Prospective appraisal of clinical diagnostic algorithms for hepatocellular carcinoma surveillance in Chinese patients with chronic hepatitis B infection

Hepatocellular carcinoma (HCC) is often detected at advanced stages among patients with hepatitis B virus (HBV), underscoring the urgency for more precise surveillance tests. Here, we compare the clinical performance of the novel - GAAD (gender [biological sex], age, alpha-fetoprotein [AFP], protein-induced by vitamin K absence-II [PIVKA-II]) and GALAD (gender [biological sex], age, AFP, Lens-culinaris AFP [AFP-L3]), PIVKA-II) algorithms to assess the utility of AFP-L3 for distinguishing HCC from benign chronic liver disease (CLD) in Chinese patients with predominantly chronic HBV infection. Eligible adults were enrolled, and biomarkers were measured using Elecsys (Cobas) or mu TASWAKO assays. In total, 411 participants provided serum samples (HCC, n = 176 [early-stage, n = 110];CLD, n = 136;specificity n = 101). HBV was the underlying disease etiology for most participants (HCC, 95%;benign CLD, 72%). For GAAD (Cobas), GALAD (Cobas), and GALAD (mu TASWAKO), AUCs were 93.1% (95% CI: 90.0-96.2), 93.2% (90.0-96.3), and 92.7% (88.4-96.9) for early-stage, and 95.6% (93.6-97.6), 95.6% (93.6-97.7), and 95.8% (93.2-98.3) for all-stage HCC, versus CLD, respectively. Interestingly, both GAAD and GALAD algorithms demonstrated comparable diagnostic performance regardless of disease etiology (HBV vs. non-HBV), presence of cirrhosis, geographic region, and within pan-tumor specificity panels (p < 0.001), indicating AFP-L3 may have a negligible role in HCC surveillance.

Objectives To review the available evidence supporting the validity of algorithms to identify asthma patients in healthcare administrative databases. Methods A systematic literature search was conducted on multiple databases from inception to March 2020 to identify studies that reported the validity of case-finding asthma algorithms applied to healthcare administrative data. Following an initial screening of abstracts, two investigators independently assessed the full text of studies which met the pre-determined eligibility criteria. Data on study population and algorithm characteristics were extracted. A revised version of the Quality Assessment of diagnostic Accuracy Studies tool was used to evaluate the risk of bias and generalizability of studies. Results A total of 20 studies met the eligibility criteria. algorithms which incorporated >= 1 diagnostic code for asthma over a 1-year period appeared to be valid in both adult and pediatric populations (sensitivity >= 85%;specificity >= 89%;PPV >= 70%). The validity was enhanced when: (1) the time frame to capture asthma cases was increased to two years;(2) >= 2 asthma diagnostic codes were considered;and (3) when diagnoses were recorded by a pulmonologist. algorithms which integrated pharmacy claims data appeared to correctly identify asthma patients;however, the extent to which asthma medications can improve the validity remains unclear. The quality of several studies was high, although disease progression bias and biases related to self-reported data was observed in some studies. Conclusions Healthcare administrative databases are adequate sources to identify asthma patients. More restrictive definitions based on both asthma diagnoses and asthma medications may enhance validity, although further research is required to confirm this hypothesis.

Simple Inclusion of Complex diagnostic algorithms in Infectious Disease Models for Economic Evaluation

Introduction. Cost-effectiveness models for infectious disease interventions often require transmission models that capture the indirect benefits from averted subsequent infections. Compartmental models based on ordinary differential equations are commonly used in this context. Decision trees are frequently used in cost-effectiveness modeling and are well suited to describing diagnostic algorithms. However, complex decision trees are laborious to specify as compartmental models and cumbersome to adapt, limiting the detail of algorithms typically included in transmission models. Methods. We consider an approximation replacing a decision tree with a single holding state for systems where the time scale of the diagnostic algorithm is shorter than time scales associated with disease progression or transmission. We describe recursive algorithms for calculating the outcomes and mean costs and delays associated with decision trees, as well as design strategies for computational implementation. We assess the performance of the approximation in a simple model of transmission/diagnosis and its role in simplifying a model of tuberculosis diagnostics. Results. When diagnostic delays were short relative to recovery rates, our approximation provided a good account of infection dynamics and the cumulative costs of diagnosis and treatment. Proportional errors were below 5% so long as the longest delay in our 2-step algorithm was under 20% of the recovery time scale. Specifying new diagnostic algorithms in our tuberculosis model was reduced from several tens to just a few lines of code. Discussion. For conditions characterized by a diagnostic process that is neither instantaneous nor protracted (relative to transmission dynamics), this novel approach retains the advantages of decision trees while embedding them in more complex models of disease transmission. Concise specification and code reuse increase transparency and reduce potential for error.

Background: Gold standard lupus anticoagulant (LA) assays and reference plasmas do not exist and detection is based on inference in a medley of coagulation assays, creating potential for interpretive discrepancies when applying different algorithms. Objectives: To investigate discrepancies from applying different algorithms to a common data set. Methods: diagnostic data on 311 non-anticoagulated patients LA-positive by dilute Russell's viper venom time (dRVVT) and/or dilute activated partial thromboplastin time (dAPTT) assays were employed to compare algorithms. Routine testing applied interpretive criteria from guidelines endorsing classification as LA-positive despite negative mixing tests, after exclusion of other clotting abnormalities. Integrated testing without mixing tests, and the classical algorithm where negative mixing tests preclude confirm tests, were then retrospectively applied to those data. Results: Initial testing showed 92/311 (29.6%) were LA-positive by dRVVT only, 156/311 (50.1%) by dAPTT only, and 63/311 (20.3%) by both assays. All dAPTT-positive plasmas remained positive with integrated testing but eight dRVVT-positives became negative. Other data suggested they were false-negatives. The classical algorithm altered 52/155 (33.5%) dRVVT and 111/219 (50.7%) dAPTT interpretations to LA-negative because of normal mixing tests, most of which were apparently weak LA in undiluted plasma. Conclusions: The classical algorithm improves diagnostic specificity and confidence but risks missing some genuine LA due to false-negative mixing tests. Integrated testing can be diagnostically accurate and logistically efficient but oversimplifies complex cases. Performing mix and confirm in response to an elevated screen with their interpretation based on clinical data, coagulation screens and the LA-assay design offers a potentially valuable option.

Cost analysis of different diagnostic algorithms for pulmonary tuberculosis varying in placement of Xpert MTB/RIF

Background We undertook cost analysis for diagnosis of pulmonary tuberculosis (PTB) using present algorithm under Revised National Tuberculosis Control programme and using Xpert MTB/RIF (Xpert) as frontline test or in conjunction with smear microscopy and/or chest radiography. Methods Costs were estimated for different strategies: (A) present algorithm involving sputum smear examination followed by antibiotic trial in smear negative patients, repeat smear examination (RE) if symptoms continue and chest radiography if RE negative;(B) direct Xpert;(C) smear microscopy followed by Xpert in smear negative patients;(D) radiography followed by Xpert in those having abnormal pulmonary shadows;and (E) smear examination followed by radiography among smear negative patients and Xpert in presence of abnormal pulmonary shadow. Results Cost to program was estimated lowest with Strategy A and highest with Strategy B. Compared to the latter, program cost reduces by 7%, 4.5%, and 17.4% by strategies C, D, and E, respectively. Cost to the group of individuals with presumptive PTB and their attendants is significantly higher for Strategy A compared to other four strategies. Among the latter, the patients' cost was minimum with Strategy B and maximum with Strategy C. Program cost per case diagnosed was lowest by Strategy A and highest by Strategy B. Patient cost per case diagnosed was highest by Strategy A and lowest by Strategy B. Using Xpert, Strategy E had the lowest program as well as overall cost per case diagnosed. Conclusion Strategy E may be chosen for diagnosis of PTB. When resources would no longer be a constraint, direct Xpert would reduce costs incurred by the patients. © 2016 Tuberculosis Association of India. Published by Elsevier B.V. All rights reserved.

Neuroendocrine tumors (NETs) are relatively uncommon heterogeneous neoplasms arising from endocrine and neuronal origin cells showing highly variable clinical behavior. By the time these tumors are discovered, up to 14% of patients with histologically proven NETs have metastasis, with the liver as the most frequently affected organ. Sometimes, no known primary site can be identified via routine imaging. Neuroendocrine tumors of unknown origin carry a poorer prognosis (compared with metastatic NETs with a known primary site) because of a lack of tailored surgical intervention and appropriate medical therapy (eg, chemotherapy or targeted therapy). A multimethod approach is frequently used in the trial to accurately determine the primary site for NETs of unknown primary sites and may include clinical, laboratory, radiological, histopathological, and surgical data. New molecular techniques using the genomic approach to identify the molecular signature have shown promising results. Various imaging modalities include ultrasound, computed tomography (CT), dual-energy CT, magnetic resonance imaging, and functional and hybrid imaging (positron emission tomography/CT, positron emission tomography/magnetic resonance imaging);somatostatin receptor imaging with new tracers is frequently used in an attempt for localization of the primary site.

BackgroundTraumatic hip dislocations in children and adolescents are rare but can lead to severe outcomes like avascular necrosis. Delayed reductions, often due to overlooked dislocations in initial imaging, pose a major risk. The variability in symptoms and emergency care challenges early diagnosis. This multi-center study evaluates diagnostic approaches to enhance protocols for identifying traumatic hip dislocations in *** retrospective multi-center study included 76 patients (aged <= 17 years) with acute traumatic hip dislocations and open growth plates from 16 German hospitals. Patient data and imaging from 1979 to 2022 were analyzed, with statistical evaluation performed using SPSS under ethical ***-rays (single and biplanar views) were the primary diagnostic method, utilized in 85% of cases. Dislocations were missed in 12% (9 cases), primarily among children under eight years, with half of those under four. Delayed reductions (15.8%, n = 12) were linked to undetected dislocations in imaging in 9 cases. Conventional X-rays frequently missed dislocations, whereas MRI successfully identified all cases. Among the 76 patients, 54 (71%) had associated injuries, with 57.9% (n = 44) diagnosed exclusively via *** diagnosis of traumatic hip dislocations is crucial, as delays increase the risk of femoral head necrosis. An algorithmic approach is essential for young children, where dislocations may not be readily suspected. MRI is vital in the secondary diagnostic phase, providing superior visualization of associated injuries, including acetabular avulsions and soft tissue interpositions highlighting the need for integration of MRI into a unified diagnostic algorithm for children suspected of such *** of evidenceIV.
