Patient perspectives of process variables in musculoskeletal care pathways

Background: The prevalence of disability caused by musculoskeletal conditions continues to increase. Little research has considered the effect of process variables of a musculoskeletal care pathway on patient outcomes. A process variable is as any modifiable factor in a pathway that can be quantified and measured and that if varied may achieve a different operational or patient outcome. The perspective of patients on what process variables are important in musculoskeletal care pathways remains unknown. Objective: The aim of this study was to investigate which process variables are important to patients and what their experiences of these processes were during the rehabilitation of their musculoskeletal conditions. Method: A qualitative study using a reflexive thematic analytical approach was undertaken. Four focus groups with 12 participants were conducted. Thematic analysis was utilized on the focus group data. Results: Four key themes were generated: 1) process matters;(2) Quantifying progress facilitated patient engagement;(3) Benefits of equitable access of care;and (4) Recovery made easier with navigation. Conclusion: Patients with musculoskeletal conditions recognise the importance of process variables, especially timeliness, order of care, coordination of care delivery, quantifying progress, equity of access and navigation. These findings offer insights to care pathway designers as well as future research opportunities examining the effects of process variables on the outcomes of patients with musculoskeletal conditions.

Evaluation of the effects of process variables on the characteristics of the products from SCFE of a Brazilian mineral coal by statistical methods

The influence of some process variables on the productivity of the fractions (liquid yield times fraction per cent) obtained from SCFE of a Brazilian mineral coal using isopropanol and ethanol as primary solvents is analyzed using statistical techniques. A full factorial 2(3) experimental design was adopted to investigate the effects of process variables (temperature, pressure and cosolvent concentration) on the extraction products. The extracts were analyzed by the Preparative Liquid Chromatography-8 fractions method (PLC-8), a reliable, non-destructive solvent fractionation method, especially developed for coal-derived liquids. Empirical statistical modeling was carried out in order to reproduce the experimental data. Correlations obtained were always greater than 0.98. Four specific process criteria were used to allow process optimization. Results obtained show that it is not possible to maximize both extract productivity and purity (through the minimization of heavy fraction content) simultaneously by manipulating the mentioned process variables. Generally the robotic arc welding process involves sophisticated sensing and control techniques applied to various process variables. One of the major important tasks in the robotic CO, arc welding process is to understand interrelationship between process variables and bead penetration and subsequently develop the mathematical models to predict the desired bead penetration. To achieve the objectives, partial-penetration and single-pass welds were fabricated in 12 mm SS400 plate by using four different process variables. The experimental results were employed to develop the curvilinear and linear equations, and find the process control algorithms for the CO2 are welding process. Mathematical models developed can predict the bead penetration with reasonable accuracy. The process control algorithms developed will be useful for identifying the various problems that result from the CO, arc welding process, and establishing criteria for effective joint design. Strategies for the simultaneous optimization of the structure and the process variables of a protein production plant

process performance models for a multiproduct batch protein plant are used to exploit alternative strategies in the optimization of both the process variables and the structure of the plant. Simple process performance models are used to describe the unit operations, which renders explicit expressions for the size and time factor model in the design of batch plants. In the proposed approach the process variables are optimized regardless the plant structure constraints, which are left as a posterior decision. This optimization is done in a single product-free intermediate storage (SP-FIS) scenario, unbiased with any plant structure. The approach is compared to the case of recipe values for the process variables and to the best optimal solution for the nonconvex mixed integer nonlinear program (MINLP), which arises when simultaneously optimizing the structure and the process variables. This last optimization model is hard to solve and its global solution remains as an open problem. The proposed approach generates solutions Very close to the ones obtained from nonconvex MINLP and is quite superior than simply resorting to recipes. We also study the role of process variables in this approach. It is found that they behave as in continuous processes by trading off cost components, with a smooth dependence on the overall cost. Moreover, for feasible designs that include the size and time constraints that correspond to the plant structure, the process variables accommodate the size and time factors to reduce idle rimes and equipment under-occupancy. Gas-assisted injection molding: the effects of process variables and gas channel geometry

Polystyrene parts with different fib geometries but having the same aspect ratio were molded using gas-assisted injection molding (GAIM). The process variables that have an influence on the gas bubble distribution, residual wall thickness (RWT), fingering formation and mechanical properties were explored. The test results revealed that there is an inherent relationship between gas fingering and gas bubble penetration that has consequences on part strength. Shot size and delay time are the most dominant factors affecting the gas penetration, fingering formation, RWT and mechanical properties of the GAIM, parts. The effects of rib geometries are also discussed. Computer simulation of the GAIM process was carried out using Moldflow, a commercial software. The outcomes predicted by simulation are compared with the experimental results. Based on the results, some useful design guidelines are suggested. (C) 2002 Elsevier Science B.V. There is a need for developing low cost, easily and abundantly available, yet efficient, adsorbents for the removal of phosphates during the tertiary treatment of wastewaters. The tamarind nut shell activated carbon (TNSAC) prepared on a laboratory scale has been used to evaluate its performance for phosphate adsorption. This paper describes the laboratory production of this adsorbent material in its various forms, and discusses the effects of the TNSAC process variables (the unrinsed and rinsed forms of the TNSAC and the impregnation ratio) on its performance in adsorbing phosphate. The material has been shown to be a good alternative adsorbent. As much as 95% phosphate removal by the unrinsed TNSAC is possible in about 30 min under the test conditions. The phosphate adsorbing capacity is about two times higher for the unrinsed TNSAC in comparison to the rinsed TNSAC. The adsorption rates, however, transit to extremely low rates towards the end when equilibrium conditions could be attained in about 2 h contact time. The phosphate removal mechanics are adsorption and precipitation/ion exchange when unrinsed TNSAC is used, and adsorption alone for the rinsed TNSAC. The maximum phosphate removal is found to take place at an impregnation ratio of 10 for both forms of the TNSAC.

Modelling effect of hot rolling process variables on microstructure and mechanical properties of low carbon strip steels

The microstructure and mechanical properties of hot rolled strip are determined by the chemical composition and the hot rolling process. Slab reheating temperature and time, roughing delivery temperature, finishing delivery temperature and coiling temperature are the four main variables to be controlled during hot rolling. In the present work a computer aided model has been developed, combining metallurgical phenomena and the thermal evolution process, which can be used to simulate the microstructural evolution and predict the mechanical properties of low carbon steel strip with 0.05-0-25 wt-% C. The developed model has been employed to analyse the effect of the hot rolling process on the microstructure and mechanical properties of the final product. It is shown that slab reheating temperature and time has little influence on the above. Roughing delivery temperature has a small influence on recrystallisation and grain evolution during roughing passes, but no influence on the microstructure and mechanical properties of the final product. Finishing delivery temperature has a larger influence, not only on recrystallisation between finishing passes, but also on subsequent phase transformation after rolling. Coiling temperature has the strongest influence on the microstructure and mechanical properties of the final product, through changing the cooling rate during phase transformation. The present work is helpful for both prediction of mechanical properties and optimisation of the hot strip rolling process.

Effect of process variables on structure and properties of aluminium-silicon piston alloy

The majority of automotive pistons are cast in one of the groups of low expansion AI-SI eutectic alloys. The structure and mechanical properties of these alloys are highly dependent upon cooling rate, composition, modification and the heat-treatment operations. The effect of these variables were investigated in this study, using locally available automotive 'scrap pistons' as basic raw materials. Strontium master alloy was used as the source of the modifier and a natural-gas-fired crucible furnace was used for melting purposes. The microstructures of the alloy, both in the modified and unmodified conditions, were studied in this investigation. Properties such as the ultimate tensile strength (UTS), percentage elongation, hardness and percentage porosity for both rates of cooling have been determined, both with and without the addition of strontium. Significant changes in structures were observed to occur, especially in the primary and eutectic silicon phases. Full heat treatment has also shown a great influence on the structure and properties of the aluminium-silicon piston alloy. The residual stress in the modified alloy is fully relieved due to thermal treatment. Finally, attempts were made to correlate the observed structures and properties obtained during the experiment. On the influence of process variables on the thermal conditions and properties of high pressure die-cast magnesium alloys

The influence of pressure and velocity in high-pressure magnesium die casting on the thermal conditions and on the casting properties is studied. Specimens with the shape of a tensile test plate with a thickness of 12 mm and a length of 295 mm were cast using the alloys AM20HP, AM50HP, AS41, AE42, AZ91HP. Two gate velocities of the liquid metal 40 and 80 m s(-1), were used for die filling and two pressures, 30 and 70 MPa, were applied to the metal during the solidification phase. Other processing conditions were kept constant. Temperature measurements al different positions in the die and at the metal/die interface were made during the operating cycle. Temperature distributions obtained from a simple two-dimensional numerical heat-flow model were found to agree generally with the measured values. The temperature distribution did not change significantly when varying the pressure and/or velocity. Calculated cooling curves for the alloy AZ91 indicate that the specimens solidified completely within 4.5 s. The measured bulk density of the casting was found to increase with velocity and/or pressure: Consequently, the tensile strength also increased. The density distribution along the specimen was examined, the density being found to decrease as the distance from the gate increases. The surface hardness for each alloy was generally similar at all positions in the test specimen and did not vary much with velocity or pressure. However, the surface hardness was always higher than the hardness inside the specimen, being due to the fine structure at the surface and the coarse structure inside the specimen resulting from different solidification times. Effects of process variables on laser direct formation of thin wall

In this paper, effects of process variables on wall thickness, powder primary efficiency and speed of forming a thin metallic wall in single-pass coaxial laser cladding are investigated, and some resolution models are established and testified experimentally. With some assumptions, each of wall thickness, powder primary efficiency and formation speed can be defined as a function of the process variables. Wall thickness is equal to width of the molten pool created in single-pass laser cladding and determined by laser absorptivity, laser power, initial temperature, scanning speed and thermo-physical properties of clad material. Powder primary efficiency and formation speed are both dependent on an exponential function involving the ratio of melt pool width, which is decided by the process variables, to powder flow diameter. In addition, formation speed is influenced by powder feed rate. In present experiment, a 500 W continual-wave (CW) CO2 laser is used to produce thin-wall samples by single-pass coaxial laser cladding. The experimental results agree well with the calculation values despite some errors. (c) 2005 Elsevier Ltd. All rights reserved.
