Limited English Proficient Patients' Perceptions of when Interpreters are Needed and how the Decision to Utilize Interpreters is Made

Our objective was to describe when Hmong- and Spanish-speaking limited English proficient (LEP) patients perceive an interpreter was needed and how they perceive the decision to utilize an interpreter is made in a health care encounter. We interviewed a total of 20 LEP patients. We used a number of strategies to recruit LEP patients including posting language-appropriate flyers at clinics, sending letters to LEP patients and asking our participants to refer their family and friends to participate in our study. Tape-recorded interviews were transcribed, translated, and systematically coded using directed content analysis. Patients identified two factors that influenced LEP Hmong-and Spanish-speaking patients' perception of when interpreters were needed are as follows: (1) their self-identified level of English proficiency and (2) the anticipated complexity of the health care communication. They described three factors that they perceived influenced the decision to utilize interpreters: (1) their self-identified level of English proficiency, (2) access to interpreters;and (3) health system automation of interpreter use. The findings show that patients consider a wide variety of factors when determining if an interpreter is necessary and how the decision to utilize interpreters is made. Given that health care decision making is complex, healthcare systems need to do more to help educate LEP patients of the importance of utilizing interpreters in all healthcare encounters. Additionally, future research should to explore how patients conceptualize the need for and utilization of interpreters in order to provide LEP patients with linguistically appropriate care in ways that are sensitive to their perceptions and needs.

A Multi-Year Study of Tobacco Control in Newspaper Editorials Using Community Characteristic Data and Content Analysis Findings

We content analyzed 1,473 newspaper editorials for topic, tone, and slant, and connected the results to community characteristic data: clean indoor air ordinance status for cities, and official smoking rates for counties. The analysis occurred during a multi-year project aimed at prompting communities to adopt clean indoor air policies. The results showed that most editorials were about tobacco restrictions or ordinances, were neutral in tone, and provided factual information about tobacco control. More editorials were negatively slanted vs. positively slanted toward tobacco control. Most editorials with positive tones were published in newspapers in towns that already had clean indoor air policies. We concluded that editorials might hold increased weight in spurring change, as the percentage of smokers in a city is unrelated to the town enacting a clean indoor air ordinance.

A phenomenology of informal caregiving for HIV/AIDS in India: Exploring women's search for authoritative knowledge, self-efficacy and resilience

Women provide informal caregiving across the world and are often expected to provide care even when ailing themselves. We explore the lived experience of 33 impoverished married Indian women living with HIV and caring for their husbands and/or children living with HIV. Drawing on concepts of authoritative knowledge (AK), self-efficacy and resilience, we found a trajectory that reveals barriers to accessing care, yet we also found that women developed strengths and resiliency. Women gather information, develop environ-info, and deliberatively weigh AK from health workers, family/friends' advice, and their own experiences. Over multiple iterations of this process, they build self-reliance and resilience.

Parental Messages about Substance Use in Early Adolescence: Extending a Model of Drug-Talk Styles

This study extends a typology of parent-offspring drug talk styles to early adolescents and investigates associations with adolescent substance use. Data come from a self-report survey associated with a school-based, 7th grade drug prevention curriculum. Mixed methods were used to collect data across four measurement occasions spanning 30 months. Findings highlight the frequencies of various drug-talk styles over time (i.e., situated direct, ongoing direct, situated indirect, ongoing indirect, never talked), messages adolescents hear from parents, and comparisons of alcohol, cigarette, and marijuana use by drug-talk style. This study advances an understanding of parent-adolescent communication about substances and holds practical implications for drug prevention efforts.

Subjective Estimation of Health Literacy-What Is Measured by the HLS-EU Scale and How Is It Linked to Empowerment?

A recently published study introduced a new instrument (HLS-EU) for the measurement of health literacy (HL) and comparative data at population level for this measure across eight European Union (EU) countries, and revealed significant differences in HL at population level. The HLS-EU instrument consists of items that aim to assess respondents' estimation of difficulties regarding different health-related tasks, and it is therefore a measure of subjective HL. With help of a two-step approach, the present study, which was conducted in Austria, aimed to investigate individual experiences and factors that are associated with high or low values of subjective HL as measured by the HLS-EU scale. In a first step, qualitative data from 20 cognitive interviews for the short version of the HLS-EU scale were analyzed. In a second step, the preliminary results of the qualitative analysis were validated with the help of a standardized survey (n = 800). Results show that subjective HL may be grounded either in high empowerment, including high personal and social skills and resources, or in a lack of health-related experience and reduced critical contemplation but high trust in the health system, which is paired with paternalistic preferences. As a conclusion, it is recommended that research should acknowledge the heterogeneous and multidimensional nature of subjective HL.

Ethical Dilemma: Deception Dynamics in Computer-Mediated Group Communication

Words symbolically represent communicative and behavioral intent, and can provide clues to a communicator's future actions in online communication. This paper describes a sociotechnical study conducted from 2008 through 2015 to identify deceptive communicative intent within group context as manifested in language-action cues. Specifically, this study used an online team-based game that simulates real-world deceptive insider scenarios to examine several dimensions of group communication. First, we studied how language-action cues differ between groups with and groups without a compromised actor. We also examine how these cues differ within groups in terms of the group members' individual and collective interactions with the compromised actor. Finally, we look at how the cues of compromised actors differ from those of noncompromised actors, and how communication behavior changes after an actor is presented with an ethical dilemma. The results of the study further our understanding of language-action cues as indicators for unmasking a potential deceptive insider.

Presenting bibliographic families using information visualization: Evaluation of FRBR-based prototype and hierarchical visualizations

Since their beginnings, bibliographic information systems have been displaying results in the form of long, textual lists. With the development of new data models and computer technologies, the need for new approaches to present and interact with bibliographic data has slowly been maturing. To investigate how this could be accomplished, a prototype system, FrbrVis 1, was designed to present work families within a bibliographic information system using information visualization. This paper reports on two user studies, a controlled and an observational experiment, that have been carried out to assess the Functional Requirements for Bibliographic Records (FRBR)-based against an existing system as well as to test four different hierarchical visual layouts. The results clearly show that FrbrVis offers better performance and user experience compared to the baseline system. The differences between the four hierarchical visualizations (Indented tree, Radial tree, Circlepack, and Sunburst) were, on the other hand, not as pronounced, but the Indented tree and Sunburst design proved to be the most successful, both in performance as well as user perception. The paper therefore not only evaluates the application of a visual presentation of bib-liographic work families, but also provides valuable results regarding the performance and user acceptance of individual hierarchical visualization techniques.

A Multidimensional Investigation of the Effects of Publication Retraction on Scholarly Impact

During the past few decades, the rate of publication retractions has increased dramatically in academia. In this study, we investigate retractions from a quantitative perspective, aiming to answer two fundamental questions. One, how do retractions influence the scholarly impact of retracted papers, authors, and institutions? Two, does this influence propagate to the wider academic community through scholarly associations? Specifically, we analyzed a set of retracted articles indexed in Thomson Reuters Web of Science (WoS), and ran multiple experiments to compare changes in scholarly impact against a control set of nonretracted articles, authors, and institutions. We further applied the Granger Causality test to investigate whether different scientific topics are dynamically affected by retracted papers occurring within those topics. Our results show two key findings: first, the scholarly impact of retracted papers and authors significantly decreases after retraction, and the most severe impact decrease correlates with retractions based on proven, purposeful scientific misconduct;second, this retraction penalty does not seem to spread through the broader scholarly social graph, but instead has a limited and localized effect. Our findings may provide useful insights for scholars or science committees to evaluate the scholarly value of papers, authors, or institutions related to retractions.

Representing Transmedia Fictional Worlds Through Ontology

Currently, there is no structured data standard for representing elements commonly found in transmedia fictional worlds. Although there are websites dedicated to individual universes, the information found on these sites separate out the various formats, concentrate on only the bibliographic aspects of the material, and are only searchable with full text. We have created an ontological model that will allow various user groups interested in transmedia to search for and retrieve the information contained in these worlds based upon their structure. We conducted a domain analysis and user studies based on the contents of Harry Potter, Lord of the Rings, the Marvel Universe, and Star Wars in order to build a new model using Ontology Web Language (OWL) and an artificial intelligence-reasoning engine. This model can infer connections between transmedia properties such as characters, elements of power, items, places, events, and so on. This model will facilitate better search and retrieval of the information contained within these vast story universes for all users interested in them. The result of this project is an OWL ontology reflecting real user needs based upon user research, which is intuitive for users and can be used by artificial intelligence systems.
