Clinical relevance of immune checkpoint inhibitors for the analgesic effect of opioids: A retrospective propensity score analysis

AimsThis study aimed to determine the clinical relevance of the influence of coadministration of immune checkpoint inhibitors (ICIs) on the analgesic effects of opioids, focusing on the amount of change in opioid *** study used data from patients who used opioids during anticancer therapy at the Oita University Hospital between September 2014 and October 2023. The primary outcome measure was the amount of change in morphine mg equivalent opioid dose during the period of anticancer therapy. propensity score matching was performed to reduce confounding *** study enrolled 235 patients;101 received ICI and 134 received no ICI. Before propensity score matching, there were significant differences between the ICI and non-ICI groups in lines of anticancer therapy, type of primary cancer, body mass index, maximum opioid dose and the amount of change in opioid dose. Following propensity score matching, 73 patients each were included in the ICI and non-ICI groups. analysis of the propensity score-matched cohort showed a significant increase in the median amount of change in opioid dose in ICI group vs non-ICI group (22.5 vs. 15.0 morphine mg equivalents, interquartile range;0.0, 40.0 vs. 0.0, 30.0, P = .044). Multiple regression analysis identified ICI administration and body mass index as significant independent factors associated with the amount of change in opioid dose (P = .014 and .027, respectively).ConclusionsICI administration significantly increased opioid dosage regardless of patient background. Our findings would provide valuable insight into future pain management strategies.

Introduction: Acute respiratory distress syndrome (ARDS) is a life-threatening inflammatory lung injury often resulting from pneumonia. The efficacy and safety of invimestrocel in patients with pneumonia- induced ARDS have been demonstrated previously in a phase II randomized, open-label trial (the ONE-BRIDGE study). In this study, we aimed to compare data from the intervention (invimestrocel) arm of the ONE-BRIDGE study with matched historical data from a previously established cohort to provide further support for the beneficial effects of invimestrocel in patients with pneumonia-induced ARDS. Methods: Twenty patients from the invimestrocel arm of the ONE-BRIDGE study (Invimestrocel group) and 104 from the historical cohort were included in this study. A matched historical data group (n = 20) was extracted from the historical cohort based on the propensity score analysis using age, sex, PaO2/FIO2 ratio, and high-resolution computed tomography scores. The primary outcomes measured were ventilator-free days (VFDs) during the first 28 days following treatment and mortality on days 28, 60, 90, and 180. Results: Patients in the Invimestrocel group showed higher VFDs (14.8 +/- 11.0 vs. 6.7 +/- 9.4 days;95 % confidence interval [CI], 1.4-14.7;p = 0.0110) and survival rates (log-rank testing;hazard ratio, 0.330;95 % CI, 0.116-0.938) than those in the matched historical data group. Conclusions: The addition of invimestrocel to the standard treatment for pneumonia-induced ARDS may result in early withdrawal from the ventilator and lower mortality. However, further randomized, blinded, and placebo-controlled studies without or addressing multiplicity are required to confirm these findings. (c) 2025 The Author(s). A common feature in cohort studies is when there is a baseline measurement of the continuous follow-up or outcome variable. Common examples include baseline measurements of physiological characteristics such as blood pressure or heart rate in studies where the outcome is post-baseline measurement of the same variable. Methods incorporating the propensity score are increasingly being used to estimate the effects of treatments using observational studies. We examined six methods for incorporating the baseline value of the follow-up variable when using propensity score matching or weighting. These methods differed according to whether the baseline value of the follow-up variable was included or excluded from the propensity score model, whether subsequent regression adjustment was conducted in the matched or weighted sample to adjust for the baseline value of the follow-up variable, and whether the analysis estimated the effect of treatment on the follow-up variable or on the change from baseline. We used Monte Carlo simulations with 750 scenarios. While no analytic method had uniformly superior performance, we provide the following recommendations: first, when using weighting and the ATE is the target estimand, use an augmented inverse probability weighted estimator or include the baseline value of the follow-up variable in the propensity score model and subsequently adjust for the baseline value of the follow-up variable in a regression model. Second, when the ATT is the target estimand, regardless of whether using weighting or matching, analyze change from baseline using a propensity score that excludes the baseline value of the follow-up variable.

Rectal cancer is universally considered a different disease entity as compared to colon cancer, except when dealing with colorectal peritoneal carcinomatosis (PC), in which the two cancers are deemed as the same one. The present study aims to investigate the influence of primary tumor location (colon vs. rectum) on oncologic outcomes in patients undergoing cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) for colorectal peritoneal metastases. Data from three referral centers undergoing CRS plus HIPEC for PC of colorectal origin were prospectively collected. The primary outcomes were overall survival (OS) and disease-free survival (DFS) according to primary tumor location (colic vs. rectal). Univariate and multivariate analyses were performed using the Cox proportional hazard model first on the total number of patients. Then, a propensity score matching using the nearest-neighbour method with a 1:1 ratio was performed. The study included 167 patients: 126 colic and 41 rectal PC. After propensity score matching, rectal primary tumor location was independently predictive of a lower DFS (HR 1.91;95%CI 1.06-3.45;p = 0.031) but not of a lower OS (HR 1.12;95%CI 0.57-2.21;p = 0.73). Post-matching 3-year DFS rates were 49.2% (95%CI 34,3-70,5%) and 19.4% (95%CI 9,4-40,2%) for colic and rectal PC, respectively. The present study shows a significantly worse DFS for rectal cancer PC undergoing CRS and HIPEC compared to colon cancer PC, suggesting a possible need for dedicated pathways for rectal PC patients and posing a question for rectal PC to be considered as a unique disease entity.

Short- and long-term outcomes of robotic and laparoscopic surgery in rectal cancer: a propensity score-matched analysis

PurposeThe relative benefits of robotic surgery and laparoscopic surgery are controversial in rectal cancer. This study compared the short- and long-term outcomes of robotic surgery with those of laparoscopic surgery in patients with rectal cancer using propensity score *** study analyzed consecutive patients who underwent minimally invasive surgery for stage I-III rectal cancer between April 2014 and October 2020. After propensity score matching (PSM), short-term outcomes, relapse-free survival, and overall survival were compared between the robotic surgery (RS) group and the laparoscopic surgery (LS) *** the study period, 251 patients underwent laparoscopic surgery and 193 underwent robotic surgery. PSM resulted in 160 matched pairs (After PSM, the percentages of patients with stage I, II, and III disease were respectively 56%, 19%, and 24% in the LS group and 49%, 23%, and 28% in the RS group (P = 0.462). Median operation time was 239 min in the LS group and 284 min in the RS group (P = 0.001). The C-reactive protein level on postoperative day 3 was significantly lower in the RS group (4.63 mg/mL vs. 5.86 mg/mL, P = 0.013). Postoperative complications, including ileus and Clavien-Dindo grade II or higher complications, were 6% vs. 1% (P = 0.006) and 21% vs. 12% (P = 0.024) in the LS and RS groups, respectively. The 5-year relapse-free survival rate was 88.5% in the LS group and 90.5% in the RS group (P = 0.525);the respective 5-year overall survival rates were 97.3 and 93.8% (P = 0.283). The 5-year cumulative local and distant recurrence rates were 3.3% vs. 3.3% (P = 0.665) and 9.7% vs. 7.7% (P = 0.464) in the LS and RS groups, respectivelyConclusionRobotic surgery can be a feasible treatment modality for rectal cancer, with lower frequencies of postoperative ileus and Clavien-Dindo grade II or higher complications than laparoscopic surgery and no difference in long-term outcomes.

Background: The outcomes and quality of life of patients with locally advanced oral or oropharyngeal squamous cell carcinoma (LAOOPSCC) following upfront surgery (US) are suboptimal. The optimal neoadjuvant therapy involving programmed death-1 inhibitors still remains unknown. We aimed to investigate the antitumor efficacy and quality-of-life benefits of neoadjuvant chemoimmunotherapy (NACI) and compare them with those of US for LAOOPSCC. Methods: A total of 570 patients with OOPSCC who underwent surgical treatment between January 2021 and January 2023 were initially reviewed and we obtained 51 unbiased patients in each of the NACI and US groups through propensity score matching based on age, sex, clinical T and N stage. The antitumor efficacy in patients in the NACI group was evaluated with the pathological response. The postoperative quality of life, as assessed with the EORTC Quality of Life Questionnaire Head and Neck 35, as well as disease-free survival and overall survival, were compared between the groups. Results: In the NACI group, the major pathological response rate was 58.8 % (30/51), and the objective response rate was 66.7 % (34/51). In NACI group, In the NACI group, patients experienced a shorter operative time (p = 0.001) and a reduced length of hospitalization post-surgery (p = 0.041), along with less intraoperative blood loss (p G 0.001) and fewer free flap reconstructions (p G 0.001). Compared with the patients in the US group, those in the NACI group had significantly better postoperative quality of life, including the sensory function (12.7 vs. 23.0, p = 0.021), speech problems (17.3 vs. 35.1, p G 0.001), social eating (20.8 vs. 31.0, p = 0.020), social contact (19.8 vs. 36.5, p G 0.001) and feeling ill (41.2 vs. 51.6, p = 0.021). There was no significant statistical difference in OS (p = 0.825) and DFS (p = 0.473) between the two groups. propensity score (PS) analysis aims to reduce bias in treatment effect estimates obtained from observational studies, which may occur due to non-random differences between treated and untreated groups with respect to covariates related to the outcome. We demonstrate how to use structural equation modeling (SEM) for PS analysis to remove selection bias due to latent covariates and estimate treatment effects on latent outcomes. Following the discussion of the design and analysis stages of PS analysis with SEM, an example is presented which uses the Mplus software to analyze data from the 1999 School and Staffing Survey (SASS) and 2000 Teacher Follow-up Survey (TFS) to estimate the effects teacher's participation in a network of teachers on the teacher's perception of workload manageability.

Objective Low-dose aspirin (LDA) has been shown to reduce the risk of preterm pre-eclampsia and it has been suggested that it should be recommended for all pregnancies. However, some studies have reported an association between LDA and an increased risk of bleeding complications in pregnancy. Our aim was to evaluate the risk of placental abruption and postpartum hemorrhage (PPH) in patients for whom their healthcare provider had recommended prophylactic *** This multicenter cohort study included 72 598 singleton births at 19 hospitals in the USA, between January 2019 and December 2021. Pregnancies complicated by placenta previa/accreta, birth occurring at less than 24 weeks' gestation, multiple pregnancy or those with data missing for aspirin recommendation were excluded. propensity scores were calculated using 20 features spanning sociodemographic factors, medical history, year and hospital providing care. The association between LDA recommendation and placental abruption or PPH was estimated by inverse-probability treatment weighting using the propensity *** We included 71 627 pregnancies in the final analysis. Aspirin was recommended to 6677 (9.3%) and was more likely to be recommended for pregnant individuals who were 35 years or older (P < 0.001), had a body mass index of 30 kg/m2 or higher (P < 0.001), had prepregnancy hypertension (P < 0.001) and who had a Cesarean delivery (P < 0.001). Overall, 1.7% of the study cohort (1205 pregnancies) developed preterm pre-eclampsia: 1.3% in the no-aspirin and 5.8% in the aspirin group. After inverse-probability weighting with propensity scores, aspirin was associated with increased risk of placental abruption (adjusted odds ratio (aOR), 1.44 (95% CI, 1.04-2.00)) and PPH (aOR, 1.21 (95% CI, 1.05-1.39)). Background To analyze disease generalization in patients with ocular myasthenia gravis (OMG) treated with immunosuppression compared with patients without immunosuppression treatment. Methods In this retrospective cohort study, we analyzed data from patients with OMG at seven medical centers in China from January 1, 2015 to May 1, 2019 and compared disease generalization in patients (treated with immunosuppression vs. not treated) within 2 years of disease onset using raw and inverse probability of treatment weighting (IPTW) analyses. Results In the study population of 813 patients with OMG, 425 (52.3%) with immunosuppression had a mean (SD) onset age of 50.0 (15.1) years, and 188 (44.2%) were women. The remaining 388 (47.7%) patients were not immunosuppressed (mean age, 48.4 [15.0] years;185 [47.7%] women). Disease generalization developed in 122 (31.4%) and 37 (8.7%) patients in the non-immunosuppression and immunosuppression groups, respectively. Relative to non-immunosuppression, immunosuppression was associated with a lower risk of generalization in a multivariable-adjusted Cox model (hazard ratio [HR] 0.27;95% confidence interval [CI] 0.18-0.40;p < 0.001) and IPTW-weighted Cox model (HR 0.28;95% CI 0.19-0.42;p < 0.001). In sensitivity analyses, longer duration of immunosuppression was associated with a lower risk of generalization (HR 0.90 for every 1-month increase;95% CI 0.87-0.92;p < 0.001;IPTW-adjusted). Combination therapy with steroids and non-steroidal immunosuppressants showed superior efficacy in reducing the risk of generalization (HR 0.14;95% CI 0.07-0.26;p < 0.001). Conclusion Immunosuppression significantly reduced the 2-year risk of generalization in patients with OMG.
