The intent of this study is to explore nursingorientation practicesat the Montreal Children's Hospital. In particular, this paper sought to identifythe gap between the existing preceptorshiporientation program and the ideal orientation program. orientationis defined as those activities and experiences that are planned, implemented, and evaluatedfor either new employees or for those employees changing roles. orientation is a means bywhich new staff membersare introduced to the philosophy, goals, produres, role expedation, physical facilities and special services ina specific setting. A preceptor is defined as an expert nurse who is responsiblefor the orientation of a newnurseto a particular unitfor a limited tirne period. The goal of a preceptor program is to integrate newly employed nursingstaff into the work setting. The study begins with a historicaloverviewof nursing. Then, an account of the existing nursing orientation program at the Montreal Children's Hospital is provided. A detailed review of the Iiteratureon nursingorientation helps shape the recomrnended changes. Interviewswith three recentlyoriented nurses and their preceptors provide more data to launch the recomrnendedchanges to the existing onentation program. By drawngon Boone'sAdult Education Prograrn Design Framework, the reviewd Iiteratureand the results of the survey, ten rewmmendations for improvingthe current nursing orientationprogram are suggested.

Objective: To determine the prevalence, duration, composition, and goals of emergency medicine resident orientation programs. Methods: Eleven-item, descriptive mail surveys were sent by the education committee of SAEM to the program directors of all emergency medicine residency programs approved by the RRC as of January 1992. Results: Sixty-nine of 86 survey instruments (80%) were returned. orientation programs were offered in 64 residencies, 41 PGY1-3 programs, five PGY1-4 programs, and 18 PGY2-4 programs. Results are tabulated below. [GRAPHICS] No statistically significant difference between groups was found. Seven different special courses were offered. Conclusion: Resident orientation programs are used in 93% of surveyed programs in emergency medicine. orientation programs consist mainly of lectures and special courses. There is great variability in the length, composition, goals, and associated courses offered. orientation programs do not differ significantly by postgraduate year in which the residency begins.

An orientation curriculum was developed for incoming residents in emergency medicine at the University of Cincinnati (UC) in July, 1976. The major objectives of the orientation were 1) to identify and delineate the subject matter of emergency medicine, and 2) to review the basic elements of emergency medicine. Results of a pre- and posttest using the residency program at the Medical College of Pennsylvania (MCP) as a control group are presented. The pretest scores of the study groups showed no significant difference at a .05 level. The posttest, however, resulted in a significant improvement of the UC scores (p < 0.05), while little change occurred in the MCP scores. An inter-group evaluation shows the UC group to have out-performed the MCP group significantly (p < 0.05). Results of a one-year posttest showed the UC residents scoring an overall average of six points higher on the test. However, the general difference between the two groups of residents was not significant at the 0.05 level.
