Die Entwicklung der pneumologischen Funktionsdiagnostik aber auch der Strukturwandel des Fachgebietes selbst folgern einen vermehrten Datenanfall. Seine Verarbeitung mit mittlerer elektronischer Datentechnik wird im Baukastenprinzip realisierbar vorgestellt. Es wurden für sämtliche konventionellen atemphysiologischen Untersuchungsmethoden einschließlich der Bodyplethysmographie Programme mit Klartext-Interpretation erarbeitet, die nunmehr auch im on-line-Verfahren bereits im klinisch-praktischen Routinebetrieb eingesetzt sind. Als Normwerte wurden hauptsächlich jene von Amrein nach EDV-gerechter Implementierung eingebaut. Weiters werden die Verwendungsmöglichkeiten bei der Dokumentation, Archivierung und Statistik sonstiger pneumologischer Diagnostik- und Therapiemethoden, so der Bronchoskopie, Thorakoskopie, ein pneumologischer Diagnoseschlüssel erwähnt. Eine Fortsetzung dieser Entwicklungsarbeit zusammen mit gleichen Arbeitsgruppen zur Ausnutzung der EDV scheint notwendig und zweckvoll.

A brief introduction to the Danish Cytogenetic Central Register (DCCR) is given, and possibilities, principles and problems concerning the establishment and maintenance of a national cytogenetic register are presented. Various data carrier media for registers in general are discussed, of which the magnetic disc is considered most appropriate. General principles for programs capable of performing insertions, deletions and other modifications in the data base and the principles for the programs in the DCCR are outlined. The individual records should preferably be identified by aid of a central person registration number rather than by name. The data should be stored and sorted by this identification to facilitate retrieval of a desired record. The structure of the records is discussed with regard to prevention of the occurrence of certain errors and the optimization of processing. Flexibility and economy of space are achieved by using programs able to handle records of unequal length, and problems occurring in connection with this are discussed. The question of how to protect sensitive data is dealt with, and 2 different methods used in the DCCR are outlined. Programs capable of analyzing karyotypes with the purpose of recognizing various cytogenetic syndromes are developed for use in the DCCR. Various examples of computing times of typical program runs are presented.

14 hospital departments and several research groups participating in the multiple sclerosis research program of the Deutsche Forschungsgemeinschaft (Dr. Fischer-Bosch-Stiftung) have organized a central documentation pool for clinical data on MS patients. An optical mark reader form documentation system was selected for dataprocessing. The system has three major advantages for the processing of large amounts of data: No code or handwriting involved, registration of a fairly detailed clinical status without omissions, results directly readable by machine; the results may also be read visually, however, making the inclusion of copies in conventional case records useful. On the basis of experience gained in the processing of several hundred cases, it was possible to improve on the first version of the documentation sheet by a revision of details. Initial difficulties are quickly eliminated by a brief training period, errors during the recording of data are identified by a computer plausibility program with printouts of the corrections required.

This corresponds to the way modern economics looks at corporate functions, enterprises, the economy... This fundamental tendency is reflected by the idea that the most important factor in electronic data processing systems is not the speed of operation but the integrative effect. Therefore it is necessary to arrange workflows in a closed control system, under consideration of the most rational path. Thus IDP advances the aims of business organization to a higher level and at the same time provides means for reaching these aims. The primary preconditions for the application of electronic data processing (EDP) are organisational measures including adequate personnel who are needed for preparatory work. Special documentation and coding systems must be introduced for the purpose of data preparation, whereby the size of the EDP equipment (hardware) depends on the problem to be tackled. The EDP programms (software) should be designed in such a way that every doctor is able to operate the EDP equipment even with little knowledge of dataprocessing. Equipment ranges from minicomputers and centralized computer systems to integrated information systems. Whereas today well-designed application models for minicomputers already exist, the integrated information system is still in a development stage.

Numerous electronic systems are used in emergency medicine to support documentation, training, communication and health care itself. These systems often seem to offer valuable support to healthcare professionals. Nevertheless regarding the benefit of these applications to emergency medicine there is hardly any profound evaluation of their functionality. Due to more and more limited financial resources, adequate steps have to be taken into account to characterise and determine the impact of electronic data processing in emergency medicine. Depending on the result of this evaluation it will have to be decided where the use of electronic systems should be fostered and where it is of minor relevance. There is hardly any doubt that the rate of fatal mistakes during treatment in emergency situations will not be improved by the use of any electronic application alone. Still emergency treatment will sustainably benefit from comprehensive, area-wide and obligatory quality management. In this setting electronic systems will play a major role not in "managing quality" but in supporting healthcare professionals.
