Amyloid Detection Using a Peltier-Based Device

Amyloid aggregation of polypeptides is related to a growing number of pathologic states known as amyloid disorders. At present, it is clear that any proteins submitted to appropriate physicochemical environment can acquire fibrilar conformation. Fourier transform infrared spectroscopy (FTIR) has been a widely used technique to study temperature- induced amyloid-fibrils formation in vitro. In this way, strict changes and temperature controls are required to characterize the physicochemical basis of the amyloid-fibrils formation. In this article, the development of a highly efficient and accurate Peltier-based system to improve FTIR measurements is presented (see An Old Physics Phenomenon Applied to a Serious Biomedical Pathology. The accuracy of the thermostatic control was tested with biophysical parameters on biological samples probing its reproducibility. The design of the present device contributes to maintain the FTIR environment stable, which represents a real contribution to improve the spectral quality and thus, the reliability of the results.

Proposed model of a digital video-based home telecare system

The purpose of our study was to develop a generic reference model for home telecare in a digital television environment and to suggest how it might be deployed at a test village. We describe here an architecture based on digital video broadcast standards, its components, services, and required additional technical solutions to provide a unified home telecare solution. We also suggest different user groups within the village context and possible service scenarios for them. We conclude that, in general, digital television provides a potentially flexible and promising platform for home telecare services.

Modeling and implementing a database on drugs into a hospital intranet

Our objective was to develop a drug information service, implementing a database on drugs in our university hospitals information system. Theriaque is a database, maintained by a group of pharmacists and physicians, on all the drugs available in France. Before its implementation we modeled its content (chemical classes, active components, excipients, indications, contra-indications, side effects, and so on) according to an object-oriented method. Then we designed HTML pages whose appearance translates the structure of classes of objects of the model. Fields in pages are dynamically fulfilled by the results of queries to a relational database in which information on drugs is stored. This allowed a fast implementation and did not imply to port a client application on the thousands of workstations over the network. The interface provides end-users with an easy-to-use and natural way to access information related to drugs in an intranet environment. (C) 1998 Elsevier Science;Ltd. All rights reserved.

Participatory design with children in the development of a support system for patient-centered care in pediatric oncology

Developing software for children with severe illness poses a number of design challenges. In this paper we describe participatory design methods used in the development of SISOM, a support system for children with cancer age 7-12 to help children elicit and report their symptoms/problems in a child-friendly, age-adjusted manner, and to assist clinicians at the point of care in addressing and integrating children's reported symptoms and problems in patient care. The particular design challenges in the development of a clinical support tool for seriously ill children are described, followed by the participatory design techniques we used to meet these challenges. Healthy children and children with cancer participated actively in different stages of the design process. We describe how children contributed to the graphical design of the system's interface;selection of understandable, child-friendly terms used in the system;iconic and graphical representations;and its usability. The methods applied helped us to significantly improve and adapt SISOM to children's cognitive and emotional developmental stage. Working with children as partners in the design also provided important insights into the role children can play in participatory design that may be helpful for other system developers who wish to design support applications for ill children. Children had very creative design ideas that considerably improved the software. However, system development for seriously ill children also requires psychological and pedagogical insights and design and usability expertise. Clinical safety of England's national programme for IT: A retrospective analysis of all reported safety events 2005 to 2011

Objectives: To analyse patient safety events associated with England's national programme for IT (NPfIT). Methods: Retrospective analysis of all safety events managed by a dedicated IT safety team between September 2005 and November 2011 was undertaken. Events were reviewed against an existing classification for problems associated with IT. The proportion of reported events per problem type, consequences, source of report, resolution within 24 h, time of day and day of week were examined. Sub-group analyses were undertaken for events involving patient harm and those that occurred on a large scale. Results: Of the 850 events analysed, 68% (n = 574) described potentially hazardous circumstances, 24% (n = 205) had an observable impact on care delivery, 4% (n = 36) were a near miss, and 3% (n = 22) were associated with patient harm, including three deaths (0.35%). Eleven events did not have a noticeable consequence (1%) and two were complaints (<1%). Amongst the events 1606 separate contributing problems were identified. Of these 92% were predominately associated with technical rather than human factors. Problems involving human factors were four times as likely to result in patient harm than technical problems (25% versus 8%;OR 3.98, 95% CI 1.90-8.34). Large-scale events affecting 10 or more individuals or multiple IT systems accounted for 23% (n = 191) of the sample and were significantly more likely to result in a near miss (6% versus 4%) or impact the delivery of care (39% versus 20%;p < 0.001). Conclusion: Events associated with NPfIT reinforce that the use of IT does create hazardous circumstances and can lead to patient harm or death. Large-scale patient safety events have the potential to affect many patients and clinicians, and this suggests that addressing them should be a priority for all major IT implementations. Merits of usability testing for PACS selection

Objectives: To compare the usability of different Picture Archiving and Communication System (PACS) workstations, determine whether a usability test has added value with respect to the traditional way of comparing PACSs based on functional requirements, and to evaluate the appropriateness of a task-based methodology for a PACS usability test. Methods: A task-based usability test of four PACS workstations was performed. Radiologists' subjective responses to the PACSs and their performance on the tasks were measured. To mimic the traditional PACS selection process, two functional requirements were defined which the PACSs met in varying degrees. The focus of the usability test was on the aspects of the PACS related to these requirements. The usability results were compared to the PACSs' ability to meet the functional requirements. Results: One PACS outperformed the other PACSs both in terms of subjective preference and task performance, indicating its superior usability. There were differences in usability between PACSs with identical functionality. Also, a PACS with theoretically advantageous functionality for a given task did not necessarily have better usability for this task than a PACS without this functionality. There was a discrepancy between participants' subjective preferences and their task performance, which indicates that it is vital to include performance measures in the usability assessment so that it accurately reflects the efficiency of interaction. Conclusions: The differences in usability between PACSs with identical functionality indicate that functional requirements alone are insufficient to determine a PACS's overall quality. A usability test should therefore be used in addition to a functional requirement list in a PACS selection process to ensure that a hospital buys the PACS with the highest quality. Direct-to-reverberant energy ratio sensitivity

Although the ratio of direct-to-reverberant sound energy is known to be an important acoustic cue to sound source distance, human sensitivity to changes in this cue is largely unknown. Here, direct-to-reverberant energy discrimination thresholds were measured for six listeners using virtual sound source techniques that allow for convenient and precise control of this stimulus parameter. Four different types of source stimuli were tested: a 50 ms noise burst with abrupt onset/offset, a 300 ms duration noise burst with gradual onset/offset, a speech syllable, and an impulse. Over a range of direct-to-reverberant ratios from 0 to 20 dB, an adaptive 2AFC procedure (3-down, 1-up) was used to measure discrimination thresholds. For all stimuli, these thresholds ranged from 5 to 6 dB. A post hoc fitting procedure confirmed that slopes of the psychometric functions were homogeneous across stimulus conditions and listeners. These threshold results suggest that direct-to-reverberant energy ratio by itself provides only a course coding of sound source distance, because threshold values correspond to greater than 2-fold changes in physical distance for the acoustic environment under examination. Effect of predefined order sets and usability problems on efficiency of computerized medication ordering

Objectives: To study the effect of predefined order sets on the efficiency of computerized medication ordering, and to analyze the effect of different types of usability problems on ordering efficiency. Methods: Crossover study to comparing the efficiency of two methods of ordering (with and without use of predefined order sets) in a laboratory setting using a computerized physician order entry system (CPOE). The excess number of mouse clicks and keystrokes (the difference in number of mouse clicks and keystrokes needed by each physician and the minimally required numbers to accomplish the ordering tasks) for each method was measured and per physician, occurrences of usability problems during the task sessions were recorded. Observed usability problems were categorized using Zhang et al.'s heuristic principles of good user interface design. The effect of different types of usability problems on the excess number of mouse clicks and keystrokes was statistically analyzed. Results: The median excess number of mouse clicks and keystrokes needed by physicians was 6.2 times lower in the method with predefined order sets (p < 0.01). The excess number of mouse clicks and keystrokes was significantly increased by vague and erroneous system messages with a factor of 2.62 (95% CI 2.24-3.07), the use of unfamiliar language and terminology by a factor of 1.28 (95% CI 1.14-1.43), and non-informative system feedback by a factor of 1.15 (95% CI 1.03-1.28), respectively. Other categories of usability problems had little influence on ordering efficiency. Conclusions: Predefined order sets can improve the efficiency of computerized ordering by reducing the excess number of mouse clicks and keystrokes. However, the efficiency of computerized ordering can be significantly impaired by usability problems due to vague and incorrect system messages, unfamiliar language, and non-informative system feedback.
