Implementation, Revisions, and Student Perceptions of a Pre-Matriculation program in a School of Pharmacy

Objective. To characterize the development, revision, and student perceptions of a pre-matriculation program entitled Pharmacy Readiness and Enrichment program (PREP) in a school of pharmacy. Methods. The program was first implemented in June 2013 for the incoming class of 2016. The main components of PREP were curriculum and scientific content review, professionalism, time management, critical thinking, and personal interactions. Entering student pharmacists were surveyed immediately and six or more months after PREP concluded. Statistical analysis was performed to determine if participation in PREP affected students' academic performance. Results. Student perceptions regarding the program and its components were favorable immediately after PREP but less favorable six or more months later. Statistical analysis showed that students who completed PREP had significantly higher cumulative grade point average (GPA) in pharmacy year one and year two. Conclusion. It is possible to implement a two-day pre-matriculation program with a wide range of components and deliver it prior to the start of the first professional year. It is also possible to deliver some PREP components during the first professional year rather than prior to matriculation into the program. The PREP may serve as a model for other schools of pharmacy that are considering the implementation of a pre-matriculation program, or that have a pre-matriculation program in place and are seeking to modify or update their program.

Do First-Year Seminars Improve College Grades and Retention? A Quantitative Review of Their Overall Effectiveness and an Examination of Moderators of Effectiveness

We review the effectiveness of first-year seminars based on the widely used criteria of first-year grades and the 1-year retention rate. Meta-analytic results indicate that first-year seminars have a small average effect on both first-year grades (k = 89, N = 52,406, = 0.02) and the 1-year retention rate (k = 195, N = 169,666, = 0.11). We discuss the implications of these small effects and show that they are meaningful and have important consequences. Results also indicate that the effectiveness of first-year seminars for both criteria is substantially moderated by first-year seminar characteristics (e.g., type of seminar), institutional characteristics (e.g., 2-year or 4-year institution), and study characteristics (e.g., study design). We use these results to make recommendations about the design of first-year seminars that can maximize the positive effect on both the grades and retention of participants.

Teaching for the Transition: the Canadian PGY-1 Neurosurgery 'Rookie Camp'

Background: Transitioning from medical school to residency is difficult and stressful, necessitating innovation in easing this transition. In response, a Canadian neurosurgical Rookie Camp was designed and implemented to foster acquisition of technical, cognitive and behavioral skills among incoming Canadian post graduate year one (PGY-1) neurosurgery residents. Methods: The inaugural Rookie Camp was held in July 2012 in Halifax. The curriculum was developed based on a national needs-assessment and consisted of a pre-course manual, 7 case-based stations, 4 procedural skills stations and 2 group discussions. The content was clinically focused, used a variety of teaching methods, and addressed multiple CanMEDS competencies. Evaluation included participant and faculty surveys and a pre-course, post-course, and 3-month retention knowledge test. Results: 17 of 23 PGY-1 Canadian neurosurgical residents participated in the Camp. All agreed the course content was relevant for PGY-1 training and the experience prepared them for residency. All participants would recommend the course to future neurosurgical residents. A statistically significant improvement was observed in knowledge related to course content (F-(2,F-32) = 7.572, p < 0.002). There were no significant differences between post-test and retention-test scores at three months. Conclusion: The inaugural Canadian Neurosurgery Rookie Camp for PGY-1 residents was successfully delivered, with engagement from participants, training programs, the Canadian Neurosurgical Society, and the Royal College. In addition to providing fundamental knowledge, which was shown to be retained, the course eased junior residents' transition to residency by fostering camaraderie and socialization within the specialty.

Effects and feasibility of a multi-disciplinary orientation program for newly registered cancer patients: design of a randomised controlled trial

Background: Diagnosis and treatment of cancer can contribute to psychological distress and anxiety amongst patients. Evidence indicates that information giving can be beneficial in reducing patient anxiety, so oncology specific information may have a major impact on this patient group. This study investigates the effects of an orientation program on levels of anxiety and self-efficacy amongst newly registered cancer patients who are about to undergo chemotherapy and/or radiation therapy in the cancer care centre of a large tertiary Australian hospital. Methods: The concept of interventions for orienting new cancer patients needs revisiting due to the dynamic health care system. Historically, most orientation programs at this cancer centre were conducted by one nurse. A randomised controlled trial has been designed to test the effectiveness of an orientation program with bundled interventions;a face-to-face program which includes introduction to the hospital facilities, introduction to the multi-disciplinary team and an overview of treatment side effects and self care strategies. The aim is to orientate patients to the cancer centre and to meet the health care team. We hypothesize that patients who receive this orientation will experience lower levels of anxiety and distress, and a higher level of self-efficacy. Discussion: An orientation program is a common health care service provided by cancer care centres for new cancer patients. Such programs aim to give information to patients at the beginning of their encounter at a cancer care centre. It is clear in the literature that interventions that aim to improve self-efficacy in patients may demonstrate potential improvement in health outcomes. Yet, evidence on the effects of orientation programs for cancer patients on self-efficacy remains scarce, particularly with respect to the use of multidisciplinary team members. A Multi-Level Assessment of the Impact of orientation programs on Student Learning

The purpose of this study was to investigate the influence of orientation programs on student academic and social learning. Moving beyond previous studies, we examined how participation in orientation programming affected student learning and how the impact of these programs on learning varied by organizational characteristics (i.e., institutional control, size of undergraduate enrollment, sponsoring division, and whether the institution has an office designated for managing orientation programs), student entry characteristics (i.e., gender, race, transfer status), and student experiences (i.e., perceived quality of orientation program in helping student transition and in meeting students' expectations, positive experiences with orientation staff, and perceptions of orientation programs and their efficacy in helping students navigate resources and in providing useful campus-based information). Hierarchical linear analysis was used to analyze these cross-level effects. Results demonstrated that having a designated office for orientation programs on campus was important for narrowing the academic learning gap between new-first year and transfer students. Implications for researchers and practitioners were discussed.

Introduction: Emergency nursing is one of the most challenging and understaffed areas of professional nursing. Currently, little is known about how new graduate nurses perceive their experiences as novice emergency nurses. The purpose of this study was to gain an understanding of how new graduate nurses who are oriented to emergency nursing as their first professional area of nursing employment perceive the orientation program and emergency nursing at the beginning and end of a 6-month program. Method: This descriptive study incorporated qualitative and quantitative methods consisting of interviews and a survey. Study participants included 15 women and 3 men. Their mean age was 32 years. All were employed full time in their first position as a graduate nurse. They were asked their perceptions of the program at 3 and 6 months. Results: Participants shared their perceptions of why they had been attracted to the program, characteristics of the emergency department and emergency nursing, being in a new job and role, reflections on their performance, the classroom and clinical components of the program, and their recommendations for future orientation programs. Results of the quantitative survey on participants' perceptions of their first job as a registered nurse indicated that they found the work of the orientation program to be stressful. Discussion: Understanding the experience of new graduate nurses to the emergency setting provides crucial information for orientation program design. Incorporating active teaching and socialization strategies early in the program may facilitate the transition from novice to beginning competent emergency nurse.

Staff development specialists and certified wound, ostomy, continence nurses must be leaders in the design of orientation programs that will assist nurses new to the practice of home health care develop wound care management skills. An orientation developed on evidence-based practice is essential to ensure quality, safe patient outcomes, and positive fiscal outcomes. This article describes the basic elements essential to a comprehensive orientation and suggests several models to assist the orientee in developing competency in wound care.

International medical graduates (IMGs) needs assessment study: comparison between current IMG trainees and program directors

Background: International Medical Graduates (IMGs) training within the Canadian medical education system face unique difficulties. The purpose of this study was to explore the challenges IMGs encounter from the perspective of trainees and their program Directors. Methods: program Directors of residency programs and IMGs at the University of Toronto were anonymously surveyed and asked to rate (using a 5-point Likert scale;1 = least important -5 = most important) the extent to which specific issues were challenging to IMGs and whether an orientation program (in the form of a horizontal curriculum) should be implemented for incoming IMGs prior to starting their residency. Results: Among the IMGs surveyed, Knowledge of the Canadian Healthcare System received the highest mean score (3.93), followed by Knowledge of Pharmaceuticals and Hospital formularies (3.69), and Knowledge of the Hospital System (3.69). In contrast, program Directors felt that Communication with Patients (4.40) was a main challenge faced by IMGs, followed by Communication with Team Members (4.33) and Basic Clinical Skills (4.28). Conclusion: IMGs and program Directors differ in their perspectives as to what are considered challenges to foreign-trained physicians entering residency training. Both groups agree that an orientation program is necessary for incoming IMGs prior to starting their residency program.

Effects of an education program on radiation oncology patients and families

Patients and family members in a radiation oncology department participated in a study of a new education/orientation program. Information was presented in oral and in written form. After the intervention, orientation program participants did not differ significantly from control group members in state anxiety, general distress, treatment adherence, or knowledge of radiation therapy. orientation group patients reported greater satisfaction with clinic care, and the orientation group was significantly more likely to utilize psychological counseling services within the clinic, as well as outside support. Discussion focused on possible explanations for the strikingly favorable program evaluations given by participants despite little evidence that the program was of significant benefit.
