Core content of pediatric specialty training Prioritization using the delphi method

Background With the help of the professional societies, the German Medical Association has drawn up new competence-based (model) specialty training regulations. The evaluation of pediatric specialty training has not yet been the subject of studies in Germany. methods In a two-stage delphi procedure, specialists in pediatric and adolescent medicine and doctors in training assessed the contents of the specialty training on a four-stage scale. For this purpose, a total of 555 items were generated from the logbook of the German Academy of Pediatrics and Adolescent Medicine as well as from learning target catalogues of German-speaking universities and corporations. These were sent with the email distributors of the German Society for Pediatrics and Adolescent Medicine and the Professional Association of Pediatricians in Germany. The rating on the Likert scale was graded from "extremely important" = 4 to "unimportant" = 1. An item with a mean value of >= 3.0 became a core content and items with a value of Results In the first round of the two-stage procedure, 7800 questionnaire emails were sent out. A total of 259 responses were collected, 135 of which were completely filled in. In the second round, 11,786 questionnaire emails were sent, 903 replies were received with 572 completed questionnaires and 446 items were identified as core content. The items with the highest priority were the management of frequent emergency situations, the recognition of child abuse and the practice of vaccination. A comparison of the specialists in hospitals (n = 344) and private practices (n = 526) showed a significantly different evaluation in 20 out of 31 topics. Conclusion This study, the first of its kind in Germany, scientifically investigated which contents of specialty training in the subject of pediatric and adolescent medicine can be regarded as core contents. The results show room for a significant reduction in the total amount of training objectives.

Development and Validation of Content Domains for Paramedic Prehospital Performance Assessment: A Focus Group and delphi method Approach

Background: The chaotic and complex nature of delivering patient care in the prehospital setting complicates the provision of real-time formative feedback to paramedic students. Although the use of simulations is widespread in emergency medical services (EMS) education, a high degree of variability precludes consistent performance assessment in EMS. Objectives: The objective of this study was to define and validate key domains required to evaluate paramedic prehospital performance. methods: We conducted a two-phase study that combined focus group and delphi methodology. Participants were purposefully selected to attain diverse panels regarding sex, race, ethnicity, professional roles, levels of education, geographical area, and experience as a paramedic and educator. In Phase I, a panel of 11 subject matter experts (SMEs) were tasked with identifying the essential domains to be evaluated in a paramedic performance assessment. In Phase II, another panel of 11 SMEs and a four-round modified delphi method with 28 paramedic program directors were used to validate the domains identified in Phase I. Results: The first focus group identified and achieved consensus on five domains: (1) effective communication, (2) scene management, (3) patient assessment, (4) patient management, and (5) professional behavior. These domains were validated by the second focus group. The first round of the delphi process generated 64 content domains, which were reduced to nine unique content domains via thematic analysis. These nine content domains fit well within the broader domains identified by the focus groups with one specific area, critical thinking and reasoning, being listed in two key areas based on the definitions of the focus group domains of patient assessment and patient management. Conclusion: The content domains identified in this study provide EMS educators a theoretical framework for designing the performance assessment of newly trained professionals in the prehospital setting

Background: Spinal Muscular Atrophy (SMA) is a rare disease that affects 1 in 11 000 live births. Recent developments in SMA treatments have included new disease-modifying therapies that require high quality data to inform decisions around initiation and continuation of therapy. In Canada, there are no nationally agreed upon outcome measures (OM) used in adult SMA. Standardization of OM is essential to obtain high quality data that is comparable among neuromuscular clinics. Objective: To develop a recommended toolkit and timing of OM for assessment of adults with SMA. methods: A modified delphi method consisting of 2 virtual voting rounds followed by a virtual conference was utilized with a panel of expert clinicians treating adult SMA across Canada. Results: A consensus-derived toolkit of 8 OMwas developed across three domains of function, with an additional 3 optional measures. Optimal assessment frequency is 12 months for most patients regardless of therapeutic access, while patients in their first year of receiving disease-modifying therapy should be assessed more frequently. Conclusions: The implementation of the consensus-derived OM toolkit will improve monitoring and assessment of adult SMA patients, and enrich the quality of real-world evidence. Regular updates to the toolkit must be considered as new evidence becomes available.

Developing indicators for sustainable urban regeneration in historic urban areas: delphi method and Analytic Hierarchy Process (AHP)

This paper presents a comprehensive evaluation model comprising indicators spanning four dimensions: social, economic, environmental, and governance. Extensive indicators were gathered from the literature and subsequently subjected to a rigorous screening, validation, and finalization process by a panel of 25 international experts through a delphi survey. The relative weights assigned to each indicator were determined utilizing the Analytic Hierarchy Process (AHP). The findings reveal that the social dimension carries the greatest significance in achieving sustainable urban renewal (SUR), closely followed by the environmental dimension. In terms of assessment categories, the highest priority and weight (0.1186) were attributed to "building and land use," while "economic viability" (weight=0.1049), "policymaking" (weight=0.0982), "community involvement" (weight=0.0882), and "housing provision" (weight=0.850) were also identified as significant areas. The paper emphasizes that these proposed indicators can serve as valuable tools to inform the review and enhancement of urban planning guidelines and policies at the local, state, and national levels, fostering sustainable urban regeneration.

Analytic hierarchy process and delphi method were commonly used to construct index system for com-prehensive evaluation. The combination of the two different methods was helpful to solve the problems of unreasonable selection of elements, unclear meaning of elements, unsystematic and incorrect relation-ship between elements, unreliable quality of evaluation results and so on. The above methods were applied to build the evaluation index of materials and equipment of emergency medical rescue team (EMTs), which ensured that the evaluation index system met the requirements of systematicness, stan-dardization and credibility, and could help to timely find and correct the problems existing in materials and equipment of disaster EMTs. Assessing the Impact of Data Sciences and Smart Technologies in Air Conditioning Project Management: A delphi method Analysis within the Construction Industry

The integration of data sciences and smart technologies in the construction industry, particularly in air conditioning project management, is an important area of research. This study employs the delphi method to explore this integration, surveying 40 experts in construction management. Participants were meticulously selected based on a set of inclusion criteria related to age, educational qualifications, and field experience. The study tests five hypotheses, each scrutinized through a score-based delphi analysis. The findings are mixed and shed new light on several dimensions of air conditioning project management. For instance, the study refutes the commonly held belief that the location of air conditioning projects significantly impacts worker safety. It also challenges the assumption that exceeding international safety standards like ISO leads to cost savings. On the other hand, the study validates the significance of global safety standards and the undeniable role of data sciences and smart technologies in enhancing human safety in the air conditioning industry. These findings not only challenge existing models, but also open avenues for further research. They indicate a complex interplay between safety, cost-effectiveness, and technological integration in air conditioning project management. The study suggests a shift towards data-driven decision-making and underscores the need for international safety standards, particularly in an era marked by rapid technological advancements and globalization.

As an important component of road traffic facilities, bridges play a crucial role in daily traffic operations, and changes in their status can have an impact on traffic operation. The existing research mainly focuses on monitoring the status of bridges themselves or analyzing the operation status of road traffic, and rarely considers the changes in traffic operation status caused by changes in bridge status. Therefore, in order to evaluate the impact relationship between the two, this article designs an algorithm that combines the Analytic Hierarchy Process (AHP), the delphi method, and Singular Value Decomposition (SVD) based on the traditional evaluation of bridges and road traffic operation status, and establishes a bridge traffic operation status impact assessment model. Then, simulation analysis and actual data verification will be conducted based on the specific situation of Ma'anshi Bridge on the Chongqing Wuhan Expressway. The experimental results show that the evaluation model established in this paper conforms to the characteristics of traffic operation, can reflect the impact of bridge state changes on traffic operation status well, effectively promote the automation level of bridge traffic impact management, and has high reliability and accuracy.

Dimethyl fumarate (DMF) is a widely used oral disease-modifying therapy for multiple sclerosis (MS). Its efficacy and safety profiles are supported by over a decade of experience. Differences exist between Asia and Europe/United States in the prevalence and characteristics of MS;most data for DMF are derived from populations outside Asia. DMF was recently (2021) approved for use in China. The objectives of this review were to evaluate the evidence for DMF's profile, to provide an update to healthcare providers on current knowledge surrounding its use and to assess the relevance of existing data to use in China. This study used a modified delphi method based on the insights of a scientific Steering Committee (SC), with a structured literature review conducted to assess the data of DMF. The literature review covered all papers in English (from 01 January 2011 to 21 February 2022) that include 'dimethyl fumarate' and 'multiple sclerosis', and their MeSH terms, on PubMed, supplemented by EMBASE and Citeline searches. Papers were categorized by topic and assessed for relevance and quality, before being used to formulate statements summarizing the literature on each subject. SC members voted on/revised statements, requiring & GT;80% agreement and & LE;10% disagreement for inclusion. Statements not reaching this level were discussed further until agreement was reached or until there was agreement to remove the statement. A total of 1030 papers were retrieved and used to formulate the statements and evidence summaries considered by the SC members. A total of 45 statements were agreed by the SC members. The findings support the positive efficacy and safety profile of DMF in treating patients with MS. Limited Chinese patient data are an ongoing consideration;however, based on current evidence, the statements are considered applicable to both the global and Chinese populations. Study on skin infection model of Staphylococcus aureus based on analytic hierarchy process and delphi method

Purpose: Infectious skin diseases are a type of inflammatory skin lesions caused by pathogenic microorganisms. Because of the uncertainty of methodology, the skin infection model usually have low replication rate and lack of good evaluation system. We aimed to establish multi-index and comprehensive evaluation method for Staphylococcus aureus (***) skin-infection models through Analytic hierarchy process (AHP) and delphi method, and screen high quality animal models through *** and methods: Firstly, the evaluation indicators of skin infection were collected basing on literature research. The weight of the evaluation indicators were decided according to AHP and delphi method. Then different ulcer models (mouse or rat) infected by S. aureus were selected as the research ***: The evaluation indicators were classified into four groups of criteria (including ten subindicators) and given different weights, physical sign changes (0.0518), skin lesion appearance (0.2934), morphological observation (0.3184), etiological examination (0.3364). Through the evaluation system, we screened and found that the mouse ulcer model which caused by a round wound and 1.0 x 1010 CFU/mL (0.1 mL) bacterial concentration got the highest comprehensive score, and also found that the model which caused by a 1.5 cm-round wound and 1.0 x 1010 CFU/ mL (0.2 mL) maybe the best rat ulcer ***: This study has established an evaluation system based on AHP and delphi method, also provided the best skin ulcer models selected by this system, the models are suitable for disease research and drug development research of skin ulcer.

Utilizing the delphi method to Assess Issues of Sexuality for Men Living with Spinal Cord Injury

This study investigated the physical and psychological issues of sexuality for men after spinal cord injury and identified areas of top concern. Using the delphi method, a panel of expert judges (twenty men with spinal cord injury [SCI], C3-T12, age 26-59) completed a series of three questionnaires in an attempt to reach a consensus on previously identified issues. It was agreed that SCI affects physical and psychosocial aspects of sexuality and that sexual support is limited for exploring sexuality post-SCI. Additionally, insight into the social constructs of masculinity, gender roles, body image and disability and their influence on the sexual experiences of this population is provided. Enhancing patient, healthcare provider and societal awareness of sexuality for men after spinal cord injury should be an immediate area of focus to improve sexual and overall quality of life for this population.
