Peripheral vascular access for therapeutic plasma exchange: A practical approach to increased utilization and selecting the most appropriate vascular access

Background Therapeutic plasma exchange (TPE) is used in the treatment of many diseases. At present, peripheral vascular access (PVA) is an underutilized method of vascular access in TPE. It should be considered more frequently due its relatively low risk for adverse events, particularly infections. Methods The Advancing Vascular Access in Apheresis Working Group met in December 2017 for an extensive review and discussion of vascular access for TPE and developed a "road map" providing detailed information regarding clinical situations in which PVA-based TPE would and would not be appropriate. Results The road map is consistent with current recommendations that PVA should be used in combination with TPE whenever possible. PVA should be considered for patients who do not have existing central lines and who are stable. The patient should have peripheral veins that will allow for adequate treatment and must be able to comply with the process of achieving and maintaining peripheral access. There should be expert clinical assessment of veins, and this evaluation may include ultrasound and/or near infrared evaluation. Conditions that would prompt a switch from PVA to an alternate method of venous access include loss of venous access, patient preference, or development of a requirement for very frequent treatment over a long period of time. Conclusions While PVA is not suitable for all patients requiring TPE, it has significant safety advantages over other approaches and should be employed whenever possible.

This is a proposal for an automated detection and remote monitoring system made up of a centralized network of communicating portable electronic devices based on biomedical sensors operating in the IoT context in synergy with wireless sensor network technologies, telemedicine and artificial intelligence. This network will be deployed to monitor a population settling in a target area (cities, region, country, etc.). The goal of this system is the detection and early diagnosis of the disease in people infected with the COVID-19 virus, using a device (such as a bracelet or a chest strap). This device collects in real time all the necessary biomedical measurements of a person, including their location, freeing them from any hospitalization or use of complex and expensive equipment. These informations are then transmitted, via a wireless connection, to a regional or national control center which takes care of its storage in a specialized database. This center executes a decision-making algorithm using artificial intelligence and fuzzy inference engine to detect accurately each possible abnormal change in the supervised biomedical signs reflecting risk factor or indicating the appearance of symptoms characterizing COVID-19 disease. In the positive case, the control system triggers a warning alarm concerning this infected person and requests intervention of the competent authorities to take the necessary measures and actions. Computer simulations with Matlab software tool have been conducted to evaluate the performance of the proposed system. Study findings show that the designed device is suitable for application in COVID-19 patient monitoring.

This work deals with the definability problem by quantifier-free first-order formulas over a finite algebraic structure. We show the problem to be coNP-complete and present a decision algorithm based on a semantical characterization of definable relations as those preserved by isomorphisms of substructures. Our approach also includes the design of an algorithm that computes the isomorphism type of a tuple in a finite algebraic structure. Proofs of soundness and completeness of the algorithms are presented, as well as empirical tests assessing their performances.

We propose a data classification model which yields an average mutual information between a set of objects and a set of class-label decisions as a function of error probability. Optimization of the model consists in minimization of the average mutual information by conditional distributions for the decisions subject to a given constraint on the average error probability. It is equivalent to calculating the rate-distortion function in a scheme of coding the source class labels with a given fidelity when a set of the class labels and a set of the objects are connected by an observation channel with known class-conditional probability distributions. Given set of the objects and known observation channel, a lower bound to the rate-distortion function is calculated. This bound is independent on a decision algorithm and yields a potentially achievable error probability subject to a fixed value of the average mutual information. The obtained bound is useful for evaluating an error probability redundancy of any decision algorithm with given discriminant functions.

Methods and probability of success after early revision of prosthetic joint infections with debridement, antibiotics and implant retention

Prosthetic joint infection (PJI) is a rare-and dreaded-complication of arthroplasty requiring multidisciplinary care. Given the dual goal of treating the infection and maintaining satisfactory function, it is preferable to determine how and when the implanted components can be retained. Bacteria and fungi organize themselves into biofilms that shield them from antibiotics and the immune system. This biofilm is in place after 15 days of active infection. Some antibiotics have a better activity on biofilms. The following factors have a negative impact on the probability of a successful debridement, antibiotics and implant retention (DAIR) intervention: fracture or revision, use of cement, bacteremia, kidney and/or liver failure, immunosuppression and elevated CRP. Hematogenous infections have a worse prognosis than early postoperative infections. Using a decision algorithm increases the chances of DAIR being successful. The KLIC score applies to early postoperative infections (< 4 weeks postoperative and < 3 weeks from the first signs) while the CRIME-80 score applies to hematogenous infections (< 3 weeks from the first signs). Arthroscopic treatments have no role here, whereas DAIR through an arthrotomy is well standardized. Wide spectrum antibiotic therapy, secondarily adapted to the causative microorganism, is indicated for a total of 3 months. The results against the infection are mixed, although following a decision algorithm resolves the infection in about 75% of cases. The functional outcomes and quality of life are close to those of patients who have undergone primary joint replacement. It is not recommended to carry out a second DAIR if the first one fails. It is logical to apply the principles set out for the hip and knee to other joint replacements, and to use the same algorithm. For the upper limb, and especially for reverse shoulder arthroplasty, one must be careful about Cutibacterium acnes infections as they are hard to diagnose. Surgeons should not hes

Information is a fundamental asset in any organization and as such is assigned a value reflecting its importance. We define information to be valuable if it either confirms the choice that a decision maker (DM) would make based on the information acquired on a given alternative or prevents him from making a regrettable choice, or both. We introduce a novel information acquisition algorithm where the value of information is used as a verification and regret-prevention mechanism determining the behavior of the DM. The proposed algorithm shows how the incentives of the DM to continue acquiring information on a given alternative are determined by his attitude toward regret and the relative spread exhibited by the domains on which the characteristics of the alternative are defined. Moreover, our model proves the existence of relative spread scenarios where indecision arises, leading the DM to behave randomly. The generality and flexibility of the model allows to easily develop extensions and applications to decision theory, psychology, economics and operational research. (C) 2018 Elsevier Inc. One of the prevailing ideas in applied concurrency theory and verification is the concept of automata minimization with respect to strong or weak bisimilarity. The minimal automata can be seen as canonical representations of the behaviour modulo the bisimilarity considered. Together with congruence results wrt. process algebraic operators, this can be exploited to alleviate the notorious state space explosion problem. In this paper, we aim at identifying minimal automata and canonical representations for concurrent probabilistic models. We present minimality and canonicity results for probabilistic and Markov automata modulo strong and weak probabilistic bisimilarity, together with the corresponding minimization algorithms. We also consider weak distribution bisimilarity, originally proposed for Markov automata. For this relation, the quest for minimality does not have a unique answer, since fanout minimality clashes with state and transition minimality. A regional-scale landslide early warning methodology applying statistical and physically based approaches in sequence

In response to the sharp increase in geological disasters resulting from localized extreme rainfall events in Korea, a landslide early warning methodology is proposed. The method is based on sequential applications of the statistical and physically based hazard evaluation approaches and devised by combining the strengths of these two mutually complementary approaches. Following the decision algorithm for five phases of the warning level, the statistical evaluation was set to be applied first by using two different rainfall thresholds and one fixed geo-property (landslide susceptibility) threshold to determine a preliminary conservative warning level. To assess whether higher warning levels with higher certainties should be assigned, the physically based evaluation was set for application in the precondition of the preliminary warning stage. This was accomplished by using a rainfall threshold related to slope instability based on an advanced analysis using the physical modeling of landslide-triggering mechanisms. Consequently, a landslide early warning model based on the sequential evaluation approach was developed. The model ran through from transforming raw rainfall data to generating a series of geographic information system-based landslide early warning level maps. The spatially-discriminating capability and temporal applicability of the model on the distributed landslide events of 2009 were discussed by comparing the simulated results with landslide historical data while contemplating the adequacy of the durations and lead times of early warning levels. As a result, several advantages were identified for both spatial and temporal landslide early warning performances in the proposed methodology.

In optical burst switched networks contention is the main source of burst loss. Burst retransmission is a reactive loss recovery mechanism that attempts to resolve contention by retransmitting the contended burst at the optical burst switching layer. Burst cloning is a proactive loss recovery mechanism that attempts to prevent burst loss by sending two copies of the same burst, if the first copy is lost, the second copy may still be able to reach the destination. Burst retransmission is better suited when the load is low, however burst cloning is better suited when the load is high. In this paper, we propose a hybrid scheme for star OBS networks that combines the advantages of burst retransmission at low load with the benefits of burst cloning at high load through a decision algorithm which aims to control the extra load due to the both loss recovery mechanisms. The results obtained from simulation confirm that our hybrid scheme through the decision algorithm can achieve better overall network performance than both burst retransmission scheme and burst cloning scheme.
