Social interaction and conversation is an essential aspect of human behavior, yet existing methods for coding conversations are outdated, and often can only be used in contrived research settings. The Taxonomy of Dyadic Conversation (TDC) is a coding system designed to code dyadic interactions in natural settings by labeling the utterances and turns taken within an interaction using speech categories. The TDC was used to code child-caregiver and adult-adult conversations in a children's museum and during a public forum, respectively. Results supported hypotheses that predicted adult-adult interactions would contain more Declarative Statement and Acknowledgment utterances than child-caregiver interactions, while child-caregiver interactions contained fewer Conversational Turns, as well as more Command and Encouragement utterances. Results also indicated high levels of inter-rater reliability. The potential for additions and modifications to be applied to the standard TDC is discussed.

For a project on development of an Electronic Health Record (EHR) for stroke patients, medical information was organised in care information models (templates). All (medical) concepts in these templates need a unique code to make electronic information exchange between different EHR systems possible. When no unique code could be found in an existing coding system, a code was made up. In the study presented in this article we describe our search for unique codes in SNOMED CT to replace the self made codes. This to enhance interoperability by using standardized codes. We wanted to know for how many of the (self made) codes we could find a SNOMED CT code. Next to that we were interested in a possible difference between templates with individual concepts and concepts being part of (scientific) scales. Results of this study were that we could find a SNOMED CT code for 58% of the concepts. When we look at the concepts with a self made code, 54,9% of these codes could be replaced with a SNOMED CT code. A difference could be detected between templates with individual concepts and templates that represent a scientific scale or measurement instrument. For 68% of the individual concepts a SNOMED CT could be found. However, for the scientific scales only 26% of the concepts could get a SNOMED CT code. Although the percentage of SNOMED CT codes found is lower than expected, we still think SNOMED CT could be a useful coding system for the concepts necessary for the continuity of care for stroke patients, and the inclusion in Electronic Health Records. Partly this is due to the fact that SNOMED CT has the option to request unique codes for new concepts, and is currently working on scale representation.

A New Measure of the Expression of Shame: The Shame Code

While shame is essential for adaptive functioning, experiencing shame more often or intensely than others is strongly associated with psychopathology. To date, no measure of the behavioral expression of shame exists, despite the great potential for use in research and clinical settings. The present study aimed to assess the Shame Code, a new behavioral coding system of the expression of shame. Participants included 149 youth between the ages of 12 and 17 (50 % female, M = 14.5). Shame was elicited with a spontaneous speech task. Participants' overall Shame Code scores were correlated only with a state measure of shame, however, structural equation modeling results showed that Shame Code variables combined differentially to assess state and trait shame scores. A two-factor model was the best fit to the data. The first factor, Fidget, consisted of Hiding, Fidget (positively loaded), Nervous Positive, and Stillness (negatively loaded). The second factor, Freeze, was comprised of Stillness, Facial Tension, and Silence (positively loaded). The Fidget factor was associated with higher Trait Shame and the Freeze factor was associated with higher State Shame but lower Trait Shame. Therefore, the Shame Code not only effectively captured the behavioral manifestations of shame, but Shame Code variables also differentially predicted state and trait shame.

Video recording technology allows for the discovery of psychological phenomena that might otherwise go unnoticed. We focus here on gesture as an example of such a phenomenon. Gestures are movements of the hands or body that people spontaneously produce while speaking or thinking through a difficult problem. Despite their ubiquity, speakers are not always aware that they are gesturing, and listeners are not always aware that they are observing gesture. We review how video technology has facilitated major insights within the field of gesture research by allowing researchers to capture, quantify, and better understand these transient movements. We propose that gesture, which can be easily missed if it is not a researcher's focus, has the potential to affect thinking and learning in the people who produce it, as well as in the people who observe it, and that it can alter the communicative context of an experiment or social interaction. Finally, we discuss the challenges of using video technology to capture gesture in psychological studies, and we discuss opportunities and suggestions for making use of this rich source of information both within the field of developmental psychology and within the field of organizational psychology.

Assessing fidelity of treatment delivery in group and individual 12-step facilitation

Twelve step facilitation (TSF) is an emerging, empirically supported treatment, the study of which will be strengthened by rigorous fidelity assessment. This report describes the development, reliability and concurrent validity of the Twelve Step Facilitation Adherence Competence Empathy Scale (TSF ACES), a comprehensive fidelity rating scale for group and individual TSF treatment developed for the National Drug Abuse Treatment Clinical Trials Network study, Stimulant Abuser Groups to Engage in 12-Step. Independent raters used TSF ACES to rate treatment delivery fidelity of 966 (97% of total) TSF group and individual sessions. TSF ACES summary measures assessed therapist treatment adherence, competence, proscribed behaviors, empathy and overall session performance. TSF ACES showed fair to good overall reliability;weighted kappa coefficients for 59 co-rated sessions ranged from .31 to 1.00, with a mean of .69. Reliability ratings for session summary measures were good to excellent (.69-.91). Internal consistency for the instrument was variable (.47-.71). Relationships of the TSF ACES summary measures with each other, as well as relationships of the summary measures with a measure of therapeutic alliance provided support for concurrent and convergent validity. Implications and future directions for the use of TSF ACES in clinical trials and community treatment implementation are discussed. (C) 2013 Elsevier Inc. All rights reserved.

Background: CCAM is a French terminology for coding clinical procedures. CCAM is a multi-hierarchical structured classification for procedures used in France for reimbursement in health care, which is external to UMLS. Objective: The objective of this work is to describe a French lexical approach allowing mapping CCAM procedures to the UMLS Metathesaurus to achieve interoperability to multiple international terminologies. This approach used a preliminary step intended to take only the significant characters used to code CCAM corresponding to anatomical and actions axes. Results: According to the 7,926 CCAM codes used in this study, 5,212 possible matches (exact matching, single to multiple matching, partial matching) are found using the French CCAM to UMLS based mapping, 65% of the corresponding anatomical terms in the CCAM code are mapped to at least one UMLS Concept and 37% of the corresponding action terms in the CCAM code are mapped to at least one UMLS Concept. For all the exact matches found (n=200), 91% were rated by a human expert as narrower than the mapped UMLS Concepts, while only 3% were irrelevant.

The French coding system of surgical procedures, the Classification Commune des Actes Medicaux (CCAM), is used in France for DRG databases and fee for services payment. Mapping between CCAM and other clinical procedures terminologies by the means of UMLS metathesaurus is essential in order to increase semantic interoperability between different healthcare terminologies and between different case mix systems. In a previous work the CISMeF team used an automatic approach to map CCAM descriptors to the French part of the UMLS metathesaurus. In another way for the French funded research project InterSTIS, we performed a mapping using MetaMap based on the top level semantic structure descriptors of anatomy and action of CCAM translated from French to English. This paper aims to present this new approach and to compare the results with the previous one. The combination of both approaches significantly improved the coverage of the mapping to 68 % for both descriptors and 95 % for at least one descriptor.

Measuring knowledge co-construction: A comparison of high-inference observation, low-inference coding and self-report of transactive communication

Transactive communication means that learners refer to, build upon, and develop the contributions of their learning partners. Thereby, learners jointly construct knowledge about the learning topic. In research, transactive communication is measured predominantly by transcription and coding of audio or video material. This approach is relatively resource-intensive and may be limited in its validity due to a lack of information about learners' nonverbal behaviors and their engagement with the learning material. This study examines how to measure transactive communication as a key feature of knowledge co-construction. At two points of measurement, various forms of transactive communication were measured during ninth graders' partner work through self-report using a questionnaire (N = 516), low-inference coding of transcribed audio recordings (n = 246), and a high-inference observation procedure (n = 92). Additionally, knowledge acquisition and liking of the learning partner were assessed. Reliability and validity of the measurement methods were tested. For the high-inferential rating, observer agreement was moderate to excellent. For the coding of transactivity, interrater agreement was moderate to good. Factor analyses revealed a 1-factor solution of the forms of transactive communication for all three measurement methods. Transactivity measured by the coding of the transcripts correlated with the observation procedure, while both methods were unrelated to the self-report. Moreover, coding and self-report were not related to knowledge acquisition. In contrast, there was a moderate correlation between the high-inference rating of transactivity and knowledge acquisition. The results indicate that transactivity can be measured reliably and validly using the high-inference observation procedure.

To translate the 11th edition of the International Classification of Diseases (ICD-11) into French, we proposed a lexical approach using Natural Language Processing techniques. This method relies on the 56 biomedical terminologies and ontologies included in the Cross-lingual Health Multiple Terminologies and Ontologies Portal. From a sample of 336 ICD-11 terms, the algorithm translated 164 (49%) terms into at least one French term each.

The Motivational Interviewing Skill Code (MISC) is a coding system developed to measure adherence to motivational interviewing (MI). MI is an effective clinical style used in different treatment situations. Counsellors practising MI have to follow general principles and avoid certain traps. In the present study, the content of the MISC is compared with the general principles of MI and the traps to avoid in MI. Investigation of the content validity raises some questions. All general principles are represented but the traps to avoid in MI are not fully covered. The consequences of this under-representation are shown in transcripts of a selection of well-conducted MI training sessions. The reliability of the MISC was investigated by having five independent coders code 39 MI training sessions of different counsellors. The reliability of the MISC is reasonable. The five coders agreed to a large extent on the absolute ratings but the intraclass correlations were low. Although the MISC can be a useful research tool for process research of MI, it remains a labour-intensive instrument and for teaching and practice audit development of a more simple coding system is recommended.
