BackgroundTranscatheter aortic valve replacement (TAVR) is an increasingly available therapy for the management of aortic stenosis in higher risk populations. Beyond addressing the procedural challenges, centers must attend to the unique requirements of developing TAVR programs from referral to follow-up. AimThe aim of this article is to outline the recommendations for best practice for program development from centers with early and extensive experience. RecommendationsThe guideline-recommended Heart Team approach requires interdisciplinary agreements, delineation of roles and responsibilities, and the development of the role of the TAVR Coordinator. To support appropriate case selection, the screening and evaluation must be organized in a comprehensive clinic visit. In addition to the multimodality imaging tests, the assessment of functional status and frailty is pivotal to the eligibility decision. Throughout the TAVR trajectory, careful attention must be afforded to the integration of geriatric best practices. Pre-procedure care requires patient and family education to manage expectations and facilitate early discharge planning. Peri-procedural care planning, including equipment requirements, monitoring protocols, and emergency intervention agreements, contributes to procedural success. The aims of post-procedure care are to monitor the recovery, facilitate the rapid return to baseline status, and optimize length of stay. TAVR programs require data management strategies to facilitate and monitor program growth, support program evaluation, and meet the requirements for submission to national registries. ConclusionTAVR represents a paradigm shift in the management of structural heart disease. programmatic success and patient outcomes depend on the development of a comprehensive and collaborative program tailored to TAVR. In times of fiscal and political uncertainty, philanthropy has become an increasingly important mechanism for building, maintaining, and expanding neurosurgical research programs. Although philanthropy has historically helped launch many hospital systems, scientists and clinicians have generally relied on government grants and industry investment to support research and program infrastructure. However, competition for funds from all sources has increased at the same time as the pipelines for those funds have eroded. Philanthropy can provide salary support to allow neurosurgeons to pursue research and, ultimately, advance the field to improve outcomes for patients. Funds raised can fill financial gaps to recruit and pay for needed research staff, equipment, and facilities. To foster charitable giving, institutions can develop both a culture and processes to promote and support philanthropy. Furthermore, it is essential to ensure that donor relationships are properly nurtured with ongoing stewardship. In addition to cultivating grateful patients, there are numerous creative models of fundraising for research that can be explored, including venture philanthropy, in which voluntary health organizations or individuals partner with academia and industry to invest in early-stage drug development and other innovations. Other approaches include formation of nonprofit foundations and partnerships with other entities to work jointly on shared development goals.

BACKGROUND: Spotted wing drosophila (SWD), Drosophila suzukii Matsumura, was found along the west coast of the United States, beginning in 2008 and 2009, infesting a wide variety of small and stone fruit crops. This pest is a serious economic threat, as noted in its native range (Asia), because it lays eggs within ripening fruit before harvest, leading to crop loss. The aim of this paper is to describe the process in order to create collaboration, communication routes and evaluation methods in response to a new invasive pest. RESULTS: Funding was secured and a program (SWD*IPM) was quickly developed to address social, economic and biological components. Communication routes were outlined, and a stakeholder advisory panel was established to guide program objectives. A central website was created to host up-to-date information. An online monitoring and mapping program for D. suzukii in Oregon fruit-growing regions illustrated the range, distribution and seasonal abundance of the pest. In addition, a program for backyard fruit growers was initiated to examine citizen scientists' roles in managing D. suzukii infestations in the urban setting. A monitoring kit, laminated educational cards, dry fly mounts and quick-time videos were some of the tools used to educate growers. First-year challenges for dealing with a new pest are discussed. CONCLUSION: The discovery and subsequent response to an exotic pest is information intensive and requires a well-planned, coordinated Extension and evaluation effort. The experiences over the past 5 years of developing and working in Boerum Hill, a 260-bed psychiatric halfway house, provide the basis for this presentation. The paper addresses itself to a definition and discussion of psychiatric rehabilitation and notes the significance of temporariness of stay in a halfway house. The method of the establishment of a proper therapeutic community environment that will bring about a corrective living experience is described. Also, aspects of the significance of the physical plant, the size of the residence, the role of psychotropic medication, and the management and staff attitudes toward regression are discussed. Finally, the psychodynamics of personality change and the importance of high expectations in the rehabilitation effort are presented.

The evaluation of institutional condition is regarded as a crucial factor in creating effective and efficient records management systems. In addition, as part of program development Studies, national and international programs' best practices and guidelines need to be investigated. In this study, the Records Management Capacity Assessment System (RMCAS) is evaluated and some practical examples that could be used for developing records management programs are examined. The basic aim of this study is to identify records management practices and to provide an example to institutions that are in the stage of program development. Continuing education has become increasingly important to the professions, including dietetics. There is some concern, however, that continuing education programs too seldom focus on practice-related problems and do not have a large enough impact on daily practice. A research and development endeavor is in progress to create an alternative approach to continuing education programming. Utilizing a process called the Practice Audit Model, this approach brings practitioners (or their representatives) and educators together to develop continuing education programs that focus on learning needs identified through an empirical assessment of practitioner performance.

Introduction: In many clinical settings, effective cancer control is hampered by barriers related to the acute-care orientation of most medical practices. These include: prioritization of medical problems on the basis of urgency; a problem-oriented medical record that provides no ongoing documentation mechanism for cancer screening; conflicting guidelines; and no method for identifying and targeting persons who are eligible for screening. In contrast, HMOs are uniquely suited to deliver effective cancer control as they have already established many of the linkages required for successful primary and secondary prevention. Methods: This paper describes the principles of a population-based approach to cancer prevention and draws on the experience of Group Health Cooperative of Puget Sound, an HMO in western Washington with more than 20 years’ experience in prevention and centralized cancer screening. Methods for selecting and determining priorities among cancer prevention issues, components of successful screening programs, and measures of short- and long-term evaluation are discussed through examples of screening for breast, prostate, cervix, and colorectal cancers. Conclusions: HMOs have provided leadership in the integration of health services and the promotion of continuity of care. Using a population-based approach, cancer prevention programs may be supported by existing automated information services to reduce morbidity and mortality through early detection.

As community college enrollments continue to rise, the role of the college in the community expands to keep pace. Yet, academic programs designed to meet the needs of an ever more diverse population often lag behind. Traditionally, college has been a place for students to open doors to diverse ideas;here, students bring diversity with them, along with a rich mix of cultures and experiences, to the mutual advantage of fellow students and teachers. Despite the challenge of addressing individual student needs, a focus on a strong literacy component can lead to a successful overall college experience, particularly if individual literacy needs are considered in program design.

When institutions are viewed not as locales but as forms of organizing and regulating human behavior, we see that we "deinstitutionalize" by changing the way we organize and regulate, not necessarily by moving people about. This paper examines residential program development as a process of designing and developing community learning environments. The paper asks "What are the essential behaviors of designing community environments?", that is, what do residential program developers do? Six essential dimensions of program development are identified, and each dimension is critiqued in terms of contemporary practices and needs for innovation. The paper concludes that behavior analysis can play an important role in changing the nature of human service organizations.
