Using a standardized algorithm to decrease central venous catheter utilization

Central venous catheters (CVCs) have many benefits in patient care but are associated with increasing risks with catheter duration. A level II trauma-certified community hospital sought to decrease CVC duration utilizing a daily assessment algorithm in the critical care unit. After implementation, CVC days decreased from 490 CVC days per 1,000 patient days to 452 (odds ratio 0.86, P < .01) and catheter duration decreased from 7.71 days to 6.57 (P = .19). Exploring the territorial unevenness of one-person households and contextual factors of vulnerability: Evidence from the Italian context

New family structures have emerged in Italy in recent decades, with a trend towards smaller nuclear families due to demographic, social and economic changes. An aging population, marital disruptions, declining fertility, and later marriages have contributed to this trend. It is important to understand the changing needs of families, especially the vulnerable, from both an economic and social perspective. Vulnerability is often related to economic factors, but people living alone are often at risk. The goal of this study is to classify Italian municipalities based on the prevailing characteristics of their one-person households, identifying areas of greater or lesser fragility. This classification constitutes a tool to plan people-based policies. Starting from the 2020 Italian Permanent Population and Housing Census data, a decision algorithm was used to identify municipalities according to the different types of their one-person households and to study their geographical distribution throughout the country. Our results show there is an unexpected heterogeneity that goes far beyond the classical North-South divide, emphasizing the urgency of approaching the study of economic and social processes at the local level.

We study the existential theory of equicharacteristic henselian valued fields with a distinguished uniformizer. In particular, assuming a weak consequence of resolution of singularities, we obtain an axiomatization of - and therefore an algorithm to decide - the existential theory relative to the existential theory of the residue field. This is both more general and works under weaker resolution hypotheses than the algorithm of Denef and Schoutens, which we also discuss in detail. In fact, the consequence of resolution of singularities our results are conditional on is the weakest under which they hold true.

BackgroundRegardless of the available antifungals, intraabdominal candidiasis (IAC) mortality continues to be high and represents a challenge for *** bodyThis opinion paper discusses alternative antifungal options for treating IAC. This clinical entity should be addressed separately from candidemia due to the peculiarity of the required penetration of antifungals into the peritoneal cavity. Intraabdominal concentrations may be further restricted in critically ill patients where pathophysiological facts alter normal drug distribution. Echinocandins are recommended as first-line treatment in guidelines for invasive candidiasis. However, considering published data, our pharmacodynamic analysis suggests the required increase of doses, postulated by some authors, to attain adequate pharmacokinetic (PK) levels in peritoneal fluid. Given the limited evidence in the literature on PK/PD-based treatments of IAC, an algorithm is proposed to guide antifungal treatment. Liposomal amphotericin B is advocated as first-line therapy in patients with sepsis/septic shock presenting candidemia or endophthalmitis, or with prior exposure to echinocandins and/or fluconazole, or with infections by Candida glabrata. Other situations and alternatives, such as new compounds or combination therapy, are also *** is a critical need for more robust clinical trials, studies examining patient heterogeneity and surveillance of antifungal resistance to enhance patient care and optimise treatment outcomes. Such evidence will help refine the existing guidelines and contribute to a more personalised and effective approach to treating this serious medical condition. Meanwhile, it is suggested to broaden the consideration of other options, such as liposomal amphotericin B, as first-line treatment until the results of the fungogram are available and antifungal stewardship could be implemented to prevent the development of resistance.

In a decade marked by 4.0 technologies, using some of them for the optimization of effluent treatment processes is one of the objectives of the current researchers in this area. In this paper, we present two models as a fuzzy rule-based expert system, utilized in a computer desktop platform “Peroxide 1.0” and a mobile app “Peroxide App.” The objective is to select the best experimental parameters-electrodes type and potential-for the maximization of hydrogen peroxide (H₂O₂) electrogeneration. In situ H₂O₂ generation has as an auxiliary function to be used in the advanced oxidative processes for the degradation of persistent organic pollutants and emerging contaminants, which are not treated by traditional chemical and biological processes. For the construction of the proposed software in this study, the data were initially correlated using the MATLAB® platform Fuzzy toolbox, and the expert knowledge and literature were associated by a desktop and mobile knowledge system that weighs the variables according to their impact on the electrogeneration productivity. The desktop end-user interface was coded using Microsoft Visual Studio Rapid Application Development, and the mobile app was developed on the Android platform similar to Peroxide 1.0, adopting the specifics and particularity of the programming system. The developed systems were validated in different laboratory scenarios, and their performance were considered satisfactory, in agreement with the literature. Therefore, it is concluded that the Peroxide 1.0 and Peroxide App models can be a significant contribution to and advance the improvement in the processes for all types of industries that generate chemical effluents and require their treatment based on oxidative processes that use H₂O₂. © 2022, The Author(s), under exclusive licence to Springer Science+Business Media, LLC, part of Springer Nature.

Cognitive agents can be used to simulate autonomous robots, which have the ability of perceiving, reasoning, judging and learning information and then reacting to this information and to the environment. In this thesis a model of simple cognitive agents learning to cross an intersection of minor and major roads is introduced. In the model the major and minor roads are modeled by cellular automata. The cars on the major road move in accordance with the modified rules of the Nagel-Schreckenberg model and the cognitive agents on the minor road learn to cross the major road using an observational social learning process. The thesis introduces four new decision algorithms for the cognitive agents to learn to cross the intersection and investigates cognitive agents' learning performance for these algorithms and for the realistic values of the car traffic parameters. The thesis investigates the effects of the model parameters on the cognitive agents' learning performance using various statistical performance indicators. Also, the thesis compares the performance of the four new decision algorithms with the performance of the previous decision formula used by the cognitive agents. The results obtained from the statistical analysis indicate that the four new decision algorithms have distinctively different performances for the different selection of the values of the model parameters.

Transposition is a well-known genome rearrangement event that switches two consecutive segments on a genome. The problem of sorting permutations by transpositions has attracted a great amount of interest since it was introduced by Bafna and Pevzner in 1995. However, empirical evidence has reported that, in many genomes, the participation of repeat segments is inevitable during genome evolution and the breakpoints where a transposition occurs are most likely accompanied by a triple of repeated segments. For example, a transposition will transform r x r y z r into r y z r x r, where r is a relative short repeat appearing three times and x and y are long segments involved in the transposition. For this transposition event, the neighbors of segments x and y remain the same before and after the transposition. This type of transposition is called flanked transposition. In this paper, we investigate the problem of sorting by flanked transpositions, which requires a series of flanked transpositions to transform one genome into another. First, we present an O(n) expected running time algorithm to determine if a genome can be transformed into the other genome by a series of flanked transposition for a special case, where each adjacency (roughly two neighbors of two element in the genome) appears once in both input genomes. We then extend the decision algorithm to work for the general case with the same expected running time O(n). Finally, we show that the new version, sorting by minimum number of flanked transpositions is also NP-hard.

Analysis of wastewater reuse options using a multicriteria decision tool for Phuket, Thailand

The act of balancing between water demand and water supply in Phuket Island is facing challenges, suggesting water reuse options in various activities on the island should be properly promoted considering the potential benefits in a variety of dimensions. This research presented options to reuse effluent water from wastewater treatment plants for Phuket Municipality in 3 main activity groups, namely, domestic reuse, agricultural reuse, and raw water for water treatment plants (WTP). Water demand, additional water treatment trains, and the length of the major water distribution pipeline for each water reuse option were designed, and its cost and expenses were calculated. Multi-criteria decision analysis (MCDA) was used by 1000Minds internet-based soft-ware to prioritize the suitability of each water reuse option based on a four-dimensional scorecard, including economic, social, health, and environmental aspects. The decision algorithm for the trade-off scenario based on the government's budget allocation was proposed to obtain weighing without subjective expert opinions. The results revealed that recycling effluent water as raw water for the existing WTP was the first priority, followed by agriculture reuse for planting coconut, Phuket's economic crops, and domestic reuse. There was a significant difference in the total scores of economic and health indicators between the first-and second-priority options because of the difference in the additional treatment system in which the first-priority option applied the microfiltration and reverse osmosis system, which could effectively eliminate viruses and chemical micro -pollutants. In addition, the first priority option required a much smaller piping system than other water reuse options because it relied on the existing plumbing system of WTP, lowering the investment cost, which was a very important indicator for decision-making.

The rapid spread of COVID-19 throughout the world, has led most of the affected countries to close their borders and implement some form of lockdown. Six months after the pandemic started, many countries made decisions tending to relax the lockdown, although without a vaccine or treatment capable of confronting SARS-CoV-2 infection, the situation could be reversed at any time. In this context, the aim of this work was to propose a decision algorithm that will allow to optimize asymptomatic case detections and strategically manage quarantine to prevent the spread of the virus and drive the transition to a managed new normal. This tentative proposal was developed for optimizing and ordering the number of tests for the detection of SARS-CoV-2, analyzing composite samples (group analysis) combining with those samples individually taken from asymptomatic members of cohorts of interest. Cohorts were defined according to their critical role in society and/or their vulnerability. The algorithm includes variables such as cohort priority, number of cohort members in the analysis groups, intra-and intergroup contact, vulnerability to contagion due to the activity performed, and time elapsed since last testing. The proposed tool was illustrated with defined hypothetical cohorts, in which, for the sake of simplification, only one analysis group was considered. The application of this tool allowed to establish in a rational way a priority order to test critical groups in society. This paper designs a process simulation training support system based on microcomputer LAN development. This paper mainly introduces the information interaction technology used in the system to realize the distributed real-time simulation of the running layer. In this paper, a new machine learning model training mode for mobile terminal privacy data is proposed. All the training tasks related to user privacy data are deployed on local terminal devices. There is no need for users to upload data in any form to protect user privacy. System simulation shows that the training system can improve the training efficiency of students. © 2023 IEEE.
