Accurate targeting is paramount for transcranial ultrasound procedures. Here we describe a new method for targeting transcranial ultrasound dubbed acoustic signature which uses acoustic feedback from a target's unique reflection patterns to guide the transducer to a previously defined orientation. We characterize the uniqueness and convergence of acoustic signatures on a macaque head phantom. We demonstrate submillimeter targeting accuracy using acoustic signature feedback with two transcranial ultrasound mechanical systems. This method provides an accurate and repeatable method for therapeutic transducer array placement.

This paper is in response to the literature review entitled "Identifying potential types of guidance for supporting student inquiry when using virtual and remote labs in science: a literature review" [Zacharia et al., Educational Technology Research and Development, 63, 257-302 (2015). ] and examines it from a value-add perspective. This literature review illuminates the benefits of leveraging the appropriate types of guidance during the relevant phases of inquiry, a much-needed type of guidance, as it aptly describes how and when to apply the types of guidance for optimal effect. In consideration of some of the weaknesses in the body of literature, as exposed via this literature review, revisiting automated guidance and leveraging any new technological affordances and awareness around adaptive feedback may provide additional insights into how more advanced technologies might contribute to enhanced guidance in computer supported inquiry learning environments.

The guidance strategy for a single rescuer plays a vital role in evacuation management which has yet to be thoroughly studied. Hence, one factor of panic evacuation involving the guidance of a single rescuer is investigated by an extended cellular automaton (CA) model in this paper. The effects of the global panic factor, the hazard position and the environmental familiarity on pedestrians' evacuation under different guidance strategies are discussed. The analysis of simulation results demonstrates that the panic makes a great impact on the pedestrians' evacuation process and their sensitivity to hazards increases consequently. When pedestrians' panic degree is relatively high, a wall-following searching (WFS) strategy should be adopted by a rescuer, otherwise an area coverage searching (ACS) strategy is a good choice. In addition, the position of the hazard hitting in the room significantly affects the pedestrians' panic evacuation process. When the hazard position is closer to the exit, the evacuation time lengthens without a rescuer present. Both ACS strategy and WFS strategy are helpful to increase the evacuation efficiency wherever the hazard is located, and WFS strategy works better. Furthermore, the pedestrians' environmental familiarity has a greater impact on their evacuation process. The better the environmental familiarity, the faster the pedestrian flow rate is. In the early stage of evacuation, the pedestrian flow rate is higher under the ACS strategy, while in the later period, the rate is higher under WFS strategy. For the entire stage of the evacuation, WFS strategy is always more useful than ACS strategy whether evacuees are familiar with the room or not. The study contributes to understanding the importance of the guidance strategy in pedestrians' evacuation and to developing an efficient evacuation strategy for evacuation management.(c) 2022 Published by Elsevier B.V.

In emergencies, guides play a significant role for evacuees who are unfamiliar with the environment. Proper guidance can improve evacuation efficiency, and the position of guidance is particularly important. However, most studies are focused on the simulation model to explore the optimal guidance position and the experiments are less conducted. In addition, the pedestrians' behaviors under emergency conditions are usually different from those in normal conditions. In this work, several controlled experiments were conducted to study the effect of guidance positions and stress levels on evacuation efficiency and pedestrians' behavior. The results show that the pedestrians have a better evacuation performance with guidance under high-stress conditions. Compared with the low-stress level, the pedestrians under the high-stress level evacuate faster and the flow is more continuous. The pedestrians under the high-stress level take longer to make decisions when the exit is unknown but show larger competitiveness when the goal is clear. Besides, guidance can improve evacuation efficiency due to providing exit information to pedestrians. It also reveals that guidance mainly influences the pedestrians who are located nearby and has the greatest effect on helping those far from the exit. These findings can provide empirical data about the effect of guidance on emergency evacuation and further the understanding of pedestrian behaviors in high-stress conditions, which are valuable for crowd management.

This paper is in response to the article entitled, "Identifying Potential Types of guidance for Supporting Student Inquiry When Using Virtual and Remote Labs in Science: A Literature Review" (Zacharia et al. in Educ Technol Res Dev 63(2):257-302, 2015). After a brief recap of the original study, this paper examines the impact of Zacharia et al.'s (2015) two-dimensional system for categorizing guidance tools in inquiry learning contexts. It suggests a number of ways researchers can apply the existing categorization system to advance the field's understanding of how to support student inquiry within digital environments. The paper addresses some limitations of the original study-such as the need to include learner perspectives when evaluating guidance tools-and ends with suggestions for future research on when and how to study guidance mechanisms in computer-supported environments.

This guidance focuses on methodological aspects of lupus anticoagulant (LA) testing, as well as interpretation of results for clinicians. The main changes in how to test for LA compared with the International Society on Thrombosis and Haemostasis Scientific and Standardization Committee 2009 guidelines, in the preanalytical phase are more detailed recommendations on how to handle testing in anticoagulated patients, and the timing of testing. Also, routine coagulation tests are advised to obtain more information on the coagulation background of the patient, and when necessary, anti-Xa activity measurement for heparins or specific assays for direct oral anticoagulants should be performed. The three-step procedure with two test systems (diluted Russell's viper venom time and activated partial thromboplastin time [aPTT]) is essentially not changed. Silica remains the preferable activator in the aPTT assays, but ellagic acid is not excluded. We advise simultaneous performance of the mixing and confirmatory step, in each sample with a prolonged screening test. The confirmatory step can also be performed on a mixture of patient plasma and normal pooled plasma. Cutoff values should be established in-house on at least 120 normals, with transference of the manufacturer's cutoffs as an alternative. Reporting of results has not been changed, although more attention is focused on what clinicians should know. Patient selection for LA testing has been expanded.

Advances in the digitization of health systems and expedited regulatory approvals of innovative treatments have led to increased potential for the use of real-world data (RWD) to generate real-world evidence (RWE) to complement evidence from clinical trials. However, health technology assessment (HTA) bodies and payers have concerns about the ability to generate RWE of sufficient quality to be pivotal evidence of relative treatment effectiveness. Consequently, there is a growing need for HTA bodies and payers to develop guidance for the industry and other stakeholders about the use of RWD/RWE to support access, reimbursement, and pricing. We therefore sought to (i) understand barriers to the use of RWD/RWE by HTA bodies and payers;(ii) review potential solutions in the form of published guidance;and (iii) review findings with selected HTA/payer bodies. Four themes considered key to shaping the generation of robust RWE for HTA bodies and payers were identified as: (i) data (availability, governance, and quality);(ii) methodology (design and analytics);(iii) trust (transparency and reproducibility);and (iv) policy and partnerships. A range of guidance documents were found from trusted sources that could address these themes. These were discussed with HTA experts. This commentary summarizes the potential guidance solutions available to help resolve issues faced by HTA decision-makers in the adoption of RWD/RWE. It shows that there is alignment among stakeholders about the areas that need improvement in the development of RWE and that the key priority to move forward is better collaboration to make data usable for multiple purposes.
