Translating ADA/EASD guidelines and the ACE/AACE Road Maps into Primary Care of Patients with Type 2 Diabetes

Optimal glycemic control is fundamental to diabetes management. New diabetes guidelines suggest that recommended glycated hemoglobin (A1C) targets for Type 2 diabetes can be, for most patients, achieved through a stringent, multi-pronged, multi-disciplined approach comprising early and focused intervention, regular monitoring, timely changes to medication, early use of combination therapy, early use of insulin, and patient empowerment through education. Using examples from real-life patient care, the implementation of these guidelines is reviewed from the perspective of the advanced practice nurse. © 2008 American College of Nurse Practitioners.

Present status of asthma management in Japan

The prevalence of asthma has been increasing in Japan, like in other parts of the world. Asthma management guidelines were developed in Japan in 1993 and 1998. These guidelines have been shown to contribute to the improvement of asthma management in Japan. Most allergists know the guidelines and make use of them. However, only half the non-specialists surveyed knew the guidelines in 1996-97, although the awareness of the guidelines increased over the next 3 years. Further efforts are required to implement the guidelines widely.

Incidental thyroid papillary carcinoma in a thyroglossal duct cyst - management dilemmas

INTRODUCTION: Benign thyroglossal duct tract remnants typically thyroglossal duct cysts, (TDCs) are one of the commonest congenital childhood midline neck masses. Clinical presentation of persistent TDCs in adults is uncommon and the occurrence of incidental primary thyroid papillary carcinoma (TPC) in these cysts is rare. PRESENTATION OF CASE: We report the case of a 32-year-old female with an asymptomatic midline neck mass compatible with a TDC that was excised by Sistrunk's procedure. Histopathological examination revealed an incidental primary intraluminal TPC arising from the wall of the TDC. DISCUSSION: management dilemmas regarding the roles for total thyroidectomy, regional lymph node dissection, radioactive iodine, and suppressive thyroxine therapy are reviewed in the context of relevant evidence based literature. CONCLUSION: The occurrence of incidental TPC in a TDC is rare. Though Sistrunk's procedure is adequate treatment for TDC, based on low, moderate, and high risk stratification, recommendations for further management of incidental TPC in TDC is discussed. Accidents continue to account for close to one half of all death in children from the age of 1 to 14 years with highest frequency in age 8. guidelines for the streetside evaluation and treatment of accident victims are well documented. Obviously a priority list needs to be established for the logical approach to the trauma victim, priorities, which are no different than an approach to the severely and acutely ill emergency department and hospitalized patient. The first priorities in managing the traumatized child include resuscitation of cardiac-pulmonary arrest, control of hemorrhage, treatment of shock and direction of attention to major thoracic, abdominal and head injuries. Restoration of the airway and artificial ventilation may need to be established by artificial ventilation. Cardiac function is also an initial priority and must be maintained with external massage, if there is no cardiac activity. External uncontrolled hemorrhage is best handeled by direct pressure. To facilitate data retreaval and clinical research, but most importantly to expedit transportation of the right patient to the right hospital at the right time several emergency associations have developed field catagorization scores for such trauma patients. Following prehospital evaluation and care of the injured child the victim is transported to the hospital. A rapid and reproduceable schema of immediate, simultaneous and subsequent evaluation and treatment principals should be applied to every child admitted to emergency department with major multiple trauma. © 1993 Blackwell Scientific Publications Ltd.

Outcomes in cancer patients are strongly influenced by timeliness and quality of multidisciplinary interventions. The COVID-19 pandemic has led to severe disruption in cancer care in many countries. This has necessitated several changes in clinical care and workflow, including resource allocation, team segregation and deferment of many elective procedures. Several international oncological societies have proposed guidelines for the care of patients afflicted with breast cancer during the pandemic with a view to optimize resource allocation and maximize risk versus benefit for the individual and society. Clinicians may utilize these recommendations to adapt patient care, based on the current availability of resources and severity of the COVID-19 pandemic in each region. This article discusses the guidelines for care of patients afflicted with breast cancer during the pandemic. © 2020 Women in Cardiology and Related Sciences.

Syracuse has been inscribed on the UNESCO World Heritage List since 2005. Notable for its visible traces of the ancient Greek city, the inscribed sites are mainly located on the Ortigia Islet and in the Neapolis Archaeological Park. In this park, in addition to monuments of global significance such as the Greek theatre carved into the rock in the fifth century bc, wild flora of great interest are preserved. Some species may have been introduced in the classical period. In 2013, significant maintenance interventions of green areas were made, aimed at safeguarding the monuments and the most relevant flora present. This activity provided an opportunity to create, through an interdisciplinary project, some guidelines for the management of the green cover, and has also created new opportunities for the use of the site through thematic routes.

Contemporary organizations need to dynamically adapt to their rapidly changing environment. This imposes challenges to the entire organization, including its supporting information systems (IS). However, current value management practices pay little attention to evolvability issues within the evaluation and deployment of information technology dependent strategic initiatives (ITDSIs). In this paper, we explore how the evolvability of an information technology (IT) application can be assessed and which practices might be affected if an organization is willing to address this issue. For this purpose, we propose a set of five specific management guidelines. These should lead to ITDSIs yielding more evolvable information systems to an organization, and hence enable the organization to more easily adapt to its environment, providing a competitive advantage.

Patients with congenital or developmental anomalies of the cervical spine are encouraged to be active and participate in sports for their overall health. This population has unique anatomic considerations that preclude standard participation guidelines from being relevant. A conscientious evaluation of these patients coupled with an understanding of consensus recommendations for participation in sporting activities should allow for their injury-free participation in sporting activities. After appropriate evaluation a subset of patients with Os odontoideum, Klippel-Feil syndrome, and Down syndrome can safely participate in sporting activities. © 2010 Elsevier Inc.

Introduction: Herpes zoster is a common disease which may cause serious ocular sequelae when it affects the trigeminal nerve. Although involvement of the nasociliary branch of the first division of the trigeminal nerve is well recognised to be associated with serious and direct ocular morbidity, the need for careful long-term follow-up of cases of frontal branch involvement is perhaps less well known. Methods: The pathogenesis, epidemiology, risk factors, clinical course and treatment of herpes zoster are discussed with emphasis on trigeminal nerve involvement. A case report is presented which illustrates the importance of continuing management when the frontal branch of the trigeminal nerve is affected. Discussion: Clinical guidelines are suggested for optometric management of these cases in cooperation with medical practitioners. © 2017 Wiley. All rights reserved.
