Keep it going: maintaining health conversations using relational and instrumental approaches

The initial moments of conversations, particularly discussions addressing difficult or unexpected health topics, establish the context and tone of the entire discussion. We examined how elements of relational and instrumental communication occurring within the first five minutes of the request impacted the length of the discussion. A sample of 16 U.S. tissue banking organizations and their staff (n = 226) making telephone requests for donation to families of tissue-donation eligible patients (n = 1,465) agreed to participate in the research. The first five minutes of 430 (29.4%) audiorecorded request conversations were qualitatively coded and analyzed. The primary outcome measure was whether the conversation continued beyond the first five minutes (yes/no). Aspects of both relational and instrumental communication were associated with discussion length. A logistic regression analysis found that the total number of distinct topics disclosed and requesters' vocal affect during the discussion as well as characteristics of the family member approached about donation predicted conversational maintenance. This research demonstrates that thin slices of effective relational and instrumental communication can have a significant, positive impact on conversation length.

Discussing mental illness in Chinese social media: the impact of influential sources on stigmatization and support among their followers

A content analysis was conducted to examine depression-related discourses by public opinion leaders and mainstream media in the Chinese social media platform Sina Weibo, as well as the impact of these discourses on their followers. The study revealed that stereotypical presentations of people with depression by influential sources often promoted stigmatization of or reduced support for depressed individuals among their followers. Environmental and genetic attributions for the disease in the original posts reduced stigmatization in the response posts. Information about recovery and treatment proved to be a double-edged sword, reducing stigmatization and support among followers at the same time. The use of a crime context to discuss depression in the original posts often promoted stigmatization, while discussing it in a health context increased support in the response posts.

Ambivalence in grief therapy: The interplay between change and self-stability

This article explores the role of ambivalence in grief therapy within a narrative framework. From this perspective, change starts with the occurrence of innovative moments, which can be nullified by reaffirmation of the problematic self-narrative as a sign of ambivalence. This study analyzed ambivalence in six complicated grief cases using the Return to the Problem coding System. Markers of ambivalence emerged in all cases, with a decreasing profile in cases with greater symptomatic improvement, suggesting an association between clinical change and ambivalence evolution in therapy. Addressing ambivalence may bring to light important aspects of client's self-reconstruction after a major loss.

Content Analysis of Diet-Related Mobile Apps: A Self-Regulation Perspective

Diet-related mobile apps hold promise in helping individuals self-regulate their eating behaviors. Nevertheless, little is known about the extent to which diet-related mobile apps incorporate the established behavior change theories and evidence-based practices that promote dietary self-regulation. Guided by the self-regulation aspect of Bandura's social cognitive theory and the Dietary Guidelines for Americans 2010 of the U.S. Department of Agriculture, this study conducts a content analysis of diet-related mobile apps for iPhone (N=400). In terms of the adherence to the self-regulation aspect of the social cognitive theory, results show that although 72.5% of the apps incorporate at least one theoretical construct, few apps tap all three processes of self-regulation (i.e., self-observation/monitoring, judgment process, and self-reaction). Additionally, outcome expectation is manifested in a majority of the diet-related apps. In terms of the Dietary Guidelines for Americans 2010, while the diet-related apps equally emphasize setting goals for calorie intake or nutrient consumption, more apps feature nutrient tracking than calorie tracking. Implications and limitations are discussed.

This study examined the progress and causes of panic disorder symptoms in a client by focusing on family culture, events, and sociocultural background. The study results revealed that the underlying cause of the client's anxiety was originated from family characteristics. The present study suggested a new perspective on panic disorder by delving into family relations and sociocultural background in Korea, such as son preference, academic background-oriented culture, and collectivism. This study intended to provide a new perspective on how the experiences within family could affect panic disorder and to propose family therapeutic method for dealing with panic disorder.

The role of religious affiliation in Christian and unaffiliated bereaved emerging adults' use of religious coping

Though research on bereavement has grown, few studies have focused on emerging adults. To add to the literature, the authors administered the RCOPE to a sample of bereaved college students (analyzed sample N=748) and explored the relationship between self-reported religious affiliation and religious coping strategies used and endorsed as most helpful. Results highlight the rich topography of bereavement previously unexamined in understudied populations (i.e., emerging adults, religiously unaffiliated). Specifically, the Christians/affiliated used negative religious coping strategies most often, yet identified positive strategies as most helpful, whereas the unaffiliated instead used positive strategies most often and identified negative strategies as most helpful.

A Psychophysiological Study of Processing HIV/AIDS Public Service Announcements: The Effects of Novelty Appeals, Sexual Appeals, Narrative Versus Statistical Evidence, and Viewer's Sex

This study used self-reports and physiological measuresheart rate (HR) and skin conductance level (SCL)to examine the effects of novelty appeals, sexual appeals, narrative versus statistical evidence, and viewer's sex on cognitive and emotional processing of HIV/AIDS public service announcements (PSAs) among heterosexually active single college students. Novelty or sexual appeals differently affected self-reported attention and cognitive effort as measured by HR. High- rather than low-novelty HIV/AIDS PSAs, perceived as more attention-eliciting, did not lead to more cognitive effort. High- rather than low-sex HIV/AIDS PSAs, not perceived as more attention-eliciting, led to more cognitive effort as reflected by greater HR deceleration. Novelty or sexual appeals also affected self-reported emotional arousal and SCL differently. HIV/AIDS PSAs with high rather than low levels of novelty or sexual appeals led to greater self-reported arousal, but not greater SCL. Message evidence interacted with message appeals to affect cognitive effort. Participants exerted greater cognitive effort during high- rather than low-novelty narrative HIV/AIDS PSAs, and during low- rather than high-novelty statistical ones. The advantage of high over low sexual appeals was more obvious in statistical than in narrative HIV/AIDS PSAs. Males reported greater emotional arousal than females during high- rather than low-sex HIV/AIDS PSAs.

Women growing older with environmental sensitivities: A grounded theory model of meeting one's needs

This article describes a telephone interview study of 21 women over the age of 65 with environmental sensitivities (ES), including both chemical and electrical hypersensitivities. We employed Charmaz's constructivist grounded theory, using incident, focused, and theoretical coding levels. We were interested in how informants thought their needs would be met as they grew older with ES. We found a central process (that which motivates informants) of balancing on a changing tightrope with great overwhelm with three categories: No Safety Net, The Knowing, and Going Through the Cracks. No Safety Net refers to the inaccessibility of most community and health resources for persons who must avoid chemicals, electromagnetic fields, or both. The Knowing refers to having the awareness that one has been dealt out of the equation and will not receive help from conventional sources. Going Through the Cracks describes living one's life by finding small openings and opportunities for living and experiencing what most take for granted. We describe these categories in detail and appeal to health care providers and the general public to view culture through the eyes of those who are unable to participate in it to an extent considered normal.

How Health Risks Are Pinpointed (or Not) on Social Media: The Portrayal of Waterpipe Smoking on Pinterest

Waterpipe tobacco is among the most rapidly growing trends in tobacco smoking, the growing use and acceptance of which are taking place at a time when cigarette smoking is regarded in an increasingly negative manner (Martinasek, McDermott, & Martini, 2011). Given the health risks and the misperceptions associated with waterpipe smoking, this study focuses on how waterpipe smoking is portrayed and represented on the social media platform Pinterest. In total, 800 Pinterest pins were content analyzed. The vast majority of the sampled pins were primarily image based. More than half of the pins linked to a website external to Pinterest that often redirected the pinner to a commercial website. Waterpipe-focused pins portrayed waterpipe smoking more in a positive light than in a negative light, and conveyed a sense of pleasure, aesthetic, and relaxation, which tended to trigger more repins, likes, and higher levels of engagement. Overall, the risks of waterpipe smoking were not represented on Pinterest, indicating that the social media portrayal of waterpipe smoking needs improvement and could benefit from a public health perspective.

We explored how low-risk, nulliparous pregnant women and their doctors in two contiguous U.S.-Mexico border communities communicate about methods of delivery and how they perceive that the delivery method decision is made. We recruited 18 women through obstetricians in El Paso, Texas (n = 10), and prenatal care providers in Ciudad Juarez, Mexico (n = 8). We observed prenatal care visits, interviewed women prenatally and postpartum, and interviewed the El Paso obstetricians. Qualitative analysis demonstrated that birthing decisions are complex and involve multiple influences, including women's level of knowledge about birth, doctor-patient communication, and women's participation in decision making.
