radiologist-trained AI models were evaluated for differentiating suboptimal(sCXR) and optimal(oCXR) chest radiographs. Materials and Methods: Our IRB-approved study included 3278 CXRs from adult patients (mean age 55 20 years) identified from a ret-rospective search of CXR in radiology reports from 5 sites. A chest radiologist reviewed all CXRs for the cause of suboptimality. The de-identified CXRs were uploaded into an AI server application for training and testing 5 AI models. The training set consisted of 2202 CXRs (n = 807 oCXR;n = 1395 sCXR) while 1076 CXRs (n = 729 sCXR;n = 347 oCXR) were used for testing. Data were analyzed with the Area under the curve (AUC) for the model's ability to classify oCXR and sCXR correctly. Results: For the two-class classification into sCXR or oCXR from all sites, for CXR with missing anatomy, AI had sensitivity, specificity, accuracy, and AUC of 78%, 95%, 91%, 0.87(95% CI 0.82-0.92), respectively. AI identified obscured thoracic anatomy with 91% sensitiv-ity, 97% specificity, 95% accuracy, and 0.94 AUC (95% CI 0.90-0.97). Inadequate exposure with 90% sensitivity, 93% specificity, 92% accuracy, and AUC of 0.91 (95% CI 0.88-0.95). The presence of low lung volume was identified with 96% sensitivity, 92% specificity, 93% accuracy, and 0.94 AUC (95% CI 0.92-0.96). The sensitivity, specificity, accuracy, and AUC of AI in identifying patient rotation were 92%, 96%, 95%, and 0.94 (95% CI 0.91-0.98), respectively. Conclusion: The radiologist-trained AI models can accurately classify optimal and suboptimal CXRs. Such AI models at the front end of radiographic equipment can enable radiographers to repeat sCXRs when necessary.
