The effects of online pilates training on cognitive functions and dual task performance in people with multiple sclerosis: A randomized controlled study

Background: cognitive impairments in people with multiple sclerosis (PwMS) are common and aggravate dualtask (DT) performance. This study aimed to investigate the effectiveness of online Pilates training on cognitive functions and the DT performance of balance, walking, and functional mobility in PwMS. Method: In this randomized controlled trial, forty-six PwMS were recruited and randomly allocated into two groups. The Online Pilates group (OPG) received Pilates training via videoconferences three days per week during six weeks at home. The control group (CG) was a waitlist with no Pilates treatment. cognitive functions were assessed with Montreal cognitive Asssessment (MoCA), The Trail Making Test (TMT), and The Stroop Test (ST). DT performance was assessed with mental tracking and verbal fluency during postural stability (PS), walking, and functional mobility. Results: At the end of six weeks, the interaction effects (time x group) revealed significant differences for MoCA (p = 0.024), TMT-B (p = 0.012), ST (p <= 0.012), PS-with mental tracking (p = 0.004), PS-with verbal fluency (p = 0.003), cadence-with mental tracking (p = 0.048), and functional mobility-with mental tracking (p = 0.043) in favor of the OPG. Additionally, MoCA, TMT, ST, and DT performance during balance and functional mobility significantly improved in after six-week online Pilates training, but did not in CG. Conclusion: Pilates training via videoconference was effective in the improvement of cognitive functions and DT performance in PwMS. Online Pilates may be considered for improving cognitive impairments and DT performances in PwMS with barriers to accessing clinics. Trial registration: ***: NCT06462339

Background: cognitive dysfunction is prevalent but inadequately treated in people with multiple sclerosis (PwMS). Central vestibular functions are associated with multiple cognitive domains in PwMS, even when controlling for disability status. To our knowledge, the effectiveness of vestibular training on cognition in PwMS has never been explored. The study's purpose was to determine the effect of vestibular training on PwMS's cognitive functions. Methods: Forty PwMS with relapsing-remitting MS, primarily fatigued, were randomly divided into two equal groups. The control group (GI) was treated with aerobic endurance exercises using stationary bicycle training. The study group (GII) was treated with aerobic endurance exercises as GI in addition to a designed vestibular training program. Outcome measures of cognition were assessed preand post-treatment using the Arabic version of Brief International cognitive Assessment for MS (BICAMS-A) and serum Brain-derived neurotrophic factor (BDNF). Results: Post-treatment, there was a significant increase of oral Symbol Digit Modalities Test (SDMT), California Verbal Learning Test II (CVLT-II), revised Brief Visuospatial retention Test (BVRT-R) of BICAMS-A and serum BDNF in both study and control groups. There was a significant increase in CVLT-II, BVRT-R of BICAMS-A and serum BDNF in the study group compared to the control group (P < 0.05). Conclusion: Vestibular training is an effective and innovative approach for improving cognitive functions in PwMS. Trial registration: PACTR202311670782148.

Gender differences in like-sex middle-aged twins: an extended network analysis of depressive symptoms, cognitive functions and leisure activities

Background Depression affects twice as many women as men. Risk factors for depression certainly impact this difference, but their strong interconnectedness challenges the assessment of standalone contributions. Network models allow the identification of systematic independent relationships between individual symptoms and risk factors. This study aimed to evaluate whether the extended networks of depressive symptoms, cognitive functions, and leisure activities in like-sex twins differ depending on gender or *** Twins, including 2,040 women (918 monozygotic and 1,122 dizygotic) and 1,712 men (730 monozygotic and 982 dizygotic), were selected from the Danish Twin Registry for having, along with their like-sex co-twin, completed measures of depressive symptoms, cognition, and leisure activities (physical, intellectual, and social). Network models were estimated and compared at three levels: co-twins to each other within groups defined by gender and zygosity;monozygotic to dizygotic twins within the same gender, and between *** No significant differences were observed when co-twins were compared to each other, regardless of the pair's zygosity or gender, nor when monozygotic twins were compared to dizygotic twins within gender. However, the gendered networks differed significantly in global strength, structure, and partial correlations between specific depressive symptoms and risk factors, all indicating stronger connectedness in women relative to *** Environmental factors appear to best explain between-gender network differences. Women's networks showed significantly stronger associations both among depressive symptoms and between depressive symptoms and risk factors (i.e., decreased cognition and leisure activities). Longitudinal research is needed to determine the causality and directionality of these relationships.

Background: The network theory posits that depression emerges as the result of individual symptoms triggering each other. Risk factors for depression can impact these between-symptoms interactions through extended networks. The study aimed to model the extended network of depressive symptoms and known depression risk factors - objective cognitive function, intellectual, physical, and social daily activities, and then, compare the observed networks between monozygotic (MZ) and dizygotic (DZ) co-twins. Methods: Twin pairs, 722 MZ and 2200 DZ, aged 40-79, were selected from the Dansh Twin Registry for having complete measures of depressive symptoms (e.g., sadness), cognitive functions (e.g., verbal memory), physical (e. g., brisk walk), intellectual (e.g., reading newspapers) and social activities (e.g., phone calls). Gaussian graphical models were used to estimate and compare the networks first between co-twins and then, between MZ to DZ twin pairs separately. Results: Specific intellectual, physical and social activities were central in the extended networks of depressive symptoms and, with the exception of processing speed, more central than cognition. The extended networks' structure was more homogeneous between MZ co-twins relative to DZ co-twins. cognitive nodes were more central in MZ than DZ co-twins. Limitations: Cross-sectional design, participants were middle-aged or older, mostly affective (non-somatic) depressive symptoms. Conclusions: In depression networks, core connecting elements were intellectual, physical and social activities. The interaction between cognition and daily activities seems critical for triggering depressive symptoms. Thus, clinical interventions aimed at preventing depression and associated cognitive deficits should focus on maintenance and/or engagement in stimulating daily activities.

BackgroundPhysical activity (PA) interventions have been shown to yield positive effects on cognitive functions. However, it is unclear which type of PA intervention is the most effective in children and adolescents with Neurodevelopmental Disorders (NDDs). This study aimed to compare the effectiveness of different types of PA interventions on cognitive functions in children and adolescents with NDDs, with additional analyses examining intervention effects across specific NDD types including attention-deficit/hyperactivity disorder (ADHD) and autism spectrum disorder (ASD).MethodsIn this systematic review and network meta-analysis, seven databases (Web of Science, PubMed, Medline, APA PsycINFO, Embase, CINAHL, and SPORTDiscus) for randomized controlled trials from database inception to September 2023 were searched. Randomized controlled trials comparing the effectiveness of PA intervention with any non-pharmacological treatment or control group on cognitive functions in children and adolescents diagnosed with NDDs aged 5-17 years were included. Frequentist network meta-analyses were performed based on standardized mean differences (SMD) using random effects models to examine post-intervention differences in cognitive functions, including attention, memory, and executive functions. Intervention dropout was assessed as a measure of treatment ***-one randomized controlled trials (n = 1,403, mean age 10.0 +/- 1.9 years) with 66 arms were included in the network. Mind-body exercise (MBE;SMD = 1.91 for attention;0.92 for executive functions), exergaming (SMD = 1.58 for attention;0.97 for memory;0.94 for executive functions), and multi-component physical activity (MPA;SMD = 0.79 for executive functions) were associated with moderate to substantial cognitive improvements compared with usual care, whereas the effectiveness of aerobic exercise (AE) was non-significant. Exergaming (SMD = 0.78, 95%CI 0.12 to 1.45) and MPA (SMD = 0.64, 95%CI 0.11 to 1.18)

Background: cognitive impairment, brain fog, depressive symptoms, and sleep disturbance are prevalent symptoms among individuals with long COVID, for which there currently is no treatment. Aim: To assess the effectiveness of an eight-week cardiopulmonary rehabilitation programme on cognition, psychological well-being, and sleep quality in individuals with long COVID-19. Methods: Forty participants diagnosed with long COVID-19 (26 female, 14 male participants;mean age 53 +/- 11 years) were randomly assigned to a rehabilitation group or to a control group. The control group maintained their regular daily habits, while the rehabilitation group was prescribed an individualized clinical programme consisting of three sessions per week. Each session involved light to moderate aerobic exercise, resistance training, and respiratory exercises. Neuropsychological tests evaluating executive function, memory, and processing speed were administered at baseline and at study completion. Participants also completed questionnaires on their psychological state and sleep quality at both time points. Results: No difference between groups was observed for neuropsychological test performance, however significant group differences in perceived stress and depression were found post-intervention. Mild Behavioral Impairment and cognitive functions: A systematic review and meta-analysis

Mild behavioral impairment (MBI) represents a recently introduced diagnostic concept that focuses on behavioral and personality changes occurring in late life and associated with cognitive decline. Nevertheless, the relationship between these dimensions remains unclear. This systematic review and meta-analysis aim to analyze the relationship between MBI and cognitive functioning. The review process was conducted according to the PRISMA-Statement. Restrictions were made, selecting the studies published in peer-review journals, including at least one cognitive measure and presenting the measurement of MBI. Studies that included participants with neurological disorders, dementia, or psychiatric disorders or that only did a neuroimaging or genetic study were excluded. Twenty-two studies were included in the systematic review, while in the meta-analysis seventeen studies featured data to be included in the analyses. The results were classified according to the following cognitive domains: global cognitive functioning, memory, language, attention executive functions, visuospatial skills, and processing speed. In the quantitative analysis, only global cognitive functioning, executive function, attention, and memory were evaluated. The results of both qualitative and quantitative analysis indicate that individuals with MBI exhibited diminished performance on cognitive tasks when compared to those without MBI symptoms. These results are stronger when evaluating the various domains individually (particularly memory and executive functions) than when a global assessment was made. These findings highlight the potential role of MBI symptoms as early indicators of neurodegenerative processes, reinforcing the necessity for comprehensive assessments that encompass both behavioral and cognitive evaluations. The early detection of these symptoms in prodromal phases can be very useful for the development of non-pharmacological interventions and may provide relevant guidelines for clin

Exploring the associations between psychiatric symptoms and cognitive functions in first-episode schizophrenia: A network analysis

Background: First-episode schizophrenia represents a critical period for intervention in the treatment of schizophrenia. Understanding the intricate relationships between psychiatric symptoms and cognitive functions is vital for early precise intervention and predicting illness outcomes. Previous research has largely overlooked this issue, and traditional analytical methods based on pre-established theoretical assumptions are insufficient. This study aims to address this gap by utilizing graph theory-based network analysis. Methods: The study employed the Positive and Negative Syndrome Scale (PANSS) to assess psychiatric symptoms. cognitive functions were evaluated using the Digit Symbol and Information subtests from the Wechsler Adult Intelligence Scale (WAIS), which measure information processing efficiency and general knowledge, respectively. A network of psychiatric symptoms and cognitive functions was constructed based on these assessments. Results: The network analysis revealed that negative symptom nodes were central. Notably, node N1 (Blunted affect) showed a negative correlation with the Digit Symbol node, being the only psychiatric symptom node linked to cognitive functions. Community detection analysis indicated that cognitive, positive symptom, and negative symptom nodes tended to cluster within their respective categories, while general psychopathology nodes showed a tendency to cluster with various types of nodes. Some general psychopathology nodes were isolated, reflecting the concealed nature of certain psychiatric symptoms in first-episode schizophrenia patients. Conclusion: This study innovatively applies network analysis to explore the characteristics of the psychiatric symptom-cognitive function network in Chinese patients with first-episode schizophrenia. The findings provide valuable theoretical insights for targeted symptom-based interventions and for predicting disease outcomes in first-episode schizophrenia.

The progressive number of old adults with cognitive impairment worldwide and the lack of effective pharmacologic therapies require the development of non-pharmacologic strategies. The photobiomodulation (PBM) is a promising method in prevention of early or mild age-related cognitive impairments. However, it remains unclear the efficacy of PBM for old patients with significant age-related cognitive dysfunction. In our study on male mice, we show a gradual increase in the brain amyloid beta (A beta) levels and a decrease in brain drainage with age, which, however, is associated with a decline in cognitive function only in old (24 months of age) mice but not in middle-aged (12 months of age) and young (3 month of age) animals. These age-related features are accompanied by the development of hyperplasia of the meningeal lymphatic vessels (MLVs) in old mice underlying the decrease in brain drainage. PBM improves cognitive training exercises and A beta clearance only in young and middle-aged mice, while old animals are not sensitive to PBM. These results clearly demonstrate that the PBM effects on cognitive function are correlated with age-mediated changes in the MLV network and may be effective if the MLV function is preserved. These findings expand fundamental knowledge about age differences in the effectiveness of PBM for improvement of cognitive functions and A beta clearance as well as about the lymphatic mechanisms responsible for age decline in sensitivity to the therapeutic PBM effects.

Effect of Trans-Auricular Vagal Nerve Stimulation on cognitive functions in Diabetic Polyneuropathy Patients

Background and Purpose: Diabetic polyneuropathy is associated with poor cognitive performance in executive function, memory and reaction time. Patients had a 50% greater risk of developing dementia. Trans-auricular vagal nerve stimulation (ta-VNS) is a new, safe and promising instrument for enhancing cognitive performance. This study aimed to investigate the effect of trans-auricular vagal nerve stimulation (ta-VNS) on cognitive functions and its impact on quality of life (QOL) in diabetic polyneuropathy patients. Methods: Forty individuals with type II diabetic polyneuropathy participated in this study. They were randomly assigned to two equal parallel groups: study group (GI) and control group (GII). The study group received active trans-auricular vagal nerve stimulation (ta-VNS) in addition to a computer-based cognitive rehabilitation (CBCR) program, while the control group received the same program with sham (ta-VNS). Both groups underwent intervention three times per week for 3 weeks. cognitive functions (attention-concentration, and figural memory) and quality of life were assessed at baseline and after 3 weeks using the RehaCom system and the World Health Organization's Quality of Life Brief Questionnaire, respectively. Results: Post-treatment, there was a significant increase in the level of attention-concentration, as well as a significant increase in the quality-of-life score. Additionally, there was a significant decrease in the mean time for attention-concentration, as well as in figural memory acquisition time for both groups, with more improvement in favor of the study group (p < 0.05). Furthermore, there was a significant increase in the task performance and level of figural memory, along with a significant decrease in figural memory solution time only in the study group (p < 0.05).
