A Review of Public Sector Engagement in Age-Friendly Community Initiatives

The global age-friendly cities and communities (AFCC) movement has centered on the involvement of the public sector, calling on high-ranking authorities to commit to improving the built, social, and service environments of their localities. This interpretive review aimed to advance understanding of the ways in which the public sector is involved in AFCC efforts. Based on emergent themes from peer-reviewed articles from the United States and Canada published since 2010, we derived a two-dimensional framework for conceptualizing variability in public sector involvement, encompassing the internal/external (a) locus of responsibility for cross-sector change and (b) target for cross-sector change. We discuss implications for research, policy, practice, and further knowledge development in AFCC implementation.

Evaluating the implementation of a Pediatric Patient Navigation Center for Children and Youth With Complex Care Needs

NaviCare/SoinsNavi was a research-based patient navigation center for children/youth with complex care needs (CCNs) in New Brunswick, Canada. This process evaluation focused on the implementation of NaviCare/SoinsNavi by exploring indicators related to program coverage and process. Key indicators were explored using client charts and program archives as well as stakeholder interviews with clients (n = 14) and members of the NaviCare/SoinsNavi team (n = 6). NaviCare/SoinsNavi served 162 families and 42 care providers between January 2017 and May 2020. Most clients lived in one of the three major cities in New Brunswick (Saint John, Fredericton, and Moncton), and were English-speaking. The most commonly reported diagnoses were autism spectrum disorder, attention deficit hyperactivity disorder, and global developmental delay. The most prominent themes among NaviCare/SoinsNavi team members related to program coverage and program process were (1) expanding the reach and coverage of NaviCare/SoinsNavi through the implementation of a sustained outreach strategy;(2) maintaining the center's research program;and (3) continuing to build stakeholder relationships to support integrated models of care. The most prominent themes among clients related to program coverage and program process were (1) client satisfaction with program implementation for both caregivers and care providers;(2) recommendations for more sustained outreach strategies;(3) a desire for more hands-on support;and (4) the need to continue building stakeholder relationships to support integrated models of care. This process evaluation demonstrated that NaviCare/SoinsNavi was successfully implemented as planned and produced recommendations for improvements to future iterations of the program.

The National Institutes of Health (NIH) diversity supplements represent an opportunity to enhance diversity in the biomedical research workforce. Despite their potential impact, practical barriers prevent effective use of these resources. The Altman Clinical and Translational Research Institute (ACTRI) at the University of California San Diego (UCSD) developed and implemented of an institutional repository and support system to improve diversity supplement applications. The centralized repository and support system incorporated three strategies: (1) a secure web-based repository housing successful examples of supplements, (2) match making for diversity supplements and mentors, and (3) web-based resources for potential applicants. The repository was implemented in June 2021 and includes 51 supplement samples across 14 divisions of NIH as of November 2024. The repository has been accessed by 51 potential applicants since implementation in 2021. Few investigators have requested match making. Early indicators show a doubling in diversity supplement applications compared to pre-implementation period at UCSD. We outlined our approach to addressing some of the barriers in diversity supplement applications, which could provide a model for other institutions. Pending solutions to some of the lessons learned, coordinated efforts aimed at diversity supplements could be a practical approach toward a more diverse biomedical research workforce.

Core functions and forms in home-delivered meal programs: a stakeholder-driven approach to identifying essential practices

BackgroundThe rapid growth of the aging population underscores the need for programs tailored to older adults' complex health needs. Home-delivered meal programs are critical, providing nutrition and socialization support to older adults with greatest economic and social need. However, variations in local implementation complicate our understanding of how specific program practices influence older adult outcomes. This present study applies the core functions and forms framework to identify and prioritize essential home-delivered meal practices-or forms-that can be replicated by other meal *** study was conducted within a pragmatic randomized effectiveness trial comparing two home-delivered meal models and their impacts on health outcomes among older adults. The study involved nine meal programs across the United States and used a three-phase approach characterized by the following: (1) core functions of home-delivered meal programs were identified based on Title III of the Older Americans Act;(2) the full spectrum of program "forms" was gathered through site visits, surveys, and listening sessions;and (3) a modified e-Delphi process was conducted with stakeholders to determine consensus on the most essential forms of home-delivered meal *** core functions were identified from Title III of the Older Americans Act: provide meals to reduce hunger and malnutrition, provide opportunities for socialization, and provide opportunities to promote health and well-being.
