External validation of a prostate biopsy simulator

Introduction. - Simulation-based training has proven to be a promising option allowing for initial and continuous training while limiting the impact of the learning curve on the patient. The Biopsym simulator was developed as a complete teaching environment for the prostate biopsy procedure. This paper presents the results of an external validation of this simulator, involving urology residents recruited during a regional teaching seminar. Methods. - Residents from 4 academic urology departments of the French Auvergne Rhone-Alpes region, who did not take part in the previous simulator validation studies, were enrolled. After a short presentation and standardized initiation session, residents carried out a simulated systematic 12-core biopsy procedure and were asked to fill in a questionnaire collecting their expectations and evaluation of the Biopsym simulator. The number of biopsies reaching each targeted sector, the total score provided by the simulator and the duration of the procedure were recorded. Results. - Twenty-three residents were recruited. The overall added value (/100) for learning was rated at a median of 100 (interquartile range 83-100), overall realism of the biopsy procedure at 80 (65-89). The median percentage of biopsies reaching the targeted sector was 66.7% (62-75). The median score provided by the simulator was 50% (37-60). For both, the difference between residents with or without prior biopsy experience was not statistically significant. The median duration of the simulated biopsy procedure was 4:58 (minutes: seconds) (3:49-6:00). Resident with prior experience required less time to complete the biopsy procedure 3:53 (3:39-4:56) vs. 5:10 (4:59-7:10), P= 0.01. Conclusion. - This external validation study confirms a high acceptance of the simulator by the target audience. Anatomy is an essential subject of the medical curriculum. Despite its relevance, the curricular time and logistical resources devoted to teaching anatomy are in decline, favoring the introduction of new pedagogical approaches based on computer-assisted learning (CAL). This new pedagogical approach provides an insight into students' learning profiles and features, which are correlated with knowledge acquisition. The aim of this study was to understand how training with CAL platforms can influence medical students' anatomy performance. A total of 611 medical students attending Musculoskeletal Anatomy (MA) and Cardiovascular Anatomy (CA) courses were allocated to one of three groups (MA Group, CA Group, and MA + CA Group). An association between the performance in these anatomy courses and the number of CAL training sessions was detected. In the MA Group (r = 0.761,P < 0.001) and the MA + CA Group (r = 0.786,P < 0.001), a large positive correlation was observed between musculoskeletal anatomy performance and the number of CAL training sessions. Similarly, in the CA Group (r = 0.670,P < 0.001) and the MA + CA Group (r = 0.772,P < 0.001), a large positive correlation was observed between cardiovascular anatomy performance and the number of CAL training sessions. Multiple linear regression models were performed, considering either musculoskeletal or cardiovascular anatomy performance as the dependent variable. The results suggest that using CAL platforms to study has a positive dose-dependent effect on anatomy performance. Understanding students' individual features and academic background may contribute to the optimization of the learning process.

Simulation-based training for prostate biopsies: towards the validation of the Biopsym simulator

Objectives: The Biopsym simulator, a virtual-reality simulator for prostate biopsies, was designed to offer enhanced teaching of the biopsy procedure. The objectives of the present article are to describe the new version of the simulator and report the results of a new validation study. Material and methods: A prospective validation study was conducted between January and March 2017. The new version of the simulator, with improved physical realism, ultrasound image deformation, and a new scoring system, was evaluated by novice and confirmed users. Results: Twenty-one users evaluated the simulator, including ten novices and 11 confirmed users. The overall realism of the biopsy procedure was rated at 7.7/10 (IQR 5.7-9). The differences between the rates given by confirmed users and novices were not statistically significant. The median overall score obtained for the performance of 12 systematic ultrasound-guided biopsies was 43% (IQR 33-55). The median score obtained by confirmed users was 54% (IQR 46-62), and the median score obtained by novices was 31% (IQR 20-35). The difference between the scores was statistically significant (p = 0.005). Conclusions: This study allowed us to gather evidence towards the validation, and particularly towards the construct validation of the new version of the Biopsym simulator.

Validating the Transfer of Skills Acquired on a Prostate Biopsy Simulator: A Prospective, Randomized, Controlled Study

OBJECTIVES: To evaluate the ability of students to reproduce the skills acquired on a prostate biopsy simulator in a real-life situation. DESIGN: A prospective randomized controlled study was conducted. Medical students with no experience of prostate biopsy were randomized between arm A "conventional training" and arm B "simulator-enhanced training. "The training was performed for both groups on the simulator. The students in arm B were provided with visual and numerical feedback. The transfer of skills was assessed by recording the position of the 12 biopsies performed by each student on an unembalmed human cadaver using a 3D ultrasound mapping device. SETTING: The study was conducted in an academic urology department and the cadaver experiments in the adjoining anatomy laboratory. RESULTS: Twenty-four students were included, and 22 completed the study. The median score obtained on the simulator at the end of the training was 57% (53-61) for arm A and 66% (59-71) for arm B. The median score obtained on the cadaver by students trained with the simulator was 75% (60-80), statistically superior to the score obtained by students trained conventionally of 45% (30-60), p < 0.0001. Themedian score obtained by all students when performing biopsies in a real-life situation was 63% (50-80) versus 60% (56-70) for their last training on the simulator. CONCLUSION: These results support the transfer of skills acquired on the simulator, and the superiority of a training curriculum integrating simulation, and performance feedback. Currently, medical education context poses different challenges to anatomy, contributing to the introduction of new pedagogical approaches, such as computer-assisted learning (CAL). This approach provides insight into students' learning profiles and skills that enhance anatomy knowledge acquisition. To understand the influence of anatomy CAL on spatial abilities, a study was conducted. A total of 671 medical students attending Musculoskeletal (MA) and Cardiovascular Anatomy (CA) courses, were allocated to one of three groups (MA Group, CA Group, MA+CA Group). Students' pre-training and post-training spatial abilities were assessed through Mental Rotations Test (MRT), with scores ranging between 0-24. After CAL training sessions, students' spatial abilities performance improved (9.72 +/- 4.79 vs. 17.05 +/- 4.57, P<0.001). Although male students in both MA Group and CA Group show better baseline spatial abilities, no sex differences were found after CAL training. The improvement in spatial abilities score between sessions (Delta MRT) was correlated with Musculoskeletal Anatomy training sessions in MA Group (r=0.333, P<0.001) and MA+CA Group (r=0.342, P<0.001), and with Cardiovascular Anatomy training sessions in CA Group (r=0.461, P=0.001) and MA+CA Group (r=0.324, P=0.001). Multiple linear regression models were used, considering the Delta MRT as dependent variable. An association of Delta MRT to the amount of CAL training and the baseline spatial abilities was observed. The results suggest that CAL training in anatomy has positive dose-dependent effect on spatial abilities.

To promote the efficiency of English pronunciation training, the computer-assisted Pronunciation training (CAPT) system is drawing more and more attention of language teaching, but the current CAPT systems for Chinese second language learners are still quite dissatisfactory. The paper briefly introduces the CAPT technology and its realizing process, and discusses the application of visual speech synthesis technology in CAPT to enhance its friendliness in detail. Comparable teaching experiment demonstrates that the CAPT system improved by visual speech synthesis can significantly improve the efficiency of English pronunciation training.

computer-assisted training of Finnish phonemic length was conducted with 7-year-old Russian-speaking second-language learners of Finnish. Phonemic length plays a different role in these two languages. The training included game activities with two-and three-syllable word and pseudo-word minimal pairs with prototypical vowel durations. The lowest accuracy scores were recorded for two-syllable words. Accuracy scores were higher for the minimal pairs with larger rather than smaller differences in duration. Accuracy scores were lower for long duration than for short duration. The ability to identify quantity degree was generalized to stimuli used in the identification test in two of the children. Ideas for improving the game are introduced.

Most football simulators available use computer simulation to mimic football motion in a virtual environment. These kinds of simulations, while fun and interactive, are primarily for entertainment rather than as a training tool. In this paper, we present a virtual training system for football kicking based on computer sensing and football impact dynamics using real-time wireless feedback. It is designed as a computer-assisted training system that will give a kicker the ability to practice off the field and receive the detailed feedback comparable yet more quantitative than they would experience at a training camp. In addition, it enables football coaches with quantitative data to design player-specific training program for kickers in order to achieve a goal with more precision. The system includes a force-sensing panel to determine the position and velocity as initial data, real-time wireless feedback to calibrate the football kicking dynamics model, and an advanced visualization environment designed to recreate the realistic atmosphere of all major football stadiums. The system has been designed and tested in a lab environment. The preliminary results show that the wireless signal is sensitive enough to detect football positions on the fly. The data collected in real-time can be used to calibrate the football dynamics model in predicting the final position of the football with respect to the field goal, which is visualized using 3D stereoscopic technology.

This work creates a 3D anatomical models of the brain as well as its virtual images. It is to be applied in virtual training of neuroanatomy. These models are based on studies of Magnetic Resonance (MRI) and computer Tomography (CT). These models contain information of the anatomical structures of the brain's soft tissues. This research was developed using techniques of virtual training. Virtual training has become a new teaching environment for learning complex disciplines, ideal for the Medical Sciences. Virtual-Reality training has a huge impact because of its versatility of interaction with the 3D models. The students follow their own learning pace using this technique. The following method was carried out based on image-studies obtained by MRI and CT. This method was developed on three steps (1) one process for image filtering, (2) one process for image segmentation with the objective of obtaining the brain's structures, and (3) one process for creation of the 3D models. (The models were designed using the 3D Slicer program). Subsequently, the scene which includes the anatomical models were created. These scenes are achieved by applying several effects such as transparency, clipping, fusion, and registration with the original slices of this study. As a result, the virtual images are created. The students have the possibility to visualize and manipulate them, in different ways using the virtual brain structures. These 3D models could be clipped by the user or be shown jointed with the original 2D slice of the MRI or CT. These images could be exported to VRML format, which can be read using one plug-in. This research is destined to achieving a virtual environment for neurosurgical training that could be applied with minimal invasive neurosurgery.

Outcomes of three different types of computerized training in sub-lexical items (word-initial consonant clusters) on reading speed for 39 German-speaking poor readers in Grades 2 and 3 were evaluated. A phonological-orthographic association group, a reading aloud group, and a combined group were compared in performance with an untrained control group. During short-term training, the intervention groups showed higher gains than the control group in reading speed of the trained sub-lexical items and of the words containing the trained segments. No differences were found between the intervention groups. In the development of pseudoword reading, the groups did not show differential improvements. The generalization effect to pseudoword reading was similar, whether the pseudowords contained the trained segment as a syllable or as a non-syllabic letter string. The gains induced by training were specific to the materials used in training and did not induce gains in general reading speed.
