Prognostic Value of Quantitative p63 Immunostaining in Adenoid Cystic Carcinoma of Salivary Gland Assessed by computerized image analysis

BACKGROUND: In a long-term retrospective immunohistochemical study of adenoid cystic carcinoma (ACC) of salivary gland, we investigated the relation of p63 immunodetection to prognosis. Although it is generally agreed that the solid pattern is the most aggressive pattern of growth, ACCs with predominantly cribriform or tubular patterns have an unpredictable clinical course, with a relatively favorable 5-year survival but a low 20-year survival. METHODS: Formalin-fixed paraffin sections from 35 cases of ACC showing a predominantly better differentiated histopathology, ie, cribriform or tubular patterns of growth, were immunostained for p63. Automated image analysis was used to quantify p63 positivity, using a modification of a previously developed algorithm, RESULTS: Patients alive for more than 10 years had a lower extent of p63 expression than those who died of disease. Kaplan-Meier analysis revealed that separation of patients with morbidity and mortality from those alive with no evidence of disease, could be achieved at a cutoff of 35% p63 positivity (P = .0031, log-rank test). Multivariate analysis using the Cox proportional hazard model revealed p63 and tumor stage to be independent predictors of survival (P = .012 and P = .0003, respectively). CONCLUSIONS: To our knowledge, the present study is the first to report prognostic significance of p63 in salivary gland ACC and the first report of a robust and well-studied immunohistochemical stain performable on routinely fixed and processed tissue with prognostic utility. MONITORING OF ERYTHROCYTE AGGREGATE MORPHOLOGY UNDER FLOW BY computerized image-analysis

The morphology of red blood cell (RBC) aggregates was studied by direct visualization of RBC aggregation at different flow conditions in a computerized image analyzer. The aggregate morphology is expressed by an Aggregate Shape Parameter (ASP), defined as the ratio of the aggregate projected area to its square perimeter. Aggregation was induced by either dextran-70 (m.w. 70,000) or dextran-500 (m.w. 500,000), and compared to that in plasma. It was found that the aggregate morphology is a characteristic of the aggregating agent-in dextran-500, the RBC form rouleau aggregates as in plasma, while in dextran-70, they form clusters. In each system, while maintaining the overall typical morphology, the ASP decreases (i.e., the aggregate becomes longer) as the aggregate size is increased. The distribution of the ASP as a function of the aggregate size remains unchanged when the aggregate size is changed by modulation of the dextran concentration or the shear stress. Stretching of a rouleau aggregate by application of shear stress is reflected by a corresponding change in the ASP. It is suggested that the ASP is a characteristic of intercellular interactions. A theoretical model is proposed for evaluation of the deviation of aggregate shape from that of rouleau structure.

VEGF and ki 67 expression in squamous cell carcinoma of the tongue: An immunohistochemical and computerized image analysis study

Over-expression of ki 67 and vascular endothelial growth factor (VEGF) is a frequent finding in squamous cell carcinoma (SCC) of the oral mucosa. The expression of VEGF and ki 67 proteins was studied in a cohort of 87 patients with primary, previously untreated SCC of the tongue, using computerized image analysis (CIA) in order to determine the potential prognostic significance of these factors. Immunohistochemical analysis was performed with monoclonal anti-ki 67 (MIB 1) and anti-VEGF antibodies. A digital image analysis assay was applied for the evaluation of the results. Using CIA, VEGF over-expression was observed in 24/87 (27.5%) of the examined cases and this finding correlated to the stage of the disease (p = 0.05). ki 67 was over-expressed in 49/87 (56.3%) of the cases and correlated to the size of the tumors (p = 0.05). Cox regression analysis showed that there was no prognostic significance associating VEGF protein expression to survival status of the examined patients (p = 0.77), whereas ki 67 over-expression was strongly correlated to poor prognosis (p = 0.017). The size of the primary tumors was also strongly correlated to survival status of the patients (p = 0.024), whereas stage of disease showed a borderline statistical significance (p = 0.091). A first-generation regression model is presented, describing a linear relationship between dead ectomycorrhizal biomass (mg dg weight) and mycorrhizal area in the xy-plane (mm(2)) (r(2) = 0.996). Monitoring of red blood cell aggregability in a flow-chamber by computerized image analysis

A system for direct observation and video recording of red blood cells (RBC) under flow, and computerized analysis of their aggregability was constructed. A narrow-gap flow-chamber was designed, in which a blood sample is subjected to controllable flow. Software was developed to provide the RBC aggregate size distribution at different shear stresses. From the size distribution curves the average or median size (or any other percentile) of the aggregate population as a function of shear stress, as well as the shear stress required for complete disaggregation, can be derived. The kinetics of spontaneous aggregation, in stasis, from singly dispersed cells, as well as the disaggregation kinetics at varying shear stress, can be derived from temporal monitoring of the cells images. This system provides quantitative measures of RBC aggregability from direct visual monitoring of the aggregation and disaggregation processes. © 1994 Elsevier Science Ltd.

Surface structure of microporous membranes by computerized SEM image analysis applied to Anopore filters

The surface morphology of two Anopore(R) filters (A01 and A02), consisting of thin porous alumina sheets anodically formed, is analyzed here. Scanning Electron Microscopy (SEM) along with computerized image analysis (CIA) have been applied. Both faces of the membranes are studied in order to quantify their presumable asymmetry. The surface porosity and the pore density, or number of pores per surface unit, are directly obtained for each filter side;while, the statistical distributions of the pore areas and perimeters along with equivalent pore diameters and pore shape factors are studied as well. Attending to the found substantial asymmetry, an active membrane side can be distinguished from the porous support. It is seen that support layers are quite similar for both membranes, while the active surfaces have very similar simple pores, being multiple ones which establish differences in mean pore sizes for A01 and A02 membranes. The results on pore size have been used to calculate the water permeabilities of the membranes studied that, when compared with the experimental ones, allow to model the membrane as consisting of an active layer, whose thickness is obtained, plus a porous background. EVALUATION OF A SIMPLE METHOD FOR MEASURING THE CELLULAR DNA CONTENT OF MOUSE OOCYTES AND EMBRYOS, HUMAN FIBROBLASTS AND PARTHENOGENETICALLY ACTIVATED HUMAN OOCYTES USING A computerized image-analysis SYSTEM (SEESCAN)

We report the use of a simple, reproducible, photocytometric method for measuring nuclear DNA content of DAPI-stained cells, using a computerised image analysis system: Seescan. As this technique is non-destructive and uses very short exposure to ultraviolet Light, it can be used for either fixed or vital material. After correcting for any background cytoplasmic staining, the intensity of nuclear stain was measured by the Seescan and compared with that of control cells of known ploidy. Fixed material was found to stain more intensely than live material initially, but demonstrated a rapid loss of nuclear intensity over the first 90 min following removal from DAPI, after which the level plateaued. In contrast, live cells showed no change in nuclear intensity with time. The system was validated by measuring the DNA content of carefully timed mouse blastomeres, human fetal lung fibroblasts and parthenogenetically activated human oocytes. The results obtained were appropriate for the developmental stage or phenotypic appearance of each of the cell types measured.

computerized image-analysis IN CONJUNCTION WITH FLUORESCENCE MICROSCOPY FOR THE STUDY OF BLOOD-BRAIN-BARRIER PERMEABILITY IN-VIVO

The present paper describes a new method using computerised image analysis techniques for quantification of tracer extravasation over the blood-brain barrier as studied by intravital fluorescence microscopy. Cats were equipped with an open cranial window and continuously infused with fluorescein isothiocyanate-labelled dextran (FITC-dextran, mol. wt. 70 000) to maintain a steady plasma concentration. Several cortical fields were recorded in each experiment and the images stored on video tape for off-line analysis. This procedure, which largely eliminates the superficial pial vasculature and allows extraction of the extravasation areas, consists of the following steps: (1) averaging of images, (2) software shading correction based on the original images for compensation of optical non-uniformity, (3) correction of displacement artefacts, (4) intensity adjustment, (5) generation of subtraction images by subtracting the first image of a series from the subsequent ones, (6) median filtering and thresholding, (7) a length recognition algorithm, and (8) elimination of small areas. Compared to the previously described method, step (2) has been newly developed and steps (4) and (8) added to enhance sensitivity for detecting tracer extravasation. The degree of extravasation in a cortical field at a given time point [E(f) value] was calculated as the mean intensity of the remaining pixels. The E(f) is a quantitative value computed by a fully automatised procedure which takes into account the number, as well as the size and intensity, of extravasation areas in a given cortical field. The E(f) values obtained at different times in a series of experiments were averaged to give the E(I) value. The E(I) value did not alter when hypercapnia was employed to induce pure vasodilatation. On the other hand it increased dramatically, indicating tracer extravasation, during topical application of high concentrations of adenosine (10(-5)-10(-3) M). Initial studies of the use of computerised image analysis to determine micronucleus frequencies in human lymphocytes that have completed one nuclear division are described. Two methods, based on (a) bromodeoxyuridine incorporation and (b) cytokinesis blocking with cytochalasin-B, were studied. The former method is directly amenable to automation. Cytokinesis-blocked cells could not be automatically recognized by image analysis but it was possible to obtain the correct macronucleus frequency from the integrated optical density histograms by using the mononucleate binucleate cell ratio obtained by visual analysis. The mean (.+-. S.E.) integrated optical density of X-ray-induced micronuclei was 11.2% (.+-. 1.1) of that measured for nuclei of G1 cells.

TOPODERMATOGRAPHIC image-analysis FOR MELANOMA SCREENING AND THE QUANTITATIVE ASSESSMENT OF TUMOR DIMENSION PARAMETERS OF THE SKIN

Background. The clinical need to identify and evaluate changes of cutaneous lesions in melanoma screening or follow-up of patients with cancer is of paramount importance. Because skin-lesion changes may be small and numerous, clinical assessment alone does not meet the requirements of quantitative assessment. Using the computer as a diagnostic tool for the image analysis of sequentially captured skin surface images has resulted in the technical problem of insufficient registration reproducibility. This paper describes the technical logistics, setup procedure, and clinical evaluation of the novel technique termed ''topodermatography,'' which performs the quantitative videographic image analysis of skin-lesion changes over time. Methods. Digitized measurements of skin-surface image parameters were performed using a high-speed processor with an onboard coprocessor, a high-resolution video camera, specifically designed image processing software, and a position framework for the adjustment of the patient's standing position. The topodermatographic image analysis was performed on 109 consecutive patients who were at risk for melanoma (N = 98), had lesions from Kaposi's sarcoma (N = 4), had metastatic skin deposits from melanoma (N = 3), and had breast cancer (N = 4). Results. Skin lesion changes over time could be identified reliably within a few millimeters of diametric enlargement. In this series, a 0.51% early melanoma detection rate was assessed in 19 of 98 patients followed for 12 months. By monitoring manifest neoplastic skin lesions, tumor growth kinetics were analyzed quantitatively to determine the total area of skin involvement, thus facilitating precise response assessment. Conclusions. Topodermatographic image analysis helps to optimize screening and follow-up procedures for patients with melanoma and populations at risk for melanoma.
