A temporal-based therapy for children with inconsistent phonological disorder: A case-series

Deficits in temporal auditory processing, and in particular higher gap detection thresholds have been reported in children with inconsistent phonological disorder (IPD). Here we hypothesized that providing these children with extra time for phoneme identification may in turn enhance their phonological planning abilities for production, and accordingly improve not only consistency but also accuracy of their speech. We designed and tested a new temporal-based therapy, inspired by Core Vocabulary Therapy and called it T-CVT, where we digitally lengthened formant transitions between phonemes of words used for therapy. This allowed to target both temporal auditory processing and word phonological planning. Four preschool Persian native children with IPD received T-CVT for eight weeks. We measured changes in speech consistency (% inconsistency) and accuracy (percentage of consonants correct PCC) to assess the effects of the intervention. Therapy significantly improved both consistency and accuracy of word production in the four children: % inconsistency decreased from 59% on average before therapy to 2% post-T-CVT, and PCC increased from 61% to 92% on average. Consistency and accuracy were furthermore maintained or even still improved at three-month follow-up (2% inconsistency and 99% PCC). Results in a nonword repetition task showed the generalization of these effects to non-treated material: % inconsistency for nonwords decreased from 67% to 10% post-therapy, and PCC increased from 63% to 90%. These preliminary findings support the efficacy of the T-CVT intervention for children with IPD who show temporal auditory processing deficits as reflected by higher gap detection thresholds.

Current theories of Autism Spectrum Disorder (ASD) suggest atypical use of context in ASD, but little is known about how these atypicalities influence speech perception. We examined the influence of contextual information (lexical, spectral, and temporal) on phoneme categorization of people with ASD and in typically developed (TD) people. Across three experiments, we found that people with ASD used all types of contextual information for disambiguating speech sounds to the same extent as TD;yet they exhibited a shallower identification curve when phoneme categorization required temporal processing. Overall, the results suggest that the observed atypicalities in speech perception in ASD, including the reduced sensitivity observed here, cannot be attributed merely to the limited ability to utilize context during speech perception.

Dyslexia is heritable and associated with auditory processing deficits. We investigate whether temporal auditory processing is compromised in young children at-risk for dyslexia and whether it is associated with later language and reading skills. We recorded EEG from 17 months-old children with or without familial risk for dyslexia to investigate whether their auditory system was able to detect a temporal change in a tone pattern. The children were followed longitudinally and performed an intelligence- and language development test at ages 4 and 4.5 years. Literacy related skills were measured at the beginning of second grade, and word- and pseudo-word reading fluency were measured at the end of second grade. The EEG responses showed that control children could detect the temporal change as indicated by a mismatch response (MMR). The MMR was not observed in at-risk children. Furthermore, the fronto-central MMR amplitude correlated with preliterate language comprehension and with later word reading fluency, but not with phonological awareness. We conclude that temporal auditory processing differentiates young children at risk for dyslexia from controls and is a precursor of preliterate language comprehension and reading fluency. Investigation of central auditory processing performance in individuals with and without stuttering

Background: Differences in core auditory processing abilities, such as sound timing, frequency discrimination, auditory perception, and auditory memory, have been suggested in stutterers, despite the fact that the precise origin of stuttering is not entirely understood. It is suggested that these differences may play a role in the development of stuttering. The aim of our study is to assess the temporal central auditory processing performance in individuals with stuttering and compare it to individuals without stuttering to uncover potential differences stuttering and compare it to individuals without stuttering to reveal potential differences. Method: In current study, a total of 60 right-handed participants between the ages of 8 and 17 were included, divided into two balanced groups based on age, education, and gender: individuals with stuttering (n = 30) and individuals without stuttering (n = 30). All participants underwent the Frequency Pattern Test, Duration Pattern Test, and Gaps-In-Noise test. Results: Individuals who stutter showed lower performance in the gap detection threshold and the percentage of total correct gap identification parameters of the Frequency Pattern Test, Duration Pattern Test, and Gaps-In-Noise test compared to fluent speakers. Conclusions: The findings indicate a potential relationship between stuttering and central auditory processing. In this context, incorporating central auditory processing measures into the assessment and therapy processes for stuttering may enhance the likelihood of obtaining more accurate results.

Manual dexterity and outcomes in a commonly used test battery to assess auditory processing disorder (APD) in children

Purpose: Sensori-motor processing is a factor in sensory and auditory processing disorders and motor skills are related to language ability. However, data on the extent of motor impairment and its relation to different auditory processing (AP) domains are sparse. This study evaluated the prevalence of impaired manual dexterity (MD) and its relationship with different AP tests comprising a commonly used diagnostic test battery for auditory processing disorder (APD). Method: Data were collected retrospectively from case notes of 513 children who had routine APD assessment. Five diagnostic AP tests of the SCAN-3 test battery ('Filtered Words', 'auditory Figure Ground 0 dB', 'Competing Words-Directed Ears', 'Competing Sentences' and 'Time Compressed Sentences'), MD component of the Movement Assessment Battery for Children-second edition (MABC-2), and Children's Communications Checklist-2 (CCC-2) were used to assess AP, motor skill and language abilities respectively. Percentile rankings <= 10 were considered as failing the tests. Contingency tables were used to compare the proportions of participants with impaired MD between three groups of participants based on AP abilities. Multiple regression analyses were used to evaluate the relationship between MD and AP tests, controlling for age, NVIQ and language. Results: Impaired MD were seen in 37.9%, 40.7% and 47.6% of participants who passed all, failed only one and failed two or more AP tests, respectively. A significant proportion of variance in MD was explained by 'Competing Sentences' and 'Time Compressed Sentences'. Conclusions: Poor MD is common in children with suspected APD. Evaluation of APD needs to include motor and other transdisciplinary assessments for different co-morbid neurodevelopmental conditions to facilitate individualized support and to help formulating an operational definition of APD. With increasing numbers undergoing intervention for hearing impairment at a young age, the clinical need for objective assessment tools of auditory discrimination abilities is growing. Amplitude modulation (AM) sensitivity has been known to be an important factor for speech recognition particularly among cochlear implant (CI) users. It therefore would be useful to develop objective measures of AM detection for future clinical assessment of CI users;this study aimed to verify the feasibility of a neurophysiological approach studying a cohort of normal-hearing participants. The mismatch waveform (MMW) was evaluated as a potential objective measure of AM detection for a low modulation rate (8 Hz). This study also explored the relationship between behavioral AM detection and speech-in-noise recognition. The following measures were obtained for 15 young adults with no known hearing impairment: (1) psychoacoustic sinusoidal AM detection ability for a modulation rate of 8 Hz;(2) neural AM detection thresholds estimated from morphology weighted cortical auditory evoked potentials elicited to various AM depths;and (3) AzBio sentence scores for speech-in-noise recognition. No significant correlations were found between speech recognition and behavioral AM detection measures. Individual neural thresholds were obtained from MMW data and showed significant positive correlations with behavioral AM detection thresholds. Neural thresholds estimated from morphology weighted MMWs provide a novel, objective approach for assessing low-rate AM detection. The findings of this study encourage the continued investigation of the MMW as a neural correlate of low-rate AM detection in larger normal-hearing cohorts and subsequently in clinical cohorts such as cochlear implant users. This study examines whether children with reading disabilities have a temporal processing deficit and whether this deficit is specific to linguistic processing. To resolve this issue, we use the same paradigm to evaluate linguistic and non-linguistic stimuli that are controlled in terms of their complexity. Order judgment tasks were administered in 10 children without reading difficulties and 11 with reading difficulties matched in age (8-9 years). The linguistic task contrasts the voiceless-voiced feature /pa/-/ba/ and the non-linguistic task contrast a mouse squeak (470 Hz) and a duck quack (260 Hz). Results showed that the temporal processing deficit is specific to the linguistic stimuli. The findings are discussed in terms of temporal processing of language related to reading.

Music Perception in Older Adult Cochlear Implant Users: Evidence About Age-related Decline in Rapid temporal processing

Age-related cognitive decline (ARCD) refers to deterioration of basic cognitive functions in normal human aging that mainly associates with areas of sensory processing, attention, memory, and executive control. Because listening to music involves a highly complex integration in various levels of cognitive processing that extend from basic auditrory fuctions to high-level feature extraction and emotional/aesthetic response, music listening tests have been frequently used in clinical reserch settings in the past two decades as a tool for screening areas of deficits in various kinds of pathological populations (e.g., hearing impaired, neurological patients, children with learning disabilities). There are two main reasons why assessing music listening abilities of older adult Cochlear Implant (CI) users is important for aging researchers. First, it shows how the combined influence of ARCD, and the reduced and/or distorted sensory information provided by a hearing implant may globally or selectively deteriorate an implantee's sensitivity to basic elements of music such as rhythm, melody, harmonic structure, and consonance/dissonance. Second, it provides evidence about cognitive domains or processes that display the greatest decline with age. In this study the effects of CI users’ chronological age on their performance on music listening tests were explored through the use of the MuSIC perception test battery, with special emphasis on the influence of aging in perceiving musical stimuli that require rapid temporal processing (e.g., rhythm patterns). Analysis is based on data from a recently completed European multicenter study on music perception in CI users wearing a MED-EL's PULSAR CI 100 or a SONATA TI 100 with an OPUS 2 audio processor ( N = 81, age range: 21,2-82,3 years), and Normally Hearing (NH) adults ( N = 46, age range: 21,1-58,8 years). This longitudinal study investigated temporal auditory processing (frequency modulation and between-channel gap detection) and speech perception (speech-in-noise and categorical perception) in three groups of 6 years 3 months to 6 years 8 months-old children attending grade 1: (1) children with specific language impairment (SLI) and literacy delay (n = 8), (2) children with SLI and normal literacy (n = 10) and (3) typically developing children (n = 14). Moreover, the relations between these auditory processing and speech perception skills and oral language and literacy skills in grade 1 and grade 3 were analyzed. The SLI group with literacy delay scored significantly lower than both other groups on speech perception, but not on temporal auditory processing. Both normal reading groups did not differ in terms of speech perception or auditory processing. Speech perception was significantly related to reading and spelling in grades 1 and 3 and had a unique predictive contribution to reading growth in grade 3, even after controlling reading level, phonological ability, auditory processing and oral language skills in grade 1. These findings indicated that speech perception also had a unique direct impact upon reading development and not only through its relation with phonological awareness. Moreover, Speech perception seemed to be more associated with the development of literacy skills and less with oral language ability. (C) 2011 Elsevier Ltd. All rights reserved.

Maturational changes in the human envelope-following responses

Introduction: The auditory ability to discriminate rapid changes in the envelope of language sounds is essential for speech comprehension. Human envelope-following responses (EFRs) are useful for objective measurement of temporal auditory processing in subjects who are unable to give accurate behavioural responses (e.g., young children). Objective: To evaluate age-dependent changes in EFRs during the first 2 years of life. Methods: The EFRs were recorded in a sample of 16 well babies distributed into 2 age groups (G1: 12 newborns;G2: 4 babies of 2 years). The EFRs were evoked by white noise carrier stimuli with a sweep of modulation frequencies from 20 to 200 Hz presented at 50 dB HL. Results: The age-related changes affected both morphology and EFR detectability. The main morphological differences were at the expense of frequencies below 50 Hz, where the first component P1 was not well defined in either of the 2 age groups. For all modulation frequencies, age significantly affected EFR amplitude and detectability. Conclusions: The present study provides the first evidence on EFR maturation. Some understanding of normal EFR development would facilitate a better use of this technique in clinically-objective measurement of auditory temporal processing in infants who cannot provide reliable behavioural responses.
