Tailoring Evidence-Based Interventions for New Populations: A Method for program adaptation Through Community Engagement

Evidence-based interventions (EBIs) are an important tool for community health practitioners, but there is often a mismatch between the population in which the EBI was validated and the target population in which it will be used. Methods of planned adaptation identify differences in the new target population and attempt to make changes to the EBI that accommodate these differences without diluting the program's effectiveness. This article outlines an innovative method for eliciting ideas for program modifications and deciding on program changes. The Method for program adaptation through Community Engagement (M-PACE) uses systematic and detailed feedback from program participants to guide adaptation. The authors describe procedures for obtaining high-quality participant feedback and adjudicating recommendations to decide on program changes. M-PACE was developed as part of the adaptation of an evidence-based, arthritis self-management program for older adults. The application and results of the M-PACE method are presented using this case as an example.

Diabetes health disparities among Hispanic populations have been countered with federally funded health promotion and disease prevention programs. Dissemination has focused on program adaptation to local cultural contexts for greater acceptability and sustainability. Taking a broader approach and drawing on our experience in Mexican American communities at the U.S.-Mexico Border, we demonstrate how interventions are adapted at the intersection of multiple cultural contexts: the populations targeted, the community- and university-based entities designing and implementing interventions, and the field team delivering the materials. program adaptation involves negotiations between representatives of all contexts and is imperative in promoting local ownership and program sustainability.

Assessing and Strengthening Evidence-Based program Registries' Usefulness for Social Service program Replication and adaptation

Background: Government and private funders increasingly require social service providers to adopt program models deemed evidence based, particularly as defined by evidence-based program registries, such as What Works Clearinghouse and National Registry of Evidence-Based programs and Practices. These registries summarize the evidence about programs' effectiveness, giving near-exclusive priority to evidence from experimental-design evaluations. The registries' goal is to aid decision making about program replication, but critics suspect the emphasis on evidence from experimental-design evaluations, while ensuring strong internal validity, may inadvertently undermine that goal, which requires strong external validity as well. Objective: The objective of this study is to determine the extent to which the registries' reports provide information about context-specific program implementation factors that affect program outcomes and would thus support decision making about program replication and adaptation. Method: A research-derived rubric was used to rate the extent of context-specific reporting in the population of seven major registries' evidence summaries (N = 55) for youth development programs. Findings: Nearly all (91%) of the reports provide context-specific information about program participants, but far fewer provide context-specific information about implementation fidelity and other variations in program implementation (55%), the program's environment (37%), costs (27%), quality assurance measures (22%), implementing agencies (19%), or staff (15%). Conclusion: Evidence-based program registries provide insufficient information to guide context-sensitive decision making about program replication and adaptation. Registries should supplement their evidence base with nonexperimental evaluations and revise their methodological screens and synthesis-writing protocols to prioritize reportingby both evaluators and the registries themselvesof context-specific implementat

Case study of an adaptation and implementation of a Diabetes Prevention program for individuals with serious mental illness

The Diabetes Prevention program (DPP) is an evidence-based lifestyle intervention developed to decrease the risk for type 2 diabetes and promote weight loss in individuals at risk for diabetes. Individuals with serious mental illness have a greater risk for developing diabetes compared with the general population. In this article, the authors provide a detailed description of the adaptation process of the DPP for individuals with serious mental illness (DPP-SMI). The adaptation process was based on a cultural adaptation framework for modifying evidence-based interventions. To assess the effectiveness of the DPP-SMI, 11 individuals from a community mental health residential agency completed a 22-session pilot study of the adapted program and provided physiological measures before and after the intervention. As primary outcomes, participants were expected to report decreased body weight and increased physical activity per week. Completers had an average weight loss of 19 lbs (8%) and their physical activity increased from 161 to 405 min per week. These preliminary results together with participants' feedback informed further refinement of the DPP-SMI. This case study supports that individuals with serious mental illness can benefit from the DPP-SMI, which is tailored to meet the unique needs of this population group.

This letter responds to Birrell et al.'s (Child and Adolescent Mental Health, 30, 92) article on universal school-based mental health interventions, emphasizing the importance of refining universal approaches rather than discarding them. The letter discusses critical aspects of program adaptation to cultural contexts, the role of meaningful co-design with students and staff, and the integration of targeted strategies in concert with universal approaches. It also highlights the potential of digital health technologies, emphasizing equitable access and personalization to diverse populations. Evidence-Based program Implementation Variability in New Contexts: The Head Start REDI program

This study involved a pilot implementation of an evidence based school readiness intervention in new community contexts. The Head Start Research Based, Developmentally Informed (REDI) program was implemented in preschools on and near a Native American reservation with both indigenous and non-indigenous students and teachers. The REDI curriculum involved 5 subcomponents targeting children's literacy skills and social-emotional development. Teachers were provided with inperson and on-line training and in-person implementation support from a REDI coach. Implementation fidelity included ratings of of curriculum dosage and child engagement, as well as coach ratings of teaching quality using a modified version of the Classroom Assessment Scoring Scale (CLASS). Teachers indicated which adaptations they made to the curriculum and the reasons for these adaptations. Teachers also completed qualitative, post-implementation interviews to obtain their impressions of REDI and the need for planned modifications in their context. Analyses included measures of implementation fidelity and qualitative evaluation of adaptations. Results indicated that teachers found the REDI program both feasible and helpful for their students. Suggestions for planned adaptations included more STEM activities and the freedom to choose the dialogic reading books. Many spontaneous adaptations were noted. Teachers indicated that pragmatic issues, such as lack of time, were the main reasons for adapting the program, and deletions and substitutions of curricular activities were the most common types of changes. Implementation of program subcomponents improved gradually over the course of the school year. Some subcomponents were more likely to be adapted than others.

Off-line programming of industrial robots - adaptation of simulated user programs to the real environment

The requirement to increase productivity by reducing the standstill time of production equipment has reinforced the use of simulation within the robotics sector in recent years. Considerable deviations occur during the transfer of an off-line generated user program from a simulated to a real production cell. These deviations affect the certainty of planning and raise doubt as to the suitability of off-line programming for everyday industrial use. The following contribution deals with the causes and effects of simulation errors during the transfer of an off-line generated user program. It presents an application-oriented test method to measure simulation errors and a practice-oriented procedure to adapt a simulated manufacturing task to the real environment. Correction of the user program takes place in the off-line programmed nominal positions, in the orientations as well as in the angular configuration of the kinematic chain. The fundamental algorithms required, referring to the real application, are further explained and developed. (C) 1997 Elsevier Science B.V.

The role of polymorphic reuse mechanisms in schema evolution in an object-oriented database

A seamless approach to the incremental design and reuse of object-oriented methods and query specifications is presented, We argue for avoiding or minimizing the effort required for manually reprogramming methods and queries due to schema modifications, and demonstrate how the role of polymorphic reuse mechanisms is exploited for enhancing the adaptiveness of database programs against schema evolution in an object-oriented database. The salient features of our approach are the use of propagation patterns and a mechanism for propagation pattern refinement Propagation patterns are employed as an interesting specification formalism for modeling operational requirements. They encourage the reuse of operational specifications against the structural modification of an object-oriented schema. Propagation pattern refinement is suited for the specification of reusable operational modules. It promotes the reusability of propagation patterns toward the operational requirement changes. This approach has a formal basis and emphasizes structural derivation of specifications. The main innovations are in raising the level of abstraction for behavioral schema design, and for making possible the derivation of operational semantics from structural specifications. As a result, both the modularity and reusability of object-oriented schemas are increased.

Evolving scientific code adaptations with modularization frameworks

Software for scientific computing has been traditionally unfavorable to changes, mainly due to its frequent use of old codebases where modern programming practices for modularity have been relatively difficult to adopt. In this paper, We present our 10-year experience of developing and evolving program adaptations in scientific software. Throughout a series of different adaptive scenario implementations, we show why managing adaptation codes using in-house tools in scientific computing has been difficult, and how we address the issue by means of modern software engineering techniques.

Achieving high performance for parallel or distributed programs often requires substantial amounts of information about the programs themselves, about the systems on which they are executing, and about specific program runs. The monitoring system presented in this paper collects, analyzes, and makes application-dependent monitoring information available to the programmer and to the executing program. The system may be used for off-line program analysis, for on-line debugging, and for making on-line, dynamic changes to parallel or distributed programs to enhance their performance. We employ a high-level, uniform data model for the representation of program information and monitoring data. We show how this model may be used for the specification of program views and attributes for monitoring, and we demonstrate how such specifications can be translated into efficient, program-specific monitoring code that uses alternative mechanisms for the distributed analysis and collection to be performed for the specified views. The model's utility has been demonstrated on a wide variety of parallel machines, including several kinds of multiprocessors and a local area network.
