Preliminary risk analysis for LNG tankers approaching a maritime terminal

LNG ships may represent a remarkable risk source, especially when approaching a land terminal, not only due to the possible occurrence of maritime accident, but also since they may represent a suitable target for terrorist attacks. A preliminary risk analysis for LNG ships approaching the Panigaglia terminal is carried out: based on literature data and on the characteristics of the location, a spill originated from a sea accident can be excluded;on the contrary, intentional damages may cause the release of a large amount of LNG, giving rise to a pool fire. Consequence analysis shows that dangerous thermal effects are expected within a radius of 700-1500 m;in the location under exam, the impact on resident population will be negligible, for the most probable attack site, and marginal for an occasionally used anchorage. which should be no longer allowed (C) 2009 Elsevier Ltd. Systematic Analysis of Extracting Data on Advance Directives from Patient Electronic Health Records (EHR) in terminal Oncology Patients

Background: Advance directives are legal documents that include living wills and durable health care power of attorney documents. They are critical components of care for seriously ill patients which are designed to be implemented when a patient is terminally ill and incapacitated. We sought to evaluate potential reasons for why advance directives were not appropriately implemented, by reviewing the electronic health record (EHR) in patients with terminal cancer. Methods: A retrospective analysis of the EHR of 500 cancer patients from 1/1/2013 to 12/31/2016 was performed. Data points were manually collected and entered in a central database. Results: Of the 500 patients, 160 (32%) had an advance directive (AD). The most common clinical terminology used by physicians indicating a terminal diagnosis was progressive (36.6%) and palliative (31%). The most common clinical terminology indicating incapacity was altered mental status (25.6%), and not oriented (14%). 34 (6.8%) patients met all criteria of having a terminal diagnosis, a documented AD, and were deemed incapacitated. Of these patients who met all of these data points, their ADs were implemented on average 1.7 days (SD: 4.4 days) after which they should have been. This resulted in a total of 58 days of additional care provided. Discussion: This study provided insight on to how ADs are managed in day to day practice in the hospital. From our analysis it appears that physicians are able to identify when a patient is terminal, however, it is typically later than it should have been recognized. Further studies should be performed focusing on harnessing the power of the EHR and providing physicians formative and evaluative feedback of practice patterns to ensure that ADs are honored when appropriate.

Treatment of Positive Urine Cultures at End-of-Life and the Effect on terminal Delirium Management

Background: The decision to initiate antibiotics in hospice patients that are very near end-of-life is a complex ethical and stewardship decision. Antibiotics may be ordered to improve urinary tract infection-related symptoms, such as delirium. However, infection symptoms may be managed using antipsychotics, antipyretics, antispasmodics, and analgesics instead. Currently, there are no studies that compare symptom management between those who receive antibiotics and those who do not. Methods: A retrospective chart review was conducted for patients admitted to a hospice inpatient unit. Charts were included if the patient was admitted for delirium and had a Palliative Performance Scale score <= 40%, the urine culture was positive for organism growth, and the patient died while in the HIU. Clinical and demographic data was collected. Medication use was tallied for the 5 days prior to the date of death. Results: Sixty-one charts met the inclusion criteria. Thirty-five patients received antibiotics (ABX+) and 26 did not (ABX-). There was no difference in any medication consumption between groups during the 5 days prior to death. The ABX+ group died 8.2 days after obtaining the urine sample vs 6 days (P =0.046). The ABX+ group had more documented urinary tract-specific infection symptoms (66% vs 38%, P =0.042). More than half of antibiotic courses were discontinued prematurely. Conclusion: The results of this study do not show a difference in overall medication consumption between groups, which suggests that antibiotics may not help improve terminal delirium symptoms in those with a suspected urinary tract infection at end-of-life.

The article describes the creation of regional transport terminals and their usage as a tool to control material flows in the distribution channels. In the first part of the article the authors analyze the role of intermodal transport terminal within international business system, legal aspects of Lithuanian and EU regulations as well as the laws of Lithuanian transport regional transport terminal establishment in Kaunas. The evaluation of alternative locations for establishment of terminal in Kaunas region is presented.

A COMPARATIVE COST-ANALYSIS OF terminal CANCER CARE IN HOME HOSPICE PATIENTS AND CONTROLS

A comparison is made between the expenditure during the last 90 days of life on 98 terminal cancer patients cared for by a home based hospice service and that on matched patients dying without the home hospice service. The control patients were matched for site of primary cancer, age and sex. The individual records of both groups of patients were analyzed and costed. It was found that the costs of providing 24 hour comprehensive medical and nursing care at home to those dying of cancer and support for their families was no more expensive than traditional institutional care.

Dependence of γ-aminobutyric acid modulation of cholinergic transmission on nitric oxide and purines in cat terminal ileum

The possible involvement of purines and/or nitric oxide (NO) in the gamma-aminobutyric acid (GABA)(A) receptor-mediated effects on the spontaneous activity of isolated preparations from longitudinal and circular muscles of cat terminal ileum was investigated. GABA had biphasic effects, which were neurogenic and muscarinic. ATP and adenosine dose dependently inhibited the activity of the muscles. A contractile response evoked by the nucleotide only was also observed. The effects of the purines were equipotent and resistant to N-Omega-nitro-L-arginine (L-NNA), tetrodotoxin and to desensitization by alpha,beta-methylene adenosine 5'-triphosphate (alpha,beta-meATP), except for the contractile effect of ATP, which was abolished by alpha,beta-meATP. Pretreatment of the preparations with ATP or adenosine produced: (i) desensitization to the effects of the respective purinoceptor agonist only;and (ii) suppression of the GABA-induced responses of longitudinal and circular muscles. Hemoglobin and L-NNA greatly reduced or completely blocked the GABA(A)-induced relaxation and decreased the GABA(A)-induced contraction. Our results indicate that purines and NO, to a different extent, mediate the relaxant phase of the GABA effects in both layers. Interactions between muscarinic cholinoceptors and GABA-nitrergic pathway and a concomitant activation of postjunctional P-1 and P-2y purinoceptors are suggested to explain the prejunctional biphasic effects of GABA. Antibacterial peptides have received increasing attention as a new pharmaceutical substance. But the molecular mechanism of lysis is still poorly understood. CMIV gene and mutant CMIV gene in GST fusion system were expressed. After cleaving with different cleavage reagents, the peptide with an excess of N-terminus and with an un-amidated C-terminus stopped the activity while the peptide with an excess Asn at the C-terminus had the activity level the same as natural CMIV. The results showed that the terminal structure of cecropin CMIV played an important role in its biological activity.

Characterization of multiple isoforms of protein 4.1R expressed during erythroid terminal differentiation

In erythrocytes, 80-kD protein 4.1R regulates critical membrane properties of deformability and mechanical strength. However, previously obtained data suggest that multiple isoforms of protein 4.1, generated by alternative pre-mRNA splicing, are expressed during erythroid differentiation. Erythroid precursors use two splice acceptor sites at the 5' end of exon 2, thereby generating two populations of 4.1 RNA: one that includes an upstream AUG-1 in exon 2' and encodes high molecular weight isoforms, and another that skips AUG-1 in exon 2' and encodes 4.1 by initiation at a downstream AUG-2 in exon 4. To begin an analysis of the complex picture of protein 4.1R expression and function during erythropoiesis, we determined the number and primary structure of 4.1R isoforms expressed in erythroblasts. We used reverse-transcription polymerase chain reaction to amplify and clone full-length coding domains from the population of 4.1R cDNA containing AUG-1 and the population excluding AUG-1. We observed an impressive repertoire of 4.1R isoforms that included 7 major and 11 minor splice variants, thus providing the first definitive characterization of 4.1R primary structures in a single-cell lineage. 4.1R isoforms, transfected into COS-7 cells, distributed to the nucleus, cytoplasm, plasma membrane, and apparent centrosome. We confirmed previous studies showing that inclusion of exon 16 was essential for efficient nuclear localization. Unexpectedly, immunochemical analysis of COS-7 cells transfected with an isoform lacking both AUG-1 and AUG-2 documented that a previously unidentified downstream translation initiation codon located in exon 8 can regulate expression of 4.1R. We speculate that the repertoire of primary structure of 4.1R dictates its distinct binding partners and functions during erythropoiesis. (C) 1998 by The American Society of Hematology.

The use of life review to enhance spiritual well-being in patients with terminal illnesses: An integrative review

Aims and objectivesTo conduct an integrative review of the current literature on using life review as an intervention to address the spiritual need of patients with terminal illnesses. BackgroundPalliative care highlights the holistic approach of care including the spiritual aspect. Life review has been used in palliative nursing intending to enhance patients' emotional and spiritual well-being, and quality of life. However, there is a lack of publications that provide a structured overview on life review programmes and their effectiveness. DesignIntegrative review. MethodsThe Whittemore and Knafl integrative review method was used. Five major online databases were included in our literature search. The keywords used were life review and palliative care, terminal care, terminally ill, death & dying, hospice, spiritual wellbeing, spirituality. Seven primary papers were identified, critically appraised and synthesised in the final review. ResultsThere are limited clinical studies on life review programmes for patients with terminal illness. The research design of these studies is too widely varied for meta-analysis. Here, we identified two major programmes of life review as an intervention to address the spiritual well-being of patients with terminal illness. However, repeated studies on the effectiveness of these two programmes are lacking. The shorter programme of life review is more likely to be applicable and effective for terminal patients. ConclusionsFurther research in this area is required to provide strong evidence on the effectiveness and applicability of life review in patients receiving palliative care. Relevance to clinical nursingThis review adds weight to the need of a better understanding on the use of life review in addressing the spiritual needs of patients with terminal illness. Such understanding would provide evidence for the use of life review as an alternative approach in palliative care delivery.

Design of Intelligent Fusion terminal System with Fog Computing Capability in Distribution Area based on Large Capacity CPU

The intelligent fusion terminal in the distribution area usually adopts the mode of cooperation between the cloud and the edge, and the workload of manual operation and maintenance is large. Therefore, an intelligent fusion terminal system in the distribution area with fog computing capability based on a high-capacity CPU is proposed. Follow the & ldquo;cloud pipe edge end & rdquo;construction framework of smart IOT system, and take this framework as the edge computing node of distribution station area and power consumption side. Mt7622b chip in Linux operating system with openwrt firmware is used as the main control chip of edge agent gateway equipment, and the recursive least square method is used to realize the data fusion of power acquisition service in distribution station area and power distribution demand terminal. The test results show that the designed system can realize real-time monitoring of power consumption and distribution data and power quality management in the distribution station area, and the data processing delay is less than 100ms, which provides a reference for the intelligent fusion terminal system in the distribution station area.
